I got my iud about a month and a half ago and it’s still bleeding, cramps. But I’m grateful for having it because my birth control pills I was on gave my extreme hyper tension
Hi Mgallowa- Based on what you’ve told me, I suspect that you’re one of the 30% who will not have a period (or maybe only occasional/ sporadic light bleeding) with a levonorgestrel IUD. That said, I often will tell patients that the first time they miss their period, it doesn’t hurt to do an at home pregnancy test for further reassurance (and especially if there were any concurrent signs of pregnancy like morning sickness/ nausea or breast tenderness...) But thereafter, it’s not necessary to do this sort of test every month (if you continue to NOT have a period). The chance of pregnancy with an IUD is very rare (2/1000 with a LNG IUD). Definitely you should attend your scheduled follow up appointment with your doctor. -Dr. Dervaitis

I've had a Mirena for a year now. At first when i got it, i bled for almost 4 weeks straight, at my check-up my dr didn't express much concern over the bleeding or cramping that I was having. Then for the next 9 months i struggled with bloody discharge for 3 weeks and then one week normal discharge. I was then prescribed Mireille and a clot forming medication to clear that up. I was prescribed the Mireille for 3 cycles and the bloody discharge was only for about a week and a half. But when i went off of the Mireille, within the same month i got a heavy period with heavy cramping. It almost feels worse having the Mirena.
I want to thank you for making these videos, Doctor. I got my kyleena yesterday and I watched a ton of your videos beforehand and it made me feel a lot better. I remembered your insertion video and how it took you about 15 seconds, luckily my gyno was just as quick! It hurt like hell but it’s over now. Thank you for what you do!

Hi kat x- In the first 4-6 weeks, it can sometimes be difficult to distinguish period bleeding from “adjustment phase” bleeding. I would refer you back to the video for more commentary about what to expect. Ultimately, if you are concerned about your bleeding, I would recommend that you see your doctor for a check up. Hi Dr. I had an IUD inserted in April. I already had a large fibroid removed in June. My period became light but this month it's so heavy with clots. Is that normal?

Hi Sonya- Sometimes bleeding can return after having been without menses (“amenorrhea”) on a levonorgestrel IUD. It’s difficult to predict what the pattern of bleeding would be, moving forward. Any time there’s a significant change in bleeding, I recommend that the patient have a follow up appointment with their doctor, in order to check that the IUD is still in proper position, and to rule out any other causes for the bleeding. Thank you for the video! I was recently diagnosed with hyperplasia with atypia. Got mirena 2.5 weeks ago. First week was spotting but for past 1.5 weeks it’s heavy bleeding. I don’t see my gynaecologist until Oct 9th and I hope it’s normal

Hi Kaylyn- When you have an “IUD change/ replacement” (immediate new IUD insertion after old one is removed) you are immediately protected from pregnancy as long as the old IUD is not overdue for removal (not expired). However, because of the small risk of expulsion after a new IUD insert (ie, risk of the IUD falling out), to be cautious, I ask my patients to use back up contraception until they see me for their one month follow up visit when I check that the IUD has remained in good position. Very informative. I am nervous because I am a week late on my period and I have no clue is it because of pregnancy or this new IUD. My old IUD was 1 month expired before I've gotten an appointment to change it. So I am very confused. I've gotten one positive pregnancy test on a blue dye test, which we all know gives out false positives. So VERY confusing

Hello negin Joseph- The amount of progesterone in your bloodstream with depo is far greater than what enters the bloodstream with a levonorgestrel (hormonal) IUD. If you were experiencing any progesterone related side effects on depo, they may decrease or no longer be present. If your cycles were very light (or if you had no cycle) with depo, there is a chance that you may have the same menstrual side effects with the hormonal IUD (although some women with no cycle on depo might go back to having a light cycle with Mirena or Kyleena). -Dr. Dervaitis
I was so concerned about my bleeding, my doctor didn't go into too much detail about the aftermath of having the Cooper IUD. Thankfully my bleeding stopped at day 5 from insertion. However, I'm still having cramps, and I sit for long periods of times. I find that my left side ovary/uterus is very tender and in discomfort. I worry about the IUD perforating my uterus (I think I'mbeing paranoid). Maybe, if you do a video similar to the bleeding but this time about cramps, pain and shifting of IUD, or debunk the miths, please!😟

This is my Day 7 with my Mirena on, day 2-4 were juz fine no cramps and no much spotting, On day 5-7, I having my heavy bleeding, I just called the dr , he said iud takes time to adjust approximately mths to adjust :(
Hi Amanda- Daily bleeding can be normal for up to 4-6 weeks after an IUD insert. I don’t think that a higher dose oral contraceptive pill would be of benefit. An IUD can be removed at any time. -Dr. Dervaitis
Hello- Generally speaking, most pain or discomfort associated with IUD insertion is described more as cramping pain (like period cramps) and in the lower abdomen. Back pain would not be typical. -Dr. Dervaitis
I am so glad I found this video. I had my copper iud put in on Monday and I am a bit worried that I did not get much information so far the cramping comes and goes and the bleeding has stopped for now I was just scared from the reviews and videos I’ve seen online
Hi Abrilia- Keep in mind that not every patient has heavier or more painful cycles with a copper IUD, so I would just see how things go. If you’re not happy with how your periods are with the copper IUD, you could later switch to a lower dose levonorgestrel IUD (you might not find as much acne as you had with the transdermal implant). Regardless, you will be protecting yourself from unplanned pregnancy using one of the most effective options available! Hope this helps! -Dr. Dervaitis
Yes it does help! I'll for sure search for that option in my country in case this IUD cause any trouble. I am still bleeding, today got heavier but taking Ibuprofen calms down the cramps. Thank you again for your thoughts!
Some women have irregular or skipped cycles for various other reasons (other conditions or situations that interfere with ovulation). I’d recommend that you see your doctor for further evaluation if you are concerned. If there is any chance you might be pregnant (if you’ve been sexually active since your last normal period), do a pregnancy test. -Dr. Dervaitis

Hello- It is definitely possible (and safe) to use a menstrual cup with an IUD. (If you search this topic on Instagram there are a lot of helpful posts with illustrations and tips. The patient just needs to be cautious when manipulating the cup so as not to inadvertently pull on the IUD strings (usually if the cup is placed low enough in the vagina it’s not a problem). Thanks I wanted to see someone ask this since I love cups but am getting my first Mirena inserted next week and my doctor said ‘we don’t have enough evidence to prove if the cup with IUD is a good or bad idea so be careful with removal’
I got mirena, and I had one day of pretty heavy bleeding at first, then it lightened. Now it's very light, and easily ignored. I have light cramps, but nothing as bad as it used to be before the iud. I'm quite happy with mine. I had a period a month straight before getting it.
Ugh. I’ve had an iud for about 6 months and have had horrible luck. Heavy bleeding for about two weeks at a time with severe debilitating cramping. I tried to hang in there as long as I could because everyone else I know who has it loves it. On the pill I use to get very light bleeding for about three days and pain the say prior to my period and the first day. I’m finally getting it removed this week. thank you for all your information!
Glad I found your channel and hopefully you can give me some info in regards to my situation. I had the Paraguard inserted in 2008 and went to have it removed in 2014. The midwife that did the removal failed to inform that it broke. My life has been changed physically by this. The broken piece has been embedded in my uterus for 4 years making it a total of 14 years. I’m currently working on trying to have it removed, but I keep getting passed around to different gynecologist. Can you shed some light on any possible long term effects this may cause me in the future? Thanks in advance
A small portion of the copper IUD is not likely to cause any health problems (although could interfere with conceiving if one is trying to get pregnant…) There have been many cases of patients forgetting about their IUD and the IUD staying in place for years past their suggested duration (sometimes decades) without major consequence. Generally it is still recommended to try to remove the IUD (or portion of IUD). -Dr. Dervaitis
I had harmonal IUD (Mareena) inserted 4 days ago, during 3rd day of the period. So far no issue of extra bleeding or prolonged periods, infact it remain same as before. Just finished now. No cramp, no headache etc. So far. I was scared before. Hope it goes well afterwards too.
I got my copper IUD inserted on May of last year, so a year and 3 months ago. But the bleeding between periods hasn’t stopped. I’ve never had that problem before the IUD. And I’ve been hesitant to go to the obgyn because of COVID. But I’m just so tired of this, I’ll be making an appointment on Monday for sure! Thank you for the informative video👍🏼

I've had an IUD (Kyleena) for a little over 6 months now and I’ve experienced bleeding and spotting nonstop, but it’s finally starting to go away, other than that I’m feeling fine (just very ocasional cramping). Bleeding is super annoying, but patience is the key! I also feel super safe (I don’t worry about getting pregnant anymore) and happy that I no longer have to remember to take the pill. PD: The first few weeks I did feel really nauseous.

I am going on vacation with my husband in 3 weeks and I got my period couple days ago, I am supposed to get my Paragrad (copper iud) when I get my period but I am hesitant to do it now because I don’t want to ruin the vacation. Do you think I should wait after the vacation to get it? Although my period is very light you could call it spotting. I can’t really answer this question, there’s no way to predict precisely how much bleeding a patient will have (or for how long) after an IUD insertion. Refer back to the video for general information about post insertion bleeding. Glad you found the video helpful! -Dr. Dervaitis

Hello great video! New Subscriber! I noticed that my coppe IUD (mona Lisa mini) has actually made my periods a lot LESS heavy which is so strange considering every where I read says it will make your period heavier. Have you seen this before?
I just got my Kyleena (first ever IUD) 3 days ago and my cramping is pretty intense, much more than I have ever experienced!! Ouch. Probably an 8/10 at times, Advil helps. They do come and go and it is not a constant pain, so that is good news. I assume the adjustment period could be a few weeks until my uterus gets adjusted? No spotting or bleeding yet post insertion!

@@tamisfincher I gave it a good 6 months, but ultimately it was not the right fit for me. I couldn't get over the idea of having the IUD inside me lol. However, post insertion I didn't have any bleeding and was period free for the entire 6 months! Cramping went away after a week or two. I would've stuck with it longer if I wasn't such a worrier. I am back to taking the pill but progesterone only.

Thank you for this Dr! I had the copper IUD fitted about a year ago. In January I missed a period due to exercise and fasting, and since mid Feb I have been bleeding continuously for about 6 weeks! Spoken to my Dr who didn't seem worried, but is this normal? I have no pain, no discharge, no cramps, i can still feel the stings, nothing at all sinister... I'm not keen to switch, as I really don't want the hormonal forms of contraception. So I'm trying to be patient. The blood looks fresh, and at some point in the cycle there was some clotting which has now stopped! But i still have some mild bleeding. I had literally no issues with it upon insertion and no issues for the first few months! But I have noticed continued bleeding after missing a period (after fasting/exercise - what I'm experiencing now is the 2nd occasion since having it fitted). I'd truly appreciate your feedback!
Hi Dr Thanks so much for this video. I had my Mirena inserted 4 months ago (replaced my Copper T with Mirena) and I'm bleeding constantly but a light bleed. I had a dermoid cyst removed two months ago so not sure if my hormones are out of wack. I see you recommend using an oral contraception concurrently with Mirena. Does this usually help patients with the prolonged bleeding problem? I'm keen to request a script from my OBGYN. I used Tranic for 5 days and it didn't help at all so I'm wondering if OC will help at all? Thank you

Hi Clara- Sorry to hear about this spotting, you must be very frustrated! While it’s less common for spotting like this to persist past the first few months, it does sometimes happen. Based on the timeframe you describe, I don’t think that there’s anything to be worried about. However, since the spotting is bothering you, I might suggest that you try switching to a Mirena (or even Kyleena) IUD. Although spotting like this is still possible with the hormonal IUD, the overall bleeding tends to be less (due to the levonorgestrel thinning out the lining of the uterus) and you might have better luck. This spotting is NOT necessarily an indication that you can’t continue to use an IUD. Hope this helps! -Dr. Dervaitis

Most IUD’s are to be used for 5 years and not 7 (but I’m not sure exactly which IUD you have, and in some situations the duration of use depends on patient age at time of insertion...) However last night I had extremely severe cramps for about an hour with about 5 minute intervals between each. Today I am not having any cramps but feeling tired and a bit sore. Still bleeding and painful cramping 😢 I only have it in to help managing painful and heavy periods. I want to go swimming next week, if I’m still bleeding can I use a tampon? (It’s the middle of summer time here).
There is not any solid evidence to say that tampons should be avoided after IUD insertion (although it’s been my practice to advise my patients to wait 3 weeks before using tampons because that’s the window of time where there is more of a risk of infection, albeit a very low risk). -Dr. Dervaitis

Hi Dr. Kristina, greetings from Melbourne. I’m Dona, and thank you so much for your informative very helpful videos. I’m writing to you to get some advise, I appreciate your time doctor. I got a Mirena inserted at a submucosal fibroid removal surgery. After two days of surgery I got an infection and given antibiotics for ten days, but my VOG asked me to stop them on the fifth day. It’s been four weeks now and I was recovering well upto 3rd week and had a little fast jogging(3rd Oct) as I was feeling good with no swelling, bleeding and pain, after 15 minutes I felt a pain on my left lower tummy and I headed back home, then my tummy significantly swelled after I got home, it improved by the following Thursday, but next Sunday(10th Oct) and Monday whole day that left lower area had a continued pain(bearable), and light bleeding could be seen. Dear doctor, my question is, can the Mirena be touched the fibroid removed wound area and not healing that patch? and giving this trouble? Shouldn’t there be a reason for this pain? (Kind note: I am scheduled to see my VOG on 25th Oct to see the placement of Mirena.) Dr. Kristina, any advice you could give me on this sudden pain and swelling, appreciate your time and I am very thankful.

i have taken out the mirena after 9 months, I kept bleeding 50% of the time. did keep getting cramps from time to time and then bleed. I prefer a heavy period over this cramps and blood all the time

i had paraguard inserted nearly two weeks ago and havent had a day without at least light bleeding (it’s extremely light but nothing too bad) but sometimes it goes medium light for like an hour and that’s it

This is my personal recommendation to my patients and not a “hard and fast rule”. The highest risk window for infection is during the first three weeks after insertion, and so I think it makes sense to minimize foreign bodies in the area during that time (although I concede that there is not a particular study that has been done to address this issue one way or the other). -Dr. Dervaitis

My spotting is finally starting to lighten up and I could tell where my period was last month through the spotting. I have not gotten my period yet this month. Hello Dr. Kristina, I asked a doctor about the copper IUD and he was talking to me about sizes? He said if the wrong size is inserted, it may not work. I have never heard anything about sizes when it comes to IUD's. Do IUD's have sizes?
Several IUD companies have marketed a slightly smaller IUD geared towards patients who have not yet had children. They may be somewhat easier to insert in these patients (although it is also possible to use the “regular” sized IUD’s). The smaller version of the hormonal IUD available in Canada is Kyleena (5 years). Some of the smaller copper IUD’s may only be effective for 3 years (check with your doctor when discussing which particular IUD is prescribed). -Dr. Dervaitis

Hi Dr. Dervaitis. I am actually one of your patients. :) I'm 54 and have had a Mirena IUD for 3.5 years. I have had blood (erythrocytes) in my urinalysis a number of times over the past 2 years. I just had a workup done to be a living kidney donor and I was disqualified due to erythrocytes in my urine even though I was not menstruating. Could the IUD be causing this? Thank you.
Hi Leanne- The IUD does not cause hematuria (unless someone is having menstrual vaginal bleeding that gets into the urine and causes a “false positive”.) It must be frustrating to go through the difficult decision making process of deciding to be a kidney donor (what a generous gift!) only to be disqualified. I’m sorry to hear this. Many (although not all) patients with a copper IUD report heavier periods that may be associated with more cramping. I'm really stressed about deciding whether to try an iud or not. I started using birth control due to incredibly heavy periods, and so far nothing has really worked. I'd say I've been bleeding daily for the past year and a half (I'm on iron pills don't worry), and have tried every pill variety that my doctor thinks is good for me and the depo. The depo shot has increased cramping and bleeding tremendously and it isn't slowing down. My doctor thinks that the next step is an iud, however, I'm not gonna say my age but, I'm very young. There is a doctor that she knows who specializes in placing iuds of young girls who haven't had sex or given birth. But I'm really worried about the pain and whether or not it's gonna work for me. So far it feels like nothing is working no matter how much time I give it, what if I go through this procedure and it just ends up being like everything else after several months?
Hi Lee- I’d recommend that you watch video #6 “IUD Insertion Step by Step”. In it, I explain the procedure (and that it’s not necessary to have had children or even intercourse ever prior to insertion). I have placed IUD’s in adolescents as young as 14, and many MANY who have not yet been sexually active. I can understand your frustration at the thought of trying yet another treatment without success. Unfortunately, there is not any way for us to predict what response any given patient will have to the levonorgestrel IUD. However, statistically, most patients notice an improvement in their bleeding (it tends to work well because the medication is delivered directly to the lining of the uterus where it needs to do its work...) I think I missed responding to this question earlier, my apologies. Bleeding can sometimes be heavy immediately after an IUD insertion, but it’s not typical. Sometimes it depends on how heavy your cycles are prior to the IUD insert (and what time during your cycle the IUD was inserted). Dr. Kristina Dervaitis I’ve had my IUD for about two months now and I started bleeding a day after it was inserted. Now two month later my period seems to come every two weeks and last for two weeks lol so I only have about two weeks out of the month . I’m gonna see how it goes through the next two more months to see if it changes but if not I’m going to have it removed . A friend of my family’s just went to have hers removed and they can’t find it . This is very scary to me .

Based on what you’ve told me, I suspect that you’re one of the 30% who will not have a period (or maybe only occasional/ sporadic light bleeding) with a levonorgestrel IUD. That said, I often will tell patients that the first time they miss their period, it doesn’t hurt to do an at home pregnancy test for further reassurance (and especially if there were any concurrent signs of pregnancy like morning sickness/ nausea or breast tenderness...) But thereafter, it’s not necessary to do this sort of test every month (if you continue to NOT have a period). The chance of pregnancy with an IUD is very rare (2/1000 with a LNG IUD). Definitely you should attend your scheduled follow up appointment with your doctor.

I've had a Mirena for a year now. At first when i got it, i bled for almost 4 weeks straight, at my check-up my dr didn't express much concern over the bleeding or cramping that I was having. Then for the next 9 months i struggled with bloody discharge for 3 weeks and then one week normal discharge. I was then prescribed Mireille and a clot forming medication to clear that up. I was prescribed the Mireille for 3 cycles and the bloody discharge was only for about a week and a half. But when i went off of the Mireille, within the same month i got a heavy period with heavy cramping. It almost feels worse having the Mirena.
I want to thank you for making these videos, Doctor. I got my kyleena yesterday and I watched a ton of your videos beforehand and it made me feel a lot better. I remembered your insertion video and how it took you about 15 seconds, luckily my gyno was just as quick! It hurt like hell but it’s over now. Thank you for what you do!

Sometimes bleeding can return after having been without menses (“amenorrhea”) on a levonorgestrel IUD. It’s difficult to predict what the pattern of bleeding would be, moving forward. Any time there’s a significant change in bleeding, I recommend that the patient have a follow up appointment with their doctor, in order to check that the IUD is still in proper position, and to rule out any other causes for the bleeding.

When you have an “IUD change/ replacement” (immediate new IUD insertion after old one is removed) you are immediately protected from pregnancy as long as the old IUD is not overdue for removal (not expired). However, because of the small risk of expulsion after a new IUD insert (ie, risk of the IUD falling out), to be cautious, I ask my patients to use back up contraception until they see me for their one month follow up visit when I check that the IUD has remained in good position.

Very informative. I am nervous because I am a week late on my period and I have no clue is it because of pregnancy or this new IUD. My old IUD was 1 month expired before I've gotten an appointment to change it. So I am very confused. I've gotten one positive pregnancy test on a blue dye test, which we all know gives out false positives. So VERY confusing

The amount of progesterone in your bloodstream with depo is far greater than what enters the bloodstream with a levonorgestrel (hormonal) IUD. If you were experiencing any progesterone related side effects on depo, they may decrease or no longer be present. If your cycles were very light (or if you had no cycle) with depo, there is a chance that you may have the same menstrual side effects with the hormonal IUD (although some women with no cycle on depo might go back to having a light cycle with Mirena or Kyleena).

I was so concerned about my bleeding, my doctor didn't go into too much detail about the aftermath of having the Cooper IUD. Thankfully my bleeding stopped at day 5 from insertion. However, I'm still having cramps, and I sit for long periods of times. I find that my left side ovary/uterus is very tender and in discomfort. I worry about the IUD perforating my uterus (I think I'mbeing paranoid). Maybe, if you do a video similar to the bleeding but this time about cramps, pain and shifting of IUD, or debunk the miths, please!😟

Hi Abrilia- Keep in mind that not every patient has heavier or more painful cycles with a copper IUD, so I would just see how things go. If you’re not happy with how your periods are with the copper IUD, you could later switch to a lower dose levonorgestrel IUD (you might not find as much acne as you had with the transdermal implant). Regardless, you will be protecting yourself from unplanned pregnancy using one of the most effective options available!

Some women have irregular or skipped cycles for various other reasons (other conditions or situations that interfere with ovulation). I’d recommend that you see your doctor for further evaluation if you are concerned. If there is any chance you might be pregnant (if you’ve been sexually active since your last normal period), do a pregnancy test.

Hello-
It is definitely possible (and safe) to use a menstrual cup with an IUD. (If you search this topic on Instagram there are a lot of helpful posts with illustrations and tips. The patient just needs to be cautious when manipulating the cup so as not to inadvertently pull on the IUD strings (usually if the cup is placed low enough in the vagina it’s not a problem).

I got mirena, and I had one day of pretty heavy bleeding at first, then it lightened. Now it's very light, and easily ignored. I have light cramps, but nothing as bad as it used to be before the iud. I'm quite happy with mine. I had a period a month straight before getting it.

Ugh. I’ve had an iud for about 6 months and have had horrible luck. Heavy bleeding for about two weeks at a time with severe debilitating cramping. I tried to hang in there as long as I could because everyone else I know who has it loves it. On the pill I use to get very light bleeding for about three days and pain the say prior to my period and the first day. I’m finally getting it removed this week. thank you for all your information!

Glad I found your channel and hopefully you can give me some info in regards to my situation. I had the Paraguard inserted in 2008 and went to have it removed in 2014. The midwife that did the removal failed to inform that it broke. My life has been changed physically by this. The broken piece has been embedded in my uterus for 4 years making it a total of 14 years. I’m currently working on trying to have it removed, but I keep getting passed around to different gynecologist. Can you shed some light on any possible long term effects this may cause me in the future? Thanks in advance
A small portion of the copper IUD is not likely to cause any health problems (although could interfere with conceiving if one is trying to get pregnant…) There have been many cases of patients forgetting about their IUD and the IUD staying in place for years past their suggested duration (sometimes decades) without major consequence. Generally it is still recommended to try to remove the IUD (or portion of IUD).

I had harmonal IUD (Mareena) inserted 4 days ago, during 3rd day of the period. So far no issue of extra bleeding or prolonged periods, infact it remain same as before. Just finished now. No cramp, no headache etc. So far.

I am going on vacation with my husband in 3 weeks and I got my period couple days ago, I am supposed to get my Paragrad (copper iud) when I get my period but I am hesitant to do it now because I don’t want to ruin the vacation. Do you think I should wait after the vacation to get it? Although my period is very light you could call it spotting. I also want to ask if the bleeding continues for a long time after insertion with women of light periods?

I can’t really answer this question, there’s no way to predict precisely how much bleeding a patient will have (or for how long) after an IUD insertion. Refer back to the video for general information about post insertion bleeding. Glad you found the video helpful!

I just had my iud (Mirena) placed in two weeks ago. I’m still lightly bleeding. Thank you for letting me know this is normal. I forgot to ask my doc about it. I had the copper one years ago. I have PCOS. My husband had a vasectomy done so I don’t need it for bc just to he,p with the PCOS.

I just got my Kyleena (first ever IUD) 3 days ago and my cramping is pretty intense, much more than I have ever experienced!! Ouch. Probably an 8/10 at times, Advil helps. They do come and go and it is not a constant pain, so that is good news. I assume the adjustment period could be a few weeks until my uterus gets adjusted? No spotting or bleeding yet post insertion!

@@tamisfincher I gave it a good 6 months, but ultimately it was not the right fit for me. I couldn't get over the idea of having the IUD inside me lol. However, post insertion I didn't have any bleeding and was period free for the entire 6 months! Cramping went away after a week or two. I would've stuck with it longer if I wasn't such a worrier. I am back to taking the pill but progesterone only.

Thank you for this Dr! I had the copper IUD fitted about a year ago. In January I missed a period due to exercise and fasting, and since mid Feb I have been bleeding continuously for about 6 weeks! Spoken to my Dr who didn't seem worried, but is this normal? I have no pain, no discharge, no cramps, i can still feel the stings, nothing at all sinister...
I'm not keen to switch, as I really don't want the hormonal forms of contraception. So I'm trying to be patient.
The blood looks fresh, and at some point in the cycle there was some clotting which has now stopped! But i still have some mild bleeding.
I had literally no issues with it upon insertion and no issues for the first few months! But I have noticed continued bleeding after missing a period (after fasting/exercise - what I'm experiencing now is the 2nd occasion since having it fitted).

Thanks so much for this video. I had my Mirena inserted 4 months ago (replaced my Copper T with Mirena) and I'm bleeding constantly but a light bleed. I had a dermoid cyst removed two months ago so not sure if my hormones are out of wack. I see you recommend using an oral contraception concurrently with Mirena. Does this usually help patients with the prolonged bleeding problem? I'm keen to request a script from my OBGYN. I used Tranic for 5 days and it didn't help at all so I'm wondering if OC will help at all? Thank you

Everything is good except for the fact that I have constant spotting (bleeding in between periods) ALL THE TIME. It’s been more than 2 years but it doesn’t seem to stop. I’m really bothered by this, and I don’t know if this is the message that my body’s trying to tell me that it doesn’t like the IUD and want it to be removed. I bleed so often, like 20 days out of 30 days in a month. Is there something wrong? Should I be worried? Should I remove my IUD? I’m 20 and I’ve never been pregnant. I think I have the Mona Lisa mini that’s supposed to last for 5 years. I also locate in Ontario.

Sorry to hear about this spotting, you must be very frustrated! While it’s less common for spotting like this to persist past the first few months, it does sometimes happen. Based on the timeframe you describe, I don’t think that there’s anything to be worried about. However, since the spotting is bothering you, I might suggest that you try switching to a Mirena (or even Kyleena) IUD. Although spotting like this is still possible with the hormonal IUD, the overall bleeding tends to be less (due to the levonorgestrel thinning out the lining of the uterus) and you might have better luck. This spotting is NOT necessarily an indication that you can’t continue to use an IUD.

Most IUD’s are to be used for 5 years and not 7 (but I’m not sure exactly which IUD you have, and in some situations the duration of use depends on patient age at time of insertion...) So my answer would be: “see your doctor to get things checked out”.

Thank you, I feel relieved to have watched this excellent video, I’ve had 3 polyps removed with myosure then mirena inserted and am 58 had 3 kids and thought all was well in menopause however I was wrong when heavy bleeding started up again
After my hysteroscopy and above procedures I’ve been shocked by more quite heavy blood loss and wasn’t prepared at all

I got my copper IUD fitted last Wednesday and I had cramps and bleeding for the first few days, which resolved after three days. However last night I had extremely severe cramps for about an hour with about 5 minute intervals between each. Today I am not having any cramps but feeling tired and a bit sore. I've also noticed a slight yellow/ brown discharge. Do you think this is something to worry about?

First Mirena period for me last week added more days and heavy blood loss. My RBC is low beginning to think my dr is crazy. Wish we had dr like you in Northern ON. Thanks for all your videos. Thank you so much for sharing. It’s now been 7 days since I’ve had the mirena in and I already want it out. Still bleeding and painful cramping 😢 I only have it in to help managing painful and heavy periods. I want to go swimming next week, if I’m still bleeding can I use a tampon? (It’s the middle of summer time here).
There is not any solid evidence to say that tampons should be avoided after IUD insertion (although it’s been my practice to advise my patients to wait 3 weeks before using tampons because that’s the window of time where there is more of a risk of infection, albeit a very low risk).

I got a Mirena inserted at a submucosal fibroid removal surgery. After two days of surgery I got an infection and given antibiotics for ten days, but my VOG asked me to stop them on the fifth day.
It’s been four weeks now and I was recovering well upto 3rd week and had a little fast jogging(3rd Oct) as I was feeling good with no swelling, bleeding and pain, after 15 minutes I felt a pain on my left lower tummy and I headed back home, then my tummy significantly swelled after I got home,
it improved by the following Thursday, but next Sunday(10th Oct) and Monday whole day that left lower area had a continued pain(bearable), and light bleeding could be seen.
Dear doctor, my question is,
can the Mirena be touched the fibroid removed wound area and not healing that patch? and giving this trouble?
Shouldn’t there be a reason for this pain?
(Kind note: I am scheduled to see my VOG on 25th Oct to see the placement of Mirena.)

i have taken out the mirena after 9 months, I kept bleeding 50% of the time. did keep getting cramps from time to time and then bleed. I prefer a heavy period over this cramps and blood all the time

During the 4-6 week adjustment phase the bleeding is usually lighter than a period, and sometimes just spotting or discharge (and some patients might not bleed at all). In a small number of women the bleeding might be heavier (like a period). If you are concerned about excessive bleeding, see your doctor.

i had paraguard inserted nearly two weeks ago and havent had a day without at least light bleeding (it’s extremely light but nothing too bad) but sometimes it goes medium light for like an hour and that’s it

Why is tampon not recommended for the first few weeks after the insertion please?
This is my personal recommendation to my patients and not a “hard and fast rule”. The highest risk window for infection is during the first three weeks after insertion, and so I think it makes sense to minimize foreign bodies in the area during that time (although I concede that there is not a particular study that has been done to address this issue one way or the other).

I’ve had my Kyleena IUD for about 5 months now and every other time after intercourse I bleed for about a day or two. I don’t experience any pain during intercourse and I can feel my strings just fine. I’m not sure if it’s something I should be worried about or not.

Based on what you are describing, it doesn’t sound worrisome. But as I mentioned in the video, when I see a patient with this issue, I do an evaluation that includes infection screening. It might be a good idea to see your doctor for a check up if the symptoms persist.

My spotting is finally starting to lighten up and I could tell where my period was last month through the spotting. I have not gotten my period yet this month. Should I be concerned about pregnancy or am I most likely in the 30% that don’t get periods anymore. I have my follow-up in a month and I was told not to worry about checking strings so I worry all the time about the state of my iud.

Passing blood clots. I woke up in a pool of blood and passed out. Doctors won’t remove it since my body needs to be adjust it. I’ll remove it myself.
Hello Dr. Kristina, I asked a doctor about the copper IUD and he was talking to me about sizes? He said if the wrong size is inserted, it may not work.
I have never heard anything about sizes when it comes to IUD's. Do IUD's have sizes?
Several IUD companies have marketed a slightly smaller IUD geared towards patients who have not yet had children. They may be somewhat easier to insert in these patients (although it is also possible to use the “regular” sized IUD’s). The smaller version of the hormonal IUD available in Canada is Kyleena (5 years). Some of the smaller copper IUD’s may only be effective for 3 years (check with your doctor when discussing which particular IUD is prescribed).

Thank you so much for video...I had my copper iud after 10 days of menstruation 3 days back and soon after insertion I had bad cramps and lower back pain and continues spotting..and mucus like thing and sometimes there's not blood but too much amount of clear water that wet my underpants and jeans.... feeling uncomfortable...

Hi Dr. Dervaitis. I am actually one of your patients. :) I'm 54 and have had a Mirena IUD for 3.5 years. I have had blood (erythrocytes) in my urinalysis a number of times over the past 2 years. I just had a workup done to be a living kidney donor and I was disqualified due to erythrocytes in my urine even though I was not menstruating. Could the IUD be causing this? Thank you.

The IUD does not cause hematuria (unless someone is having menstrual vaginal bleeding that gets into the urine and causes a “false positive”.)
It must be frustrating to go through the difficult decision making process of deciding to be a kidney donor (what a generous gift!) only to be disqualified. I’m sorry to hear this. Most importantly, I trust that you will have further investigations/work up for your hematuria.

Many (although not all) patients with a copper IUD report heavier periods that may be associated with more cramping. There can be some breakthrough/ irregular bleeding- particularly in the first 4-6 weeks after insertion (as things are adjusting). Please see video #7 and #11 for more info.

I'm really stressed about deciding whether to try an iud or not. I started using birth control due to incredibly heavy periods, and so far nothing has really worked. I'd say I've been bleeding daily for the past year and a half (I'm on iron pills don't worry), and have tried every pill variety that my doctor thinks is good for me and the depo. The depo shot has increased cramping and bleeding tremendously and it isn't slowing down. My doctor thinks that the next step is an iud, however, I'm not gonna say my age but, I'm very young. There is a doctor that she knows who specializes in placing iuds of young girls who haven't had sex or given birth. But I'm really worried about the pain and whether or not it's gonna work for me. So far it feels like nothing is working no matter how much time I give it, what if I go through this procedure and it just ends up being like everything else after several months?

I’d recommend that you watch video #6 “IUD Insertion Step by Step”. In it, I explain the procedure (and that it’s not necessary to have had children or even intercourse ever prior to insertion). I have placed IUD’s in adolescents as young as 14, and many MANY who have not yet been sexually active.
I can understand your frustration at the thought of trying yet another treatment without success. Unfortunately, there is not any way for us to predict what response any given patient will have to the levonorgestrel IUD. However, statistically, most patients notice an improvement in their bleeding (it tends to work well because the medication is delivered directly to the lining of the uterus where it needs to do its work...)

Bleeding can sometimes be heavy immediately after an IUD insertion, but it’s not typical. Sometimes it depends on how heavy your cycles are prior to the IUD insert (and what time during your cycle the IUD was inserted). If the bleeding doesn’t slow, I’d recommend that you see your doctor.

Now two month later my period seems to come every two weeks and last for two weeks lol so I only have about two weeks out of the month . I’m gonna see how it goes through the next two more months to see if it changes but if not I’m going to have it removed . A friend of my family’s just went to have hers removed and they can’t find it . This is very scary to me .

Generally speaking, “adjustment phase” bleeding can sometimes last longer than 6 weeks (sometimes up to a few months) and the amount can vary.
