If you been going to your doctor, continued your treatment and taking your meds then you should be ok. I was approved in 2005 and just got my first CDR last yr, filled the forms out, sent them back and within 3-4 months received a letter saying they found my disability is continuing and I'll hear from them from time to time. I didn't have to talk to nobody, send no extra information in or go to the disability doc for further evaluation. So just hang in there.

They asked about my daily activities and how my disability affect them, last dr/hospital visits/surgeries. I'm 41 and suffer from a rare disease called stickler syndrome and has had 52 surgeries but only getting worse. The information you sent really should be enough. Like they say in these videos they want to make sure your disability is continuing and it sounds like it is. My mom and auntie got the short form and did what you did and that was enough. If it's not enough they might just want the medical records to back up your claims if that makes any sense.

My daughter was awarded for an iq of 50. From there not much doctors she needed to see. What if I have no records because of the nature of her disibility (intellect type)?
