You don't necessarily need to remove the guts, if you do it that way. They're only after the fillets or flesh of the fish, so the guts isn't important. You wouldn't even smell it ir it, being contaminating the flesh. This method is as old as time itself. The massive amount of salt that applied over the scales of the fish act as seasoning, as well as protection against charring. In case you're wondering. I had the same question as you, when I first tried it back in 2001, then realized it's actually tasty.
