It's a Duo Classic Tandem made by Van Raam Aalten BV in the Netherlands. Sometimes called Doove Duofiets or HJD duo tandem. Also search for Hase Pino from Germany or Gustav W Strada trike if a combination upright/recumbent is what you're looking for.
