    I just want to say THANKS FOR THE VIDEO! I have my oral board coming up in a couple weeks and this is so great to get to see an example of what it is like. From what I gather it appears that all bases were covered. In my case I was told that I would have to choose the correct hospital (choice of two) and I would have to give a radio report. Great job

    There are alot of hoops you have to jump through to pass stations in the national registry that you would not do or would do in a different order in the field and this is one of them. Its not necessarily testing your interventions but your ability to think. Again just a hoop you have to jump through. For example in our oral stations we have to ask what level partner we have and what class of ambulance, what type of residence (apartment, single story, etc).

    I agree, this is usually a refusal. Identify AMS quicker and take a sugar. Not a breathing issue. Get vitals, start IV, give D50, reevaluate sugar, closing vitals. Meet pt, get sandwich, sign refusal.

      Generally for national registry it's best to follow the format on the sheets over what you may do in the field.

      smj1245 my school is doing partial practicals. one at a time. blew bleeding control out of the water. Easiest one. i think it may even be easier than setting up a tank.

    Wow. There are some ridiculously brutal remarks on here. This kid is obviously practicing for or taking an exam, so that would say to me that he probably doesn't have any real-world, stand-alone experience. Other than being a bit slow and giving the guy high-flow O2 when a NC would have worked, he did a fine job... Why be a douche and tear him down, Oh Great ParaGods Of RUclips?

      You're obviously not a medic if you're calling people paragods. If you were you know the hundreds and close to thousand hospital and ambulance hours that go into paramedic school just for clinical. Yeah, you might have an idea what you're doing at this point. I would hope you know what you're doing at this point. If not, you fail and you don't get to take care of great internet warriors like you.

    Working on a BLS truck as a basic going to 911 calls daily, I would have (of course started ALS en route due to AMS) but checked glucose on scene due to the "sugar problems" or whatever the wife told. Not until some 4 minutes into the video did the hx of diabetes come up.... Sometimes these practical skills are so silly compared to real life scenes.

      Travis Williams this is for nremt oral testing though. you have to follow the flow chart and approach in a systematic manner to get all the points because the proctor is checking off boxes as you go down the list.

    The guy was confused and running around naked with a BGL of 27. Having a BGL that low is worse for the brain than 25g of IV dextrose is for the kidneys.
    Factor in the aspiration and vomit risk with a confused pt given PO sugar, and IV dextrose is the clear favorite for this scenario.

    If the patient has a patent airway wouldn't you administer oral glucose instead of D50?

      If you believe the patient is altered and can't swallow, oral glucose is a bad choice. Also this pt has a blood glucose reading of 27, he needs d50 since it will act way faster. I have used oral glucose with Pt's that are completely awake and alert and are slightly hypoglycemic.

      Jacob Lipsitz oral is under 60. i would probably d50 at 35 or below because they should have a syncopal episode and lose ability to control airway

      You don't give oral glucose if a patient is alerted. Even with a patent airway you don't want to administer anything orally. Great video of this student! One thing you need to remember when doing these's skills is when they give you a life treat you need to correct it. He did a great job of going through his assessment but once they tell him the BGL is 27 you need to address that and correct it. Once you correct the life threat then move on and continue the rest of your assessment.

    My only concern is that the patient is now alert and oriented, you didn't ask him if he still wanted to go to the hospital, you just took him. No bueno. The timing of the D-stick, who cares, he got it is all that matters.

    I would have got dinged a couple of times. I would have gone 6 lpm NC, BS stick immediately on hearing "sugar issues" not to mention his mentation, GCS 14, and per our protocols. vitals and monitor. spiked a bag of LR at a TKO rate, he wasn't hypotensive so no need to run fluids, and I would have given him the full 50g of D50 (per our protocols), rechecked vitals and BS, through a 10gtts set and an 18g cath in his AC.I wouldn't have considered it a load and go. Our proctors are hard-asses.

      +Matt yeah that got me too, he had a patent airway and was breathing adequately, sats were 97% RA but IV, O2, And monitor are all one word for a paramedic i guess that's why he put him on O2

      +cesar jimenez but in Australia it isnt indicated for those vital signs and I dont know why you wouldnt have taken it off when he was told 99 spo2.

      maybe he considered him to be in an Altered Mental Status (AMS) which is a valid reason to provide high flow.. It is a gray area though

      In my school we do NC for medicsl unless spo2 is under 94÷, they have signs of shock, or are critical.

    In response to most comments this is a load and go pt. as per NREMT-P standard due to the confusion/lethargy. I agree I would've flowed with the scenario different but the candidate did properly hit the key points throughout the scenario. Its just gonna be up to the proctor at this point whether he felt that the candidate appropriately completed all key points in the order of his choice.

    This is a no brainer unless there is a legal DNR Handed to the Treating Officers at that time you continue with treatment of the patient.

    national registry is not 100% clear on this (I'm talking about the administrators in Ohio not the sheets themselves). As long as the vitals are taken after you've ruled out immediate life threats ie: the Primary Assessment, you are permitted to do the vitals whenever. Sort of. Again different people at NR say different things. I would always teach that their first set of vitals is always done as the first thing on the Secondary Assessment. No one has ever failed that point by doing vitals then.

      His glucose level was 27 so he needed it fast. Also PO meds are a contraindication for altered consciousness patients

    visamedic-your protocols do not state to give 50g of D50....the entire amp of D50 is only 25g total in 50mL. This student administered the correct dose.

    Not bad, but I would have gotten a set of vitals on scene and taken his glucose levels. And I agree with everyone here, dextrose is a bit too much in the beginning. Some simple oral glucose would have worked on scene if the pt had a gag reflex and could swallow

      His AMS could be due to hypoglycemia though. If he has the ability to swallow and can maintain his own airway then I see no problem with administering some glucose to see how he responds. You can also give him D50 and immediately after give him oral glucose once his symptoms of AMS are gone. That's all I would do, speaking from an EMT-B level. Paramedics may be able to do more for him, I don't know.

      ***** Not everyone is is a paramedic; I may be wrong but ,oral glucose is not a PO medication it is Bucchal it does not need to be swallowed. Careful administration could prevent this guy from having seizures and slipping into a hypoglycemic shock.

      Brigerre Berry, I would tend to disagree.
      While you are correct in your observation that diabetes does impair oxygen delivery, there is no need for oxygen in the acute hypoglycemic patient who is not hypoxic. The problem is Neuroglycopenia, which is treated with some for of glucose. What protocol says on the matter is secondary to good clinical judgement.
      In this particular patient, if he is able to swallow, then oral glucose is an appropriate option. Just be aware that it will take a prolonged time to see an appreciable rise. D10 is better than D50 in most patients (lower instances of phlebitis, more sustained rise in BG with no rapid spike, less impact on A1C's), but it depends by service.

    One time I had a call of insulin overdose pushed two D50 couldn’t raise the sugar 😐 he forgot a d5w drip that sugar will drop again

      @Bamberrydude For a patient with an altered mental status put the pulse oximeter on the patient and then get a blood sugar reading.

    I just dislike the Right Pt. and Right time, like you are going to get the D50 and turn round and give it to your partner one hour before we get there. The rights came from nursing when they would go in the right room and wrong bed or they would mix up the time.

      1. Graduate HS.
      2. Attend a community college that has an EMS program.
      3. Become an EMT-Basic first.
      4. Apply for your school's paramedic program.

      Serge P You can take the EMT course during high school (between the junior and senior year) and then take intro biology, a prereq for Anatomy and Physiology at most community colleges,during your senior year. If you take this during your junior year you can then take A&P at college during your senior year. In fact, take as many hours of college credit as possible during high school and see if your district offers concurrent enrollment where they actually pay for you to attend college.
      Volunteer as an EMT if you have to at first, just for experience. Small town fire departments often need people. Then work as an EMT as much as possible while taking the rest of the paramedic prerequisites and applying to P-school.

      Serge P The EMT class has to be approved by both the NREMT and the State Board of Emergency services and the EMT student must be 18 before testing but not before entering the class. Usually offered at a community college or hospital but hs students can take college classes on a college campus as long as they take and pass the ACCUPLACER or have high enough ACT/SAT scores.

      Depends on what state you're in. Do well in your sciences. You should understand Anatomy and Physiology and basic Bio and basic Chemistry. Become an EMT for a year minimum in a high call volume area, to be a good medic you need to have strong patient assessment and Basic Life Support Skills. I'll be realistic with you, don't settle for medic, go to PA school. MEDIC is hard, underappreciated and not. Lot of good benefits, use it for growth and experience

    I am going to school to be an EMT in a month , can anyone give me advice ? And this like overwhelming information to know but will It be as easy as knowing the ABCs later on? Thanks !

      ABCs. JB learning prep is good for taking you NEMT written. Some of the stuff you learn in class wont be used in the field. Not every patient gets O2 and load n go is more common than you'd think. If you decide to do this as a job, DO NOT I REPEAT DO NOT go to medic school after you finish your B. You will be a walking liability to everyone in your community. BLS comes before ALS.

    I know anyone will be nervous taking these examinations. You have to CYA so much crap. Guess no fail for load-and-go CYA. I dont know where the SAMPLE Hx is going to be taken since they are in the rig for load-and-go. Sounds like a private ambulance company that wants to hijack someone out of the house so they can not RMA after they get d50. Big mike gave me an LOL when he said get some clothes for the guy faster! LOL Maybe this medic will pop someone with 2mg narcan and get knocked out.

    Remember that if you give D50, the sugar will actually drop rather quickly if the patient does not eat something. While appropriate education is key, I as a paramedic refuse to administer a medication of any type and then allow the patient to refuse and regardless of the situation I am not going to spend an excessive amount of time on scene. The dose of D50 is 25 grams to 50mL. I would consider oral glucose if patient able to swallow and maintain ariway.

    i would have think its a critical fail for this guy? d stick of 27 and he still continue doing assessment? i would understand checking pupils to rule out opiate induce aloc. but i thought when you have a sugar that is low you have to correct it right away.. :)

    Love administering D50 before getting a d-stick. How do you know that is the actual problem even if you do have that history. You did the right thing but still you don't administer medication before having the information you need. When you get to the hospital the first thing they are going to ask is what is his d-stick. You're going to look like an idiot. Just my two cents.

      +FrostDesigns | Graphic Design it's the best thing ever you learn alot and if your heart is to be there for people in their time of need and to save lives... and you also learn alot also about yourself, just know that this field comes with blood and things that may give a regular person nightmares... I love it beats working on cars for a career

      +FrostDesigns | Graphic Design ask yourself what do you want to do with life and write down EVERYTHING that comes to you, research it look into it ... this works for me pray to Jesus about it ... you're young the earlier you start doing what you are happy and good at doing the better, get out of debt stay out of debt,build credit, save money, start your small business or whatever, keep building working thinking and never never quit

    Sublinqual glucose! (icing) This could have raised his glucose level within seconds.

      ***** Oral (po) glucose or anything else given orally for that matter, is contraindicated in AMS but sublinqual or buccal glucose is not po. Your State and your agency's protocols should address this, specifically, as icing is the drug of choice for the emergent treatment of hypoglycemia. The patient and family are taught to do this by their diabetic nurse educators and by many pharmacists as well so they can self-administer this easy to carry glucose. Use judgement of course; a small amount a time and waiting a few seconds for absorption, doesn't cause it to be swallowed. just absorbed. But the rule is one tube for glucose < 50 mg/dl.

      ***** It is only contraindicated if the patient cannot manage his airway. As a basic you can only give glucose so you need to be able to give it to a hypoglycemic patient w/ a decreased LOC, you just need to be careful, e.g. if the patient is unconscious place him left lateral recumbent and rub a small amount of glucose on the inside of the downhill cheek until it's gone, repeat until he wakes up. You could save someone from serious complications, just make sure it's the right call.

      Dude, I'm not giving anyone with AMS anything orally, especially some home remedy like icing that may work but is in no way is an approved treatment of an AMS patient. You're going to tell your superior(s)/this patient's family that you took it up yourself to follow the "icing" protocol and it's ok because you "just needed to be careful"? Lol, I don't know where you guys work but that wouldn't fly in most reputable organizations/departments.

    This guy needs more practice. He got the D stick way late. The pt is acting this way because of his sugar. Cool pale clammy give him candy!!

      I'm assuming you've never been, in any point of you life, in the medical field. As EMT's and paramedics we have a strict guideline of patient assessment. Before we get to vital signs(not including a quick pulse check) we have to obtain a chief complaint, and then check their airway, breathing, circulation and then identify whether our not our patient is a priority transport and also update our incoming EMS units. It's at this point you can get a D stick and other vitals.

      lmao he was literally going straight down the sheet word for word almost. In real life he would have gotten it just fine but for NR he needs to go down the sheet exactly as he did.
