At the end of the episode, 15 talks to her that he's worried about bringing Ruby along because he might be bad luck, and then the show cuts away to Ruby as she figures out that the Doctor is a time traveler. She rushes downstairs and Mrs. Flood urges her into the TARDIS, and once inside we see the Doctor again, but he's changed his shirt. My theory is that Mrs. Flood went on at least one trip with 15 in the time it took Ruby to get downstairs, which is what has convinced him that he is not bad luck, as he feared, so he can now permit himself to take Ruby with him. In the process, Mrs. Flood has become a more understanding person, shown by her newfound warmth towards her neighbor at the end. It would also be a fun nod towards the theory that 9 left for an extended time at the end of Rose before returning to tell Rose that the TARDIS is a time machine. As for the fourth wall break (assuming that's occurring in-universe), it could be a lingering side effect from her journey - an encounter with one of the "powers beyond the universe" that 15 mentions in the season's trailer.
