TROUBLESHOOTING VENTILATOR CRISIS | MEDICAL LECTURES | DR ANUPAM MOHAPATRA
We discuss how to troubleshoot high peak inspiratory pressure in mechanically ventilated patients. In the beginning , we discussed briefly, resistance and compliance.
How to use peak pressure and plateau pressure to monitor for changes in airway resistance and lung compliance. We have discussed equation of motion, and sequentially, monitored different components like, resistance, flow, tv, compliance, peep etc. Then by applying inspiratory pause, we segregated, peak pressure or driving pressure and plateau pressure. we discussed Poiseuille's Law, laminar and turbulent flow, how diameter of endotracheal tube affect the resistance and pressure in airway. viscosity of gas, secretion, clot and kink. factors affecting compliance etc..
In the video I attempt to connect the dots between peak inspiratory pressure and plateau pressure and how these affect dynamic and static compliance. How to identify if you have an airway resistance problem and what diseases you should consider. Also, how to identify a pure alveolar compliance problem.
we also discussed low pressure or low mv alarm, high mv alarm, apnea alarm..
