A lot of time if your skin is elastic it will spring back in when the underlying fat is removed. Laser resurfacing can also be used in tandem to cause the skin to tighten up. Neither involve removing excess skin.
It ranges, partially based on how skilled the surgeon is. Soon after the procedure up to 20-40%ish can die. For the remaining fat, if it establishes a new, good blood supply it can last a lifetime. No guarantees though.
