Tatsuya is the real yotsuba. Miyuki is only a homonculi created by the clan to protect Tatsuya of assassination and be Tatsuya’s wife.
You are right that Tatsuya is the real Yotsuba, but they don't want Tatsuya to be the heir. They replaced Miyuki instead of him and made Tatsuya her guardian in order to protect her till she marry someone else. Also, a point to be noted is that he also don't want to be the heir of the Yotsuba clan.

His backstory is confusing. On one hand, it's explained that the reason his mother and aunt did that Artificial Magician Experiment on him was because he didn't have a strong enough Magic Calculation Area due to his Decomposition, Regrowth, and Elemental Sight abilities dominating it. This procedure resulted in Tatsuya being able to use magic (at a very low speed and power), but unable to feel strong emotions regarding anything other than for his sister.
Then, it's later revealed that the "real reason" for the experiment was to nerf him, to insure that he wouldn't lose control of his emotions and become a berserker and destroy the world with his magic. They basically took away his ability to feel strong emotions. And the reason they feared this possibility was because Tatsuya was so powerful even as a baby. Some members of the Yotsuba initially wanted to kill him after discovering how powerful he was as a new-born.
Now, with all this exposition out of the way, the reason none of this makes makes sense is if the "real reason" for the experiment was simply to keep his power in check, why? He can't cast magic fast enough compared to most Magicians. Essentially, we see what Tatsuya's powerset is. And there are plenty of Magicians who can take him out with the proper strategy. All that "real experiment" did was enable Tatsuya to remain calm and calculated, it also gave him a perfect memory, enabling him to remember everything he's experienced since he was 6 years old. The original reason for the experiment made much more sense than this retconed reason.

Either way, Tatsuya and Miyuki in***t is so wrong in many ways regardless of what people say. Even if it does explore a messed up society, especially on the even more messed up Yotsuba family’s side.

There's an arc called Reminiscence Arc, where Tatsuya's backstory is given. The Reminiscence Arc is also going to have anime adaption. So, maybe next year, we will be able to watch it.
