I was diagnosed with hip arthritis six years ago when I was just 50 years old. Ever since I've had to use painkillers every time I'm out of the house for more than half an hour maximum. Some weeks ago, I happened to watch one of your videos and started contracting my backside muscles the way you explain, as many times a day as I can, sitting or lying down. Unfortunately, for some reason, I can't do it when I walk or when I climb stairs. I'm not saying that I don't need any paracetamol any more, but I definitely see a difference in the pain level and my walking stamina. (I suppose after 30 years of office work my muscles there went totally flat and I developed arthritis in my right hip.) I can't thank you enough! I'll continue working on it. Greetings from Greece.
Hello Alicemi, It sounds like you are on the right track!!!!! If you had a significant amount of weakness prior to starting to contract your backside muscles then you may need to continue int he lying down position before you transition to standing and over activities. You are on the right track , keep it up!!

Hey Ogirima, Thank you for reaching out! We are sorry to hear that you are suffering from hip arthritis.🥺 Cycling can be helpful if you are activating the right muscles. You'll want to feel the glutes working when you are cycling. You'll also feel activation in the hamstrings and quads but you should feel it mostly in the glutes. My sister recommended I subscribe to your channel. My Dr said my hip X-rays show moderate osteoarthritis. She sent me to a physical therapist. He recommended some exercises and to ride the recumbent bike. I have super strong quads and no glutes. Is the recumbent bike going to increase my quads and so more harm?
I'm almost 57 and my hips especially my right one have gotten so bad to the point where I can barely walk at times, I'm also unsteady on that hip and it feels like it wants to collapse underneath me. And then I came across your video doing a Google search on bursitis because I've been diagnosed with that as well as arthritis and my doctor gave me an injection in my right bursa and it helped for about a day and now it's back to hurting me again although not quite as bad. In a roundabout way I'm telling you that I am going to be watching your videos like my mobility depends on it because I think it may. I appreciate you being very detailed in your explanations and I would ask that you continue to do so. The funny thing is is my hip started bothering me when I lived in El Paso LOL, now I live in Oklahoma and unfortunately I cannot go see you. And I have one of those massage gun things and so I watched your video first about how to massage your bursa area and my hip feels better just using the massage gun, I do not want to be forced to use a cane or a walker or a wheelchair. They gave me some exercises to do to stretch out and work my muscles and things connected to my bursa. But with the arthritis on my inner hip those are almost impossible for me to do without a lot of pain. So here I am binge watching you LOL to try to get myself mobile again. Because I really want to feel better. So please any videos you can come up with on how to get rid of the arthritis and bursitis pain in a person's hip would be much appreciated. Again thank you
Without surgical intervention the only relief from arthritis is exercise. The muscles must be strong and stable around the joint. Weak musculature causes imbalances. The body is a kinetic chain. If hamstrings are tight and weak you’ll experience low back pain. Because the hamstrings insert into the lower back. I just recently had a hip replacement. I’ve been incredibly active my entire life. Avoid the surgery at all costs. Because the pain is inexplicable. No one or idea can prepare you for the level 10+ pain that lasts a solid month. I’m at month 3 of recovery. I can walk fine but it’s still tender and painful to the touch. My physical therapist calls an arthroplasty brutal. It is. Lotsa saws and hammers for that procedure.
I have had an extensive labrum tear with cartilage loss which was repaired in 2018. I have done so much research on this topic. I cannot begin to tell you how much your videos have helped me. Never did I realize the root problem was weak glutes. I am struggling to activate my glutes as you have shown in the video. I will work to isolate my glute muscles and continue to strengthen them. I am definitely quad dominant with tight hip flexors along with bursitis and outer knee pain on the affected side. I need to fix this imbalance on the affected side while also decreasing the chances of this same issue happening on my other hip. Thank you 🙏, thank you 🙏 so much for this information you provide. It is invaluable.
OMG!! Thank you for this video! I have pain from back, down my leg, through the knee, down the shin to ankle. And I became seriously out of shape! I will work on this issue! Thank you!
Dr. D, You are a God sent. I wish I had discovered your video three years ago but its better late than never. Thanks a million for your dedication to helping us.
I can't thank you enough I've been having weak glute for two years and I've been diagnosed with hip arthritis, those exercises have been so helpful to me. Thumbs up for good work
Besides mentally telling yourself not to fire the quads and hamstrings and hip flexors, how can we manually manipulate these muscle groups to back off when we're trying to get the glutes firing? That's been the biggest issue for me. Thanks for this great explanation of how all these muscle groups work together!
I too, have huge quads and I am 4'8" and overweight but can do over 100 deep squats--yet I can't isolate my glutes. You are brilliant! I have hope for overcoming my hip pain and documented OA! Thank you!!!!
Hi Doc - I've undertaken the glute strength thing earnestly and with success so far. I've cut out quads work at the gym...and surprise surprise the osteoarthritic pain in my hips is backing off, and the clunks are not as acute. At 52 I'm not going to be a fitness model lol but I wonder, could I ever confidently return to squats, deadlifts and lunges?
I love your videos Dr David! Thank you so much for sharing all this information. I suspect I have a sciatica (shooting pain in the left lower back and hip when e.g. trying to get up from laying flat position). Just recently I began getting pain on the left knee's inner side too. Would glute strengthening help here? Maybe you have a video on sciatica pain as well? Thank you so much! Many Blessings!

Thank you-This is enlightening about identifying muscle imbalances for resolving hip problems/osteoarthritis. I was told I had osteoarthritis 6 years ago after xrays and tests and then one PT mentioned I had an overused TFL and my hamstrings were not firing. So wierd - since Im a lap swimmer and been an athlete all my life and practiced yoga and was a hiker. How do these muscle imbalances start?
Excellent advice!! This is such a common problem But what is the root of the issue for not using the glutes? Is it neurological ? Our lifestyle ? Sitting too much? I also had no idea this could lead to cartilage loss So now I w Nader if this is something that caused my parents to both need hip replacements and soemthing I should pay even more attention to!!! Can anything else cause cartilage loss (besides joint incongruence)
Can thus cause a tugging pulling constant pain and feeling like body being twisted inwards. It goes down leg and knee collapses inwards to now along with constant lower right abdomen and back pain
I have tight hip flexors and am using a lateral machine (scoop) to help. Almost all of the pain I experience is my flexors. So can I keep using that with glute exercises, or stop using it and just concentrate on my glutes
I'm not even sure how I stumbled across your videos but I'm glad I did! I have hip pain only when I sleep ongoing for several months. I have to sleep on my back but I also developed plantar fasciitis. Months before this I had surface numbness in my left thigh that went away. I have been working out for over 5 years 2 to 3 days a week and am in pretty good shape for my age (50). I just had this feeling that possibly all of these new problems are somehow related and I am big on finding the root cause of things and don't like to go to doctors. I finally went to a podiatrust but I have not opted for injections for the plantar fasciitis. Anyway, after watching your videos I'm leaning towards thinking my problem may be Bursitis. I'm going to start doing the glute muscle strengthening excercises and see if it helps. The classes I do have a heavy focus on the quads and mine are strong so the muscle inbalance would make sense! Thank you so much! I wish you were closer to me in Texas!
Hey Lisa! Sorry to hear about your plantar fasciitis problem and hip problem :-( Working on the glute strength is a fantastic starting point.
Thank you!
You're welcome Val!!! Please let us know how it goes trying this out. 😀
Dr. D, You are a God sent. I wish I had discovered your video three years ago but its better late than never. Thanks a million for your dedication to helping us.
Hey Harriet! Thank you so much for your kind words 😊
Thanks Doc. This is such an honest and helpful explanation. You are a breathe of fresh air.
Glad to hear it was helpful! You are very welcome 😊
I have been doing workouts for my glutes several days a week since I started watching your videos. I am doing much better! Thank you!
Hey Liz, you are so welcome! We are so glad we were able to help you get to doing better!
I can't thank you enough I've been having weak glute for two years and I've been diagnosed with hip arthritis, those exercises have been so helpful to me. Thumbs up for good work
Are there any cues or techniques to fire the glutes only? I’m struggling with this
😊Doctor, you are soooo very good at explaining! Thank you😊
Thank you Sunshine! ☀️
Hey Golden Light! Sorry to hear about your sciatica problem :-(

This is such a common problem
But what is the root of the issue for not using the glutes? Is it neurological ? Our lifestyle ? Sitting too much?
I also had no idea this could lead to cartilage loss So now I w Nader if this is something that caused my parents to both need hip replacements and soemthing I should pay even more attention to!!!
Can anything else cause cartilage loss (besides joint incongruence)
Can thus cause a tugging pulling constant pain and feeling like body being twisted inwards. It goes down leg and knee collapses inwards to now along with constant lower right abdomen and back pain
Excellent info! That's why the exercises are working. Would traction help?
Leg press a good exercise to heal bad hip? Figure it will make the glutes and hamstrings stronger?
I have tight hip flexors and am using a lateral machine (scoop) to help. Almost all of the pain I experience is my flexors. So can I keep using that with glute exercises, or stop using it and just concentrate on my glutes
Dr, your the man. Now I can begin to walk better ( I hope)
Hey Lisa! Sorry to hear about your plantar fasciitis problem and hip problem :-(
Working on the glute strength is a fantastic starting point. It may also be sciatica, as that can cause both hip and heel pain. Either way, addressing glute strength will help sciatica or a hip bursitis problem. In case you need it, here is a playlist for sciatica problems.

Hugely helpful, Dr. David. Thankyou.....Going back to your exercises specific to glute/hip arthritis again. So much easier to stick with when I know the reason.
David, I don't know if you read these comments. If you do, can you comment on issues that may be caused or exacerbated by the golf swing. If you watch pros golf swings in slow motion there is a tremendous amount of hip rotation in the swing.
WOW - Thank you so much for this very helpful video.
Could a loss of hip cartilage also be caused by weak abdominal muscles. Like from abdominal surgery that resulted in a lot of scar tissue through the abdomen that formed adhesions? Thanks again!
You're really good at explaining this concept, Dr. David.

@@epmanualphysicaltherapy Thank you!! Your tip to engage the glutes throughout the day is making a huge difference. Engaging my lower abs wasn't enough. As you mentioned, it doesn't matter how strong our muscles are if we don't properly engage them throughout the day. Sending much love to El Paso

Thankyou you helped me
This sounds like me.
This is me…So interesting…
Thank you Pj Pj!!!
You have many videos pertaining to this issue …Ty by the way….they all seem to have a gluten or two or three exercises….a helpful next step would be a cohesive glute activation/ strengthening video…maybe step by step…I realize the severity etc would effect the perfect program…or maybe one or two videos that you have out that you suggest me and others could refer too…again thanks…very enlightening
Another great video. 👍
Can you comment on impingement and labral tears? My understanding is that if the impingement has caused the labral tear there isn’t much you can do outside of surgery. Thoughts?
Watch the video .. this is not at all what he’s saying… it’s ur glutes.. they are weak..it’s a muscle imbalance u gotta figger it out.
Thank You. This is a great video

You explain these issues so well. The info you share is so appreciated
You have explained so much that it's taken me over a year to figure out in my searches for help for my pain and problems. I'm still learning... and learning quite a bit from your videos. I think I'm on the right path after some mistakes. I don't know if I can ever reach a truly satisfactory level of functioning but I don't give up! So much retraining to do. Years of unbalance and misalignment. Your videos are encouraging to me and I know the info you explain will help me move forward even more.
