    i had my rhinoplasty on Monday and they took my cartilage too. It’s been very painful 😣 i can hardly move without pain. they also left something to drain the blood with. my recovery has been very tough, hope it gets better soon.

    Dr. Yoo what are the chances of complications when using the rib cartilage?
    I read about possible complications such as:
    - Harvest site complications including chest pain (including chest pain)
    - Risk of warping of the implanted cartilage over time, deforming the appearance of the nose
    - Pneumothorax

    Hello doctor I'm from India and I'm 21 y/o female...my nose looks snub and more doll like but I am not happy coz I want a bit higher nose bridge and tip...     Your work is beautiful! If after the rynoplasty the person gets a nose injury, an accidental punch, or hit with a ball etc what would happened? Would the nose fall off? Would the rib implant will be lost? Can you please explain.

    I've noticed that most the videos are in respect to Asian/Asian American. I am Native American, but I am aware that my culture has an Asian influence.     A doctor suggested using rib cartilage for septoplasty (my 2nd). What's the risk of doing this? Is the rib weakened? How much is removed? Does it grow back in the rib? 3 months to heal the rib or?

      Hi Ian Leahy
      During rhinoplasty with rib cartilage, typically a small portion of the cartilaginous segment of one rib (4-5 cm) is harvested. With proper surgical technique, this will leave only a small, linear scar (~1 cm) as a sign of the surgery. Most patients will be able to return to light exercise at 2 weeks, and full exercise at 4 weeks after surgery.

      @@tonato17 Rib cage has been permanently damaged. Scar and hole left in my chest. Multiple unnecessary implants in my nose. I have suffered for a very long time and was left with an expensive mess to address. Multiple corrective surgeries to undergo. Strongly do not recommend this procedure.

      @@badrhinoplasty7994 Thank you so much for your answer. I really appreciate it and it means a lot to me. I had a septoplasty a few months ago which left my breathing in a very bad condition. It wasn't bad before the surgery and now my breathing is a mess which made me very sad and upset. Willing to correct it I thought of rib cartilage grafts..but after your msg I will reconsider other options!
      How is your nose and your rib cage at the moment? Thank you again for sharing your valuable experience, I wish more people would do it!

      The rib cartilage harvest will leave a scar and depression in your chest wall. The scar and depression can potentially be minimized, but there will be a scar and a depression. In women, the surgeon might hide the incision in your breast fold. Your rib cartilage is hard and bone like, much harder and more rigid than your nasal cartilages. When you dissect a nose and sew transplanted cartilage grafts from your rib-cage into your dissected nose, the grafts do not become soft like your natural nasal cartilages are, they remain hard and rigid, just like the cartilage was in your chest. You are going to have hard pieces of cartilage inside your nose that act as structural implants. These hard pieces of cartilage sewn into your nose can be irritating and painful, to very irritating and painful. You are risking your health and well being under-going a procedure of this type, especially when it is unnecessarily done for cosmetic purposes.

      @@tonydisabatino Yea, and no... I mean, I had a 4 hour reconstructive nose job with Rib harvest... Ok, So I did have pain for a few months, and I was scared it would never go away.. I mean the pain under my breast from the rib harvest.. it eventually went away. A scar ? yea, I'm fat so my breast flaps right over it so it's not noticeable... However, there is a scar!!!! a big one lol...Now to the nose. My nose was solid like a brick for months ... I mean I'm a year out , and the tip is someone hard...But I can honestly say it feels like my nose now. its' not to hard! if you massage your nose regularly you will soften the bone up ...My nose is pretty soft ... so yea.... it took a year for it to get this way...

      @@thinline8504 sorry, my wife just had rhinoplasty 2 weeks ago using rib graft. She feels pain in her incision when she moves, and clicking sounds in her chest, although pain when she deeply breath. Did you experienced such things?

      @@husamaleryani5349 no, but every recovery process is different. The rib pain should go away though... I think I remember a clicking noise too... it was weird I mean the rib recovery process was by far the worst for me...Like the pain would come back at random moments.. Maybe wait a month or so, and then if she continues to hear it then maybe you should contact a doctor ...

    Hello Dr! I have 2 Questions Im From Dubai and I might come to LA on January. I did a Rihonoplasty 6 months ago and I'm not satisfied with the result I went back using Filliers to have higher bridge even thought fillers have once enfaced my nose because of the number of times i keep adding fillers to build up the bridge.
    1. Is it okay to do another surgery using carlatge to build the bridge of the nose after 6 months from doing the first Rhinoplasty? (Im really not satisfied at all because of the bridge)
    2. How much surgery time needed to do just a graft (Asian Rhinoplasty)?
    I would really appreciate your help by answering.

      Hi Sara Alsari,
      1) Normally it is safest to wait around 9-12 months after rhinoplasty to consider undergoing a revision rhinoplasty. As long as you are otherwise healthy, and not smoking or use nicotine-containing products, a revision rhinoplasty can be performed in a safe manner.
      2) Surgery time will depend on how much augmentation will be performed and what, if any, changes will be necessary to the tip of the nose but will generally range from 4-6 hours.

      So there is no way i can do the surgery on January? (6 months passed since my old surgery)
      The main reason is I don't visit the US often and Im from Dubai, There for its very hard for me to be there in LA any time, but i will visit in January and i really do believe that you are the only doctor that could actually fix my nose.

    Hi, Dr. Yoo!. I really admire your work and i am looking forward to get a Rhinoplasty using diced rib cartilage fascia done by you. However, i would like to know how long is the recovery time after the procedure since i am only allowed to get 15 days max of leave from work. Is there going to be visible bruises and stitches 10 days after the surgery? I would appreciate your reply to my query. Thank you.

      Most patients will feel comfortable returning to work or school after 2 weeks of recovery. The nose at that point will still be swollen, but not look strange nor "surgical".

    Hi Dr. Yoo! Thank you for this education video. From your practice, do you see a higher resorption rate with donor rib graft? Or it is comparable to autologous cartilage. I got an augmentation rhinoplasty this July with donor rib graft in Boston. The result is great so far. I’m very worried that I will see resorption and some people claim the graft can be absorbed 50%. Im very confused with all the mixed information. Apologies for the long comment. Any comment is greatly appreciated ❤

      From a theoretical perspective, donor rib cartilage is not viable tissue since it is coming for a cadaver, and it is also be radiated to ensure there is not viable tissue. Given this, it most certainly resorbs at a much higher rate than autologous tissue since it will never be a living, viable part of your nose, since it is by its very nature non-viable material.
      That being said, if your nose is looking great, enjoy it and simply observe it. If it continues to look great, leave it alone. If it does not, then that's when you can consider other options.

    Hi Doctor , Here is a patient from China. actually after two weeks i am gonna do surgery on my nose . My hospital gives me a way to do surgery is use my Rib cartilage, for just in case to reduce the curve possibility, They will chop my Rib cartilage as many tiny cubes and use my fascia to cover it . Then put in my nose . do you think is that good plan ? or you think just do traditional way like just go straight to use rib cartilage then carve . Your advice is important to me .       Donor cartilage is safe in terms of not causing any increased risks of infection or skin ulceration, but it is not safe in terms of creating a permanent result. Donor cartilage is irradiated, meaning it is not living tissue. Therefore, there is a high risk of resorption and the nose losing shape.

      It's important to wait until after you son is nearly fully grown to pursue rhinoplasty. For males this is typically around 16 years of age.

      vidvidvid I know I’m stepping out of bounds here, but please don’t let your perception iof your son’s nose affect how you raise him. The tiniest comments can create insecurities in him for a lifetime. As long as he’s healthy and happy (at 8, I’m sure he has other things on his mind to pass the time the way a child should), he can make the decision for himself when he’s an adult.

      Cadaver cartilage is a potential option if your own rib cartilage is not suitable for use (due to calcification). However, cadaver cartilage is not living tissue as it comes from a deceased person, and it has been irradiated and treated to remove any viable or living cells. Due to this, there is a chance of resorption that you would not encounter with using your own tissue.

      Rib cartilage and ear cartilage are simply sources of graft material. Depending on the skill level of the surgeon and techniques used, the results will vary widely - not based on the source of the cartilage, but on the surgeon. The best approach for revision surgery will depend on your existing anatomy, and the aspect of your nose you wish to change.

      I feel that the safest material to use during rhinoplasty is material from your own body (autologous grafts). Gore-tex is a foreign material and will never become fully incorporated into the nose. This results in a higher rate or infection, migration and extrusion than with tissue from your own body.

      Some patients may be able to use cartilage from inside the nose (septum) alone, or in combination with cartilage from the ears. Rib cartilage is necessary when the baseline nose is lacking in projection and structure, but significant projection and definition is desired.

    Dr. it's 5 months after my surgery. I still feel pain. I know I'm a fat guy, so the doctor harvested rib from right under my male breast so the scar is not noticeable unless you lift it up.. However, when I workout, and do chest for example it hurts!! I mean I can barely lift weights due to the pain! will it ever heal?!

      On a male patient the rib incision will heal as a thin, 2 cm line typically in the crease under the pectoralis muscle. Occasionally patients will want the incision designed lower along the lateral border of the rectus muscle.

      This surgery left me with a 3.5cm visible scar, and an indentation and hole in the center of my chest. My incision was under my pectoralis muscle where my floating ribs meet my sternum. My chest has been in pain and throbbed, twitched, and gone through contractions for 8 years now and requires surgical correction as a result of this surgery. I am looking at stem cells to repair what this procedure caused. I would compare this surgery to being stabbed in the chest with a knife. As far as my nose goes, my nose is hard as a rock and bone like from 4 rib grafts. My advice as a male would be to avoid this procedure. If you have an athletic build, I would sincerely recommend that you avoid this procedure. No two people are alike, and all due respect to the surgeon in this video, but I only know this procedure to be harmful.

      @@tonydisabatino omg that is awefull i can imagine your nose feeling alot different and harder now because of the different bone its in your nose now. I got 2 surgeries on my nose first was a rhino second was a closed nose surgery to remove the hump he did not get rid of in the first surgery.
      Long story short the hump is gone and i have a dent now cause he scraped to much off so i was thinking about a revision to add some cartilage to fill up the dent in my nose.
      After i read your comment make me think twice cause the problems your facing i really dont want its like getting rid of one problem and get another back.

    I had rhinoplasty for 4 times already with silicon, I took the silicone out for 2 times because there was a big deposit of calcium in the middle of my nose ,then the mid nose got red and look like a hump because of the calcium deposit. It happened after 10 years. The third time was ugly nose and the last one was too long silicone, it was deviate and the nose got swollen. Then I took the silicone out for 4 years already.
    I thought that I would not have a rhinoplasty again cause I am almost 60. But now my nose tip is a bit dented and the middle is flat which can be seen more and more easily. Will the rhinoplasty using rib cartilage last for my life time? Will the nose be swollen or turn rad again after years? And the calcium deposit will happen again ?

      YES!!! NEVER USE SILICONE ! the minute you hear silicone run!! Good plastic surgeons would never use silicone in the nose..They used rib for my nose, and I love the results. Rib is the hardest bone, and the best when reconstructing the nose!

      I personally recommend strongly against using synthetic implants in your nose, as they have a far higher risk of complications than your own tissue, and they will not provide a permanent result.

      That's a great question, and I typically will not unless there are extenuating circumstances that warrant it, such as a patient with completely calcified rib cartilage that desires a significant change but is willing to accept the potential for resorption with cadaver cartilage.

    Hi dr. Yoo what is the difference between using silicone and rib cartilage as a support for the nose bridge?
    I heard, if using cartilage there might be some change to the nose shape over a long period of time as the rib cartilage get absorbed by the body.
    After the surgery can patient perform extreme sport? Diving etc? Thank you

      A dorsal implant inside of a nose serves the same purpose whether it is made of silicon or rib cartilage. The only difference is that an implant made from rib cartilage comes from a doctor cutting a piece of your rib cage off and carving the implant out of the piece of removed ribcage, versus a silicon implant that is manufactured by a company in a lab. They call an implant made of rib cartilage an Autologous implant because it is made from a piece of your body and is not foreign matter. You hear the term "graft resorption" although I find it impossible for a graft to dissolve inside of a human's nose. I only know grafts to remain inside of your nose exactly as they were initially sewn into your nose. Harvesting a rib graft is 100% going to leave you with a scar on your chest, and almost certainly a depression or a dent in your chest wall. The depression and scar can be hidden on women in their breast fold, where as in men the scar is visible and apparent. There are videos on RUclips of doctors harvesting rib grafts. Undergoing the surgery is not easy and takes many painful years to heal from, at which point you are still going to be left with a hard and painful piece or pieces of your rib cage inside of your nose. Some surgeons like to combine a dorsal onlay graft with spreader grafts as a "platform" for the dorsal onlay graft. The more grafts you add, the more complicated your procedure becomes. You are going to have to let your chest incision heal and your nose heal before attempting extreme sports. The rib grafting will likely leave your nose hard and bone like since the implant(s) are made from your own rib bone. A silicon implant has the risk of becoming infected and expelled from your body. Personally, from my own experience, I would not recommend either procedure although you will find plenty of surgeons telling you the exact opposite to get your money. Be very careful.
