It wasn’t the whole world, it was mainly Europe and North America, but the radiation spread around through water and air
well tbh, I think it's very much plausible to go back to safer levels in 40 years, looking at Fukushima, Chernobyl, and how long the fallout is dangerous from an atomic detonation, with Chernobyl already being pretty safe, and fallout from nukes going away way faster.

Most people don’t know what nuclear fallout is actually like, or how current nuclear stockpiles can’t even destroy 0.1% of Earth’s surface. Even if nuclear stockpiles increased by 10 times and were used to cause maximum casualties, it would “only” kill hundreds of people. Most people would survive just by gaining distance from cities, not nuclear bunkers.
Meanwhile, as bad as fallout is, the total amount of radioactive material released cannot be more than the nuclear material that was in the nukes themselves. Since nukes exist to permit the MAD principle and not just to cause misery across generations, nukes are designed to minimize fallout. What fallout exists has a very short half life, so after a few decades, 90% of the radioactive material is gone.
So post-apocalyptic radiation wouldn’t kill that many people, nor would it contaminate much land, and what it would contaminate would be safe again in 40 years.
The biggest killers would actually be the destruction of infrastructure, various levels of anarchy, the lack of rescue operations, and fighting.

Late reply, but the problem isn’t so much that many Swiss people would die, it’s just that they are in the middle of one of the most radioactive regions in the world, so they wouldn’t be able to come out for a while.
