Truffle worms are actually unobtainable from quality bait crates. The only way to get them is from volcanic geodes (which is a really low chance) or completing 50+ angler quests. Angler is easier, so just do that.
I see thouh is indeed using the correct environmental orchestrated pieces from the popular franchise "Undertale" and if thy doth like these orchestrated pieces my thyn introduce thoust to the wonderous would of the sperate franchise "Deltarune" which was indeed made by the same maker of the other

when he spawns, you need to go to the zone, use fish head as bait (or no bait) and just fish there and try your luck

you can open bait crates to get them, you can either fish for them or you can buy them on Moosewood on the beach to the left or on a lot of islands !
