IMO Moldaver had a vault of her own with a cryogenic suspension pod. (like the ones on level twelve) That is how she survived, and given that she appears older than she did in the past, she must have come out several years earlier. The current vault four dwellers are not conducting experiments they are the descendants of the original vaults test subjects that rebelled and took over. They are now caring for the remaining test subjects. Lucys line about what her father would think about her destroying a community, keep that in mind when you watch the finale.. How does an elixir fix a foot? Game logic that's how. Instant healing is possible in fallout. IMO Betty poisoned the prisoners their usefulness as a distraction was no longer needed. Steph is from 31 that is why she is overseer. There are no cameras outside the vault so no way to see who is at the door.
The dad isn't there and I would say 90% of your questions will be answered. Especially what is in Vault 31. As a nice thing for gamers, the way how Lucy's brother hacked into the computer is the same as in the game :) (It is kind of a hangman game, where you select terms that pop up in the code)

I see you fell for The Trap of thinking the current Vault 4 people were the bad guys. Everyone does. ☺ They're so good at deceiving you, that many people keep thinking they're the bad guys, even after the reveal. Weird? Yep. Bad? No. Did you notice the whole scene with the supplies is a gameshow? The woman putting them on the stage even pranced off like one of the girls that used to show new cars and washing machines on Wheel of Fortune and those types of shows. 😆 Edit: The Ghoul actually isn't after the head any more. He's after Moldaver for....reasons, and he knows where she is. He thinks that's where the head is going, because that's what Lucy told him.
