I use a pleated sleeve also. I do make sure that the depth of the pleat (the excess size of the outer layer of the sleeve) is not quite enough to allow the sleeve to show above the binding if the sleeve is flat, not filled with a hanging rod. If I need a really deep pleat, I make sure the top edge of the sleeve is stitched down a little from the top edge of the quilt or the binding.
On sleeves for wall hangings, I split the sleeve into two parts with about 2" of space between them in the middle. By doing this, one can use picture hangers on the flat rod to hang your quilt.
