I’m pretty sure Brian is the only one who always can understand Stewie. And the occasional side character that can understand him, like that kid he beat up and threatened in one of these scenes.
only Brian and Chris can understand Stewie's words, the other family members can just hear him yapping like a baby only but can tell if he's angry or happy etc. there is only that one episode when he goes to the future to meet his older self, his family can understand him.
