I read this can be mounted to tail lights how does that work?

Cool. How hard is to get behind say the rear quarter panel area. Can a tech pull back the wheel liner and get a tool up into that area.

No sir, 98% of the cars have a second skin underneath the outer skin, therefore there's no direct access to the area. You can drill a hole on that inner skin but that's your only access point to fit the tool. Hope this gives you a better idea.
