So I do wanna offer a slight correction: It's not necessarily a mafia thing, but rather a gang thing. Deuce is basically a former yankii (delinquent) and yankii are often associated with biker gangs in Japan. Gangs have turf wars and all that jazz, but not all gangs are connected to or affiliated with the yakuza.

I think it was more like a collaboration. Deuce's biggest wish was for him to became a honor student, and his most important friends are Yuu, Grim, and Ace, and also everyone in Heartslabyul. Malleus only created the realm and amplify those already existed desires. For everything else, it was filled and created by Deuce's imagination himself. See, the 810 book he was holding were full of blanks because the real Deuce didn't even remember all of them, but he was still having the thought that he was a honor student. His wish for being a honor student was still granted, though, and that was enough for Malleus. Malleus didn't actually give them happiness--he just assume that granting people's biggest ambitions/desire and/or erase their most painful memories they want to forget are one way for them to achieve happiness, and that's why he called it 'a happy dream'. It's not wrong, but the boy truly need to learn that happiness and unhappiness are one package deal.
