Noone actually dies. Not even the memories of the paralel worlds as everything other than Makinohara meeting with Sakura happened in the third timeline just like in the first. The young Makinohara remembers everything that the old in did and based on the ending Sakura remembers too so even the first/second timeline meeting of middle school Sakura and Makinohara exist as memories in the third timeline.
Guide to timlines:
1st timeline where Mai san dies
2nd where the timetraveller Sakura "dies"
3rd where Makinohara's heart is from a stranger.

If only Sakuta rejected Shoko from entering in his life again things wouldn't get ugly. Yes, Shoko was the person who helped Sakuta in his roughest time and the first person who he loved but it's just not right because he is already in love with Mai and is in a relationship.
But I guess that's how they want the movie to go. Just my opinion :,)

You highlight more about Mai and Makinohara here because they were too thoughtful to Sakuta, but Rio Futaba may have not been on Sakuta's side but she was really thinking about him all the time, she was worried when she heard that Sakuta is about to get involved in an accident, she gave him a place to stay for a meantime, and at Christmas Eve, the day where Sakuta is about to die, she was at her lab trying to solve how to alter events from the past, so that after Christmas Eve, she'll go back, and might be able to get a hold of Sakuta before he even gets at the accident, she may not put her life on the line for Sakuta, but she risked her friendship with Sakuta, she knew that Sakuta would get angry because Makinohara would die if noone else died.
