Yoda was largely at fault because he didn’t allow the order to change, to adapt to the times, and to follow the will of corrupt politicians instead of following the will of the force.
Surely one of them could have read the mind of a clone and found the programming hidden deep in their consciousness. It would have been quite simple for a Jedi Master, yet no one ever thought to do it during the entire duration of the Clone War.
No no, if you take it as canon, the problem is they were presented with evidence multiple times that there was a plot against them and they just swept it aside. Evidence stems from star wars the clone wars. A malfunctioning inhibitor chip and reopening the investigation into master Sifo Dyas.
