idea is great, now coming to real life. how much time a physician is allowed to spend on each patient, and how much capability is the physician expected to have an understanding of concepts that he is instructing the patient on.
Those are great questions! Shared decision making can be really time consuming when done well, even with the use of decision aids. I have an advantage as a hospitalist because it's relatively easy for me to block out 30-45 extra minutes with a patient whom I've already been treating daily for the problems we're going to discuss. But clinic docs, who have 15-20 min per patient (to discuss everything), often don't have such luxury. I imagine that some highly specialized clinics (e.g. heart failure clinic, diabetes clinic) may have PAs, NPs, or NAs who may have more of a dedicated educator role where they can conduct such time-consuming and nuanced conversations, but that will be the exception.
