    That's a variac. It's basically a type of transformer, commonly known as an autotransformer. It's used to regulate AC voltage. This one doesn't seem to have a high current capability, but it was probably operating at a higher voltage. The voltage was likely fed between 1 and 5 with 3 as the output. Normally the load is going to be connected either between 1 and 3 or 5 and 3. Connection 2 seems to be a fixed output that is a relatively low voltage in comparison to 1 and high in comparison to 4 and 5. Lead 4 is similar, but the voltage there is adjustable with the gear mechanism and the second roller contact.
    My guess is that this came from some type of a 3 phase equipment where there would have been 3 of these variacs ganged together.
