I'd say probably the hl2 e2 gnome achievement, because theres just so much you have to do with that stupid gnome to get it to the end without losing it or leaving it behind.
For what I personally believe to be the hardest achievement to get in video games that I know, it would be the metal massacre MvM achievement for TF2. You have to kill one million robots. Assuming you don't cheat, it would take literal months of in game play time to get that. I only have 10k robots killed on my current steam account, and I've played MvM on this account off and on for a few years now.

I’ve done all of these and I have to say it goes to little rocket man due to the gnome always flying out of vehicles etc outlast insane mode is not that bad if you know what you are doing and you can finish it in a short amount of time the real challenge is outlast 2 messiah run and the black mesa one was a breeze besides climbing ladders with that hat
it was the gravity gun in ravenholm LMFAO
the one in half life 2 where you can’t step on the sand, that is the single achievement that I can’t get

Try frequently quicksaving. It's not that hard if you do it that way.
I did it on my second try, I failed at the beginning of the antlion part. If you make a lot of save files it will be easy.

Little Rocket Man was more tedious than hard. That was until the chopper chase section. It didn't help that I did it on the hard difficulty. I almost lost my mind when I found out you had to close the door on the rocket for it to count. Good thing I kept a manual save just in case.

Hardest part of gnome was if he pinged out of the car at full speed lol

The little rocket man achievement was easy for me, I completed it on my first playthrough of Half-life 2: episode 2 lol

Finishing all the "vidmaster" achievements back in halo so i could get the flaming helmet in halo 3. Those odst achievements were a btch
The hardest one of the three is the rarest specimen.
But for me, Pepperoni precipitation in black mesa is the hardest one, because this frickin' box has a bad collision, and the hardest part for this achievement is chapter 17 interloper. This achivement is harder than rare or the rarest specimen, 'cause as I mentioned before the box has collision while you are standing on top of it, and it sometimes disappears, clips through every possible texture, and the most importantly, is that I can't pick it up in very closed areas while I am standing on it, like the vent in bm_c2a5g. Sometimes because of that I am getting softlocked, and having to restart from the latest autosave. WHEN I REALISED, that I could use console commands to spawn a prop, and change it's classname...
Rare specimen overall isn't that hard:
Chapter 3: Unforeseen Consequences
Find a hat, and run like hell.
Chapter 4: Office Complex
Leave a hat in the areas you will return to, and carefully jump onto the ladder to the next chapter.
Chapter 5: "We've Got Hostiles"
Leave a hat near a silo door and proceed with the level.
Chapter 6: Blast Pit
On the first level of the chapter, skip the tram ride, leave a hat in the tentacle room, on the last level run and swim like hell.
Chapter 7: Power Up
Leave a hat in quarter open gate, where 3 trams are hiding and proceed with the chapter.
Chapter 8: On A Rail
Drop a hat in the direction your tram will go to, and quickly pick it up. Do it for the rest of the chapter.
Chapter 9: Apprehension
Do not use yellow big fast tram, kill all hecu's, andjump in the waters, get the crossbow, throw a hat in the door you will go to, come with it into the trap room.
Chapter 10: Residue Processing
Run, crouch, and swim like hell.
Chapter 11: Questionable Ethics
Leave the hat in the main room, and proceed with the chapter.
Chapter 12: Surface Tension
Get the hat through the drainage pipe, run like hell, leave a hat, destroy a helicopter, tank and a light tank, get the hivehand, save Jackson, run like hell, leave a hat and get the guard to open a lambda door, kill gargantua.
Chapter 13: "Forget About Freeman, we are cutting off our losses, and pulling out. Anyone left down there now is on his own. Repeat, if you are not already... what, oh sh*t NO, NO NO NO *boom*"
Run through drainage system like hell, destroy a tank, run like hell.
Chapter 14: Lambda Core
Leave a hat in the beginning of the assasin room, kill assasins, return to the hat, acquire the gluon gun, leave a hat near a scientist, that wants you to hurry up, flood the core, get into the laser lab, get through teleportation labs, jump into xen.
Rarest specimen is a bit harder:
Chapter 15: Xen
Find a hat and run like hell. Juggle hat and cyanogen canister like an idiot, without putting a hat on, run and juggle like hell, leave cyanogen canister and a hat in the main teleporter room, activate the teleporter, throw a gas canister and a hat into the teleporter.
Chapter 16: Gonarch's Lair
Scare the Gonarch, juggle two props like an idiot, send gas canister to the second battle arena, run like hell with the hat, kill Gonarch with the cyanogen and jump like hell.
Chapter 17: Interloper
Leave a hat, run like hell, leave a hat, jump like hell, shoot controllers, repeat.
Chapter 18: Nihilanth
Kill Nihilanth.
Chapter 19: Endgame
Acquire the rarest specimen.
Pepperoni precipitiation
All chapters from 11 to 19
Leave a pizza, smack it with the crowbar, get frustrated, repeat.
Or you can cheat using ingame console commands to spawn a prop on a needed map, get all of your weapons and a suit, run like hell, kill Nihilanth.
I had the most fun with the pepperoni precipitation, the rarest specimen, the plan and little astronaut in hl2ep2 (I didn't have fun at all, these achievements truly are the worst, but I will rank each one of them by 10 point system, 10 being the hardest, 0 being the easiest).
The plan and unquestionably ethical: 2.5 - 10
A little astronaut: 4 - 10
Rare Specimen: 5 - 10
Rarest specimen: 7.5 - 10
Pepperoni precipitation: 11 - 10

the factory section of interloper is horrific with the hat, but the pizza tops it 10 times.

That one's not hard, especially not with the game's current state. That one is just time consuming in the most literal way.
Mastermind on GTA 5 was the hardest for me. Having to do all 25 Heist missions on the highest difficulty with the same team with nobody dying was so stressful.
Outlast's lunatic achievement was kinda easy after mastering hard and nightmare mode, But outlast 2's messiah achievement is pure hell.
If you want a challenge that I know you won't get is the tough as nails achievement in the game called brink
Hidden hat + peperoni pizza. I did it the two "together", until gargantuan nest, in which you could only carry one.
But also in xen i taxi 3 itens( the hidro canister) for gonarch achievements.
But the most time consuming of all? Age of mithology achievements. Which require 1000 online games.
You can't imagine the time that it will take to pick up the Purple Hat and the Pizza Box in one playthrough ( I'm doing it right now to try to get all the achievements in one playthrough )
People say that getting the real gary achievement is hard? You can bypass it by sam. But think about the rebel rush event, one in a lifetime opportunity
7 day survivor on Dead Rising I’d say is still the hardest achievement. Not just cuz it’s hard but the fact it’s so easy to lose focus and get bored and die from not eating food. I’m glad I got this achievement done and over with shortly after going for the 100%
Legendary achievement
Metal massacre in team fortress 2 is brutal.You have to kill 1m robots in the mode man vs machine.Thats basicaly a 1000 hours.
Comet is a lego skin so she would likely appear with the lego update
Dude the Half Life 2 Episode 2 one where you have to save all houses in the Strider assault...
Instead of having to do this, just get a prop in The Rocket entrance, do ent_setname gnome (isnt considered cheating) and put the prop into the rocket and close the door, and you get the achievement!!
one or the hardest achievements I've seen yet are the metal gear rising revengeance ones
the vr missions are absolutely insane
vr missions was brutal, but 16 th mission(the turret one) made me give up on that achievement. but i would say the hardest are the speedrun achievements and the dlc armstrong boss no dmg which was so insane i gave up on the platinum.
l4d2 where you have to complete the last stand campaign in 13 minutes or less. was about to tear my hair out til i got it at 12 minutes
If the Insane mode without reloading camera batteries was on there I would say that one since I have learned the hard way
Gnome the helicopter chase was a nightmare
50 times over the mountain in getting over it. (This took me 40 hours)
Hardest achievement I ever got was beating Max Payne 3 in one shot without dying , “the shadows rushed me” was the name of it
ngl gnome achievment was really easy- I did it on my first playthrough- if you dont believe me I livestreamed the whole thing. I think I just thought it was easy because after doing purple hat and pizza achievement in black mesa
Yeah rarest specimen is much longer. Plus all the ladders and vents.

It might be 7 Days survivor in Dead rising 1 infinite mode but you have to get true ending to get that mode
I got the gnome to the rocket part and put him in the rocket,because I thought that since its called little rocket man,I thought you need to put him in the rocket.
Gnome/Hat one's aren't hard, just tedious.

For me it was "I chose the impossible " achievement in bioshock remastered the reason why I say this is because you have to beat the game on survivor difficulty without using a vita chamber it took me 1 month to get

I wouldn't classify this as the hardest. But my luckiest achievement/trophy was winning 5 crowns in a row on Fall Guys. I did this in the launch month of the games release. Couldn't really do anything else at the time due to Covid so I just kept playing until I unlocked it. And this all me playing solo without any help from like a boosting team/friends.
I'll be honest I got the little rocket man achievement on my first run in episode 2, and I don't know how I didn't quit lol (hardest part was where an heli followed the car, it was pain to transport gnome while being chased by heli)

These are tedious achievements but not difficult ones.
Difficult requires a level of skill to beat them like gears of war 3 on insane mode or gears of war 4 on insane mode with the death restart game option on, making it so that when you die it restarts the game from the beginning.
The one you mentioned with one life is the only one that actually is difficult.

I finished the black mesa one and hl2 episode 2, but honestly I have never played outlast so I guess it’s hard to not die once.
The outlast one seems the hardest to me. I've played and beaten the game three timed the and it definitely would take some time and skill. The rest are easy, I have carried dogs ball to the end of HL2 and taken the gnome to the end of Half Life: Alyx and it isn't hard. The only parts that are particularly annoying is when the ball exploded due to fire and grenades, or when the gnome would fly across the map or into a wall or the floor upon dropping it. Causing me to reload my save many times. But aside from those you just have to beat the game regularly.
However the hat one seems very tedious, as Black Mesa is a very long game. So props to you for completing that as well.
Okay not to one-up you with the gnome, but I did it on the PS3. On my first ever run of EP2.

The outlast is probably the hardest
the two are just timetaking, it doesn't really make the game harder that much. You can drop it in a safe place and kill the enemies and pick it up again.
The Hl2 is the easiest out of the three since HL2 EP 2 is not that long compared to black mesa.
I haven't tried doing these so I'm just answering based on my knowledge.

The hat is bullshit, the game was atleast built with the gnome in mind, but the hat? Nope that's an addition in the remake, copying the gnome, and not changing anything so it's annoying as hell

There are some ridiculous achievements in Mega Man 9, one is for beating the game without taking damage at all and another is for beating the game 5 times in 1 day.
