In linked wiki, the writer says this game is 'almost completely impossible to beat', but I think it's very exaggerated description. If you are good at playing original Hotline Miami game, you can easily kill all enemies in the map. The only problems are uncomfortable control(you can't even move diagonally!) and too fast reaction of enemies.
According to wiki, the level design of this game was re-used as first floor of 'Full House(chapter 5)'.

Here is my story for Super Carnage:
The protagonist is taking a shower when a bunch of armed guys and dogs break into his house. He was so angry he goes in a murder spree. He has no time to take a towel.
