• Authenticated affidavit of support from a relative within the 4th civil degree of consanguinity/affinity duly certified by the Philippine embassy/consulate in the country of destination.
• Copy of sponsor's passport bio-page, visa, and POEA Clearance or OEC (if sponsor is an OFW)
• Original NSO issued birth/marriage certificates of all persons connecting passenger to the sponsor
• if former OFW, bring a copy of the latest OEC or POEA information sheet
FOR EMPLOYMENT:
• Proper work visa/permit at the country of destination
• Authenticated employment contract duly certified by the Philippine Overseas Labor Office (POLO), Philippines Consulate/Embassy
