The "Sisters of Karn" first appeared in 1976's story The Brain of Morbius. It would be interesting to see your reaction to that story. I don't think you need any backstory to it
Spoilers to Classic Who below The Sisterhood of Karn are keepers of the flame of eternal life. In classic who, The Doctor learned that they are an offshoot of the Time Lords, though they took a more monastic lifestyle. The flame they guard produces a liquid they can use to control regeneration without limitation. Meaning a time lord with access to the flame could in theory regenerate forever and into whomever they desired to be.
