If I was to take a guess at the inspiration and explanation behind Madame Nubescu's very... dated depiction, like you surmised, I'd source it back to their WoW roots where the Troll race is VERY coded to those kinds of cultures with things such as voodoo practices and even the stereotypical accent. Of course there it's a bit more excusable since they're distinctly another species and people entirely with their own culture. Certainly dated at times still, don't get me wrong, but they've certainly tried making it better from that infancy.
The problem here is when translating it to a distinctly human character the racial undertones come out WAY more, since you no longer have the excuse of it being inspired by a culture, it just is your depiction of that culture.
Of course also compounded by their just blatant ignorance of the world at the time that was kind of the norm at the time, especially this side of the pond unfortunately...
(For what it's worth, from the writing standpoint it could've been much MUCH worse though, Nubescu could've always ended up some evil witch doctor trope or something. At least she's distinctly depicted as a good character and a good person.)
And from what I've seen, she was played by Sjin, which may explain a lot...
