      I'm currently a student for a surgical tech course in the Army and they taught us not have any part of our body hover over the sterile field because of microbial contamination. Is it different in the civilian side? You opened a peel pack right over.

      I’m training as a CST now and trust me it does you get a flair for it lol at first you’re scared then suddenly after practicing and practicing, without noticing it you just do it like it’s nothing it’s amazing.

    Your technique is fine. The guy critisizing you is one of those know it all scrubs. Every hospital has one. And they usually are limited in scope to gyn cases or general and most docs dislike them and the have no idea. Keep up the good work.

    This is badass I’m a 68D in the army and we do the laparotomy set for setting up the back table and it’s very enjoyable and interesting because the job is never boring

    It's just training and practice,it's good learning all the different specialities,also bear in mind different hospitals do things very differently,can be confusing doing agency cus they look at you as if you're wierd cus you do it differently to them, also bear in mind theatre is a very tense,stressful place and a lot of bullying goes on!! Unbelievable but true !!!

    You may not realize it or know it, but these videos are super helpful for us lay folks who don't get to observe all that goes on in the operating room. Even more beneficial is the fact that I'm totally blind, and I really appreciate how well you describe everything. I've been blind since birth, so it's no problem for me, but when people tell me that I don't need to know all these facts and such, that really bothers me. I understand that there are things that are better unseen, but when it comes to life saving surgeries and procedures, I think that spiritual vision is a absolute must!
    Keep up what you're doing, and thank you for all these videos. I'm still listening up for the anesthesiology machine. (Yes, I hear what I'm typing right now, and I use text-to-speech software called JAWS that reads all unimbedded text onscreen. Any depicted text inside a video is off limits, but Vispero is working very hard on developing more technology that'll hopefully make it possible to know more about pictures. Emojis are great, and some of them are downright hillarious! (winking face)

    First thing you do before picking up any sterilized instrument sets or consignments is to make sure that the indicator is visible on top and that it met the sterilization process. Filters and checking hole in wraps are secondary.

    It’s mind boggling how differently things are being done in Australia. For example our surgical techs are actually scrub nurses and everyone is pro turning around to fasten the gown.. you don’t actually go to a teching school you become a nurse and learn surgical teching on the job. You learn the instruments and how they relate to the anatomy and what the surgeon is doing on the job. Which is tricky as sometimes your surgeon won’t be as talkative or the nurse won’t tell you much in detail so how do we even learn properly?

    3 years with a license and still no ST job .. currently working in sterile processing ( I still have faith ) ...after it expires tho .. that’s it for me. 🙅🏾‍♀️

    The only thing that I see you forgot to do was put your goggles on before you did your surgical scrub. They were on your head but not “ready” for surgery. Other than that, it is helpful for me because I just started school this summer. I actually watched your videos when I was researching ST as a possible career change! Can’t wait to go through skills check off!

    Have worked as an EMS First Responder, however, after transferring to our CCT division, we transport many of our patients to surgery and for procedures, such as Cardiac Caths, Head Trauma, etc. The very first thing the surgical staff taught us, was to be aware of the presence of the Sterile Field(s), and NOT to touch them...and for Heaven's sake, do NOT sneeze on them, either!

    I just have the dumbest question. I've always wondered how the field stays sterile, once it's exposed to air. The open fluids, the instruments all out, how does this remain sterile?

      It's basically because they have a positive air pressure in the operating room, so their vents push air out of the room. This keeps and pathogens from entering the room, but it doesn't completely stop it. So they honestly have to set up as close to the surgery because it's a matter of time before the really are unsterile. (Sorry for the long, late answer)

    Pearl of wisdom, always count your instruments before case. Circulator may say it's not an open belly or it's all percutaneous, I always count to ensure that the set came to the or complete. If I discovered and have found discrepancies I let the circulator know about it. Central sterile might claim to you about missing instruments, but you're covered you and the nurse aware.

      I guess that depends on how early you scrub, if you have a lot of instruments to prepare, it's always best if you scrub early, but for minor cases, you can scrub once the patient is in the theater, so while the anesthesiologist is putting the patient under and intubating the patient, that gives you time to scrub and prepare your sterile field. In the hospital I work in, the scrub nurse normally scrubs a good 15 to 20 minutes before a major case starts, so that gives them ample time to prepare their sterile field. Hope this helps :)

    When setting up a case, how far in advance do you start setting up? When do you load your knife blades? You need at least one ready to go at the outset, right? When do you load needle holders? I know that instrument trays are set up according to count sheets, but generally speaking, what is on the count sheet? What I mean is, is the count sheet just what is needed for the case or are there also "extra" items that may be needed during the case?

      +Stephanie Lake on average 30min before a case starts. I load knife blades after I do the initial count. I usually don't pre-load needle holders unless I know I won't have time to do it during the case. Everything that is in an instrument tray is on the count sheet, nothing less, nothing more. If more instruments "peel packs" are added during the procedure, the nurse will write down on the sheet what has been added.

    Thank you for all the helpful and meaningful tips you give in each video. I am attending a surgical program this spring(January) and I am excited and anxious to get started, I am looking forward to clericals. I plan to complete my clericals with a guaranteed job. I am not yet, nervous because I am looking to make this a career. One quick question, by having a MBA in healthcare management but no patient care experience besides clericals when applying for a job. Would the MBA help, with a certification in surgical tech-to get hired quicker than someone with just a Certification? What are some tips you have in terms of studying for the certification?

    I see that you did a basic video on some of the ppe, can you do one on what type of shoes to use? since some hospitals can use OR detacated shoes with out covers. thanx

    This may be an odd question but while you're wearing all the scrubs, hairnet, face mask and such, does it get very warm while working?

      It does, but the average temperature in the OR hangs around 68 degrees anyways, so you really don't notice....unless you do pediatric cases, then you will be sweating like crazy all day.

    I've been told in school to spin when gowning. Also not to open it drop anything over the table. Was told to toss it into the field. Is this just how your facility does it?

      I heard the reason why from my instructor. She said that you shouldn't spin because your backside on the surgical gown is unsterile. And spinning exposes your backside to the sterile field.

      The idea is just to get things onto the table without extending any non-scrubbed parts of your arms over it that may bear loose skin cells. Tossing is generally not the best method because items can tumble off, but it's kind of nit picking. As for the spinning during gowning, you should step away from the field to avoid exposing your backside to it. The reason you rotate is because it's less choreography than the tag holder winding around you.

    Hey! I've got several questions about some of the little details in this video. Here goes.
    In the video you mentioned covering the table with towels or drapes. How do you do that safely as in without knocking things off or compromising sterility? Is covering the table done to be able to move the table to the field safely? I noticed you put the suction tip in the container that the saline was dispensed into. Is there a specific reason for that? Later you put something else in there as well. More suction tips, I think? The filters in the instrument tray. What exactly are you looking for as an indicator? I'm curious about the heparinized saline. In my lab, I'm used to fluids being in closed containers that are only opened at the immediate time of use. That freaked me out the first time I watched this video. LOL

      +Stephanie Lake reasons for covering a table would be to reinforce the table drapes itself from the instrumentation being placed on the table. You can do this easily by opening up with that in mind. Open everything you can on the far right side of the table and leave the left side open with nothing on it so when you scrub in you can lay out your 3/4 sheet or towels on the left side, then transfer everything on the right side of the table over to the left on top of the towels or 3/4 you have started to unfold. Drape the rest of your table out and you have a reinforced table.

      +Stephanie Lake suction tip is easier to install on the tubing when it's wet, so I always have my suction tips sitting in saline for that reason.

      +Stephanie Lake with the instrument tray filters I'm looking for any breaks, tears, or holes in them. If I find a hole, that entire tray would be considered unsterile. The sterile indicators are not on the filters themselves, but in the instrument trays.

      +Stephanie Lake heparinized saline is something that is mixed off the field, then given on the field by the use of a bag decanter. "Sterile tube" common concentrations would be 1000:1000 or 10,000:1000 hep/saline.

      Thanks so much, Shane. I should have known the suction tip answer. We do the same thing when we build equipment in our sterile processing department but with alcohol to make the tubing slippery. Then everything gets rinsed before it’s packaged and sterilized.

    Question: I did not see if the pen/marker you used was a one-time use sterile pen/marker. Was it a packaged sterile pen/marker? I bet the answer is yes. But, just curious.

      We had them in our custom packs. They're also packaged individually. Yes, they're thrown away at the end.

      Bigworld DDD delays can happen for all sorts of reasons. He strategically chose a smaller case so he could take his time and go over the basics.

    Does your hospital use "double decker" back tables with the small shelf above the main table surface? I saw them mentioned on line the other day and was wondering if you had ever come across them? What would be placed on the second shelf? Thank you!

      +Jesse Green our facility doesn't use them, but I have seen them in use before. Most commonly used in Ortho and spine cases where you would normally have 2 back tables, one for your normal sterile setup and another for all of your instrumentation for implantation and trials. The double decker table just combines it so you can have all the implant trays on that second level. Works great if your OR is on the small side.

      Jesse Green after you double cover, you’d have the instruments that you’d be using the most, closest to you. It’s also good to have a small table in addition to the double decker for all sorts of reasons.

    How many of the cases you typically work on are laparoscopic and how many are open? The one I'm having is supposed to be laparoscopic?

      Jesse Green 90% I would say are laproscopic, robotic, or minimally invasive of some kind. There are certain circumstances health wise, that force the surgeon to "open up" a patient, but it's becoming more and more rare.

    What if there's no second tech or circulator around when you pick up the instrument tray? How would you check??

      The sterile indicator should be viable in the tray. You look inside, if you can't see it, you don't grab it. Or you use one hand to move things in the tray to find it. If the indicator is OK you use both hands to pick it up. ( 20 yrs as a tech)

    Hi- i am just starting lab at my school and we are going over this exact thing. I have a question- when writing down med info, are we allowed to use a pen once we are gloved? Or is that something the circulator should be writing down for us? I really enjoy all your videos- started watching before starting school.

    Three quarter sheets are not considered "truly" impervious, so you should not use them to drape a ring stand (microbes will communicate if fluid spills and is left to soak on the three quarter drape for an extended period of time)
    Also, you should set an example and wear eye protection. Not to mention, your glasses could have fallen off of your head onto the sterile field.
    Also, you guys got way too close when opening that ioban. Large items like this should be dawned by a non sterile team member to ensure that you don't hover over the sterile field.
    Also, you guys were getting far closer than 12 inches in general.
    Also, you should ball your hands (in gown) prior to them being inside your gloves. The cuff is only considered contaminated once your hand passes through. So when you have it balled, your hands only slide though the cuff after it's completely covered by a glove.
    If this was a real case, talking should have been kept to a minimum- do a mock surgery next time.
    Also, phones are typically not allowed in the OR (phone in circulators pocket), let alone a video camera.
    Also, the sharps should be secured first.
    Also, a chemical indicator is usually taped on the count sheet, so you should also keep it for that reason.

      1. Why do we use 3/4 sheets to drape out a patient if it has the possibility to transfer microbes when fluid hits them? Go ahead and drop some saline on a 3/4 sheet tomorrow and tell me if it seeps through the sheet.
      2. You do not have to use eye protection when setting a case up. Its use as PPE is directly related when being exposed to potential bodily fluids. ie: When you are actually doing surgery. Do you just wear your safety glasses at all times throughout the day, never taking them off? The glasses were perfectly safe on my head, and if they fell off, that would be MY FAULT and I would have to breakdown the table and re-set up.
      3. I agree.
      4. You are right, that is how they tell you to do it in school.
      5. Personal preference.

      7. Depends on your hospital policy. We are allowed. I also got privileges from my administration to do these videos as well. You think I would just do this without anyone knowing?
      8.I have never heard of this as a standard. If you open your sharps in the same place every time this should not be an issue. Its a great thought for safety though.
      9. Why would I need to keep the chemical indicator after I have seen it? Don't understand the purpose of that.
      All that being said, I truly feel that you are a genuine internet troll, first I have had to deal with actually. I did not appreciate the comments you made on a previous video, and arguing with some of my viewers. Yes I read it all. You are coming on to these videos with the intent to just discredit and belittle everything I do. It seems that you have been quite jaded in your current position in CA. I work with a team of people that appreciates me, and I appreciate them equally. We work as a team cohesively and effectively to give proper patient care in our OR's. You are a sour, disrespectful, and hateful person. I hope you find peace in what you do at some point in your future, but something tells me nothing will satisfy you.       Surgical Tech Tips 1) 3/4 drapes bleed through, buddy. Hence why the correct sequence for an extremity is: 3/4 drape, IMPERVIOUS PLASTIC U-DRAPE- LEVEL 4s, followed by an extremity sheet. Lap sheets aren't really applicable because you're working only on the abdomen. Why do you think in every major ortho case we use impervious plastic split sheets? BECAUSE LIQUID BLEEDS THROUGH. I've seen it hundreds if not thousands of times.
      2) so you're saying that you're going to monopolize other staff to put your eye protection on while they tend to the patient? Not to mention, get unnecessarily close to you, and shed more skin cells all over you? Or, you're going to scrub out and waste supplies, and put them on when you come back? It's about setting an example for future techs.. would you tell a student to follow your lead? Absolutely NOT. When you're under the gun, you don't ever do that- makes you look inexperienced. Patient arrives: "nurse, can you put on MY eye protection now"? Yeah, you see where I'm going with this? Beginner techs are watching you, and prolly thinking that that's acceptable, which it's not.
      3). Personal preference? Not really, actually. One is the correct way, the other is not.
      4) in theory, more talking INCREASES the risk of infection, and since we are so anal about aseptic technique, this case should be treated no different than a total joint case.
      5) I'm sure if that was your mom or dad on the table you would not want people recording a video prior to them going in/having the room open for a prolonged period of time. We treat all cases with the same delicacy, right? Or, are you deciding which one is more important than the other? It's less invasive, so I am going to give less of a fuck type thing?
      6) sharps should be secured first because in a big case, pins, hypos, drills, blades, sutures, burrs, etc- can easily get lost if you were in a pinch. Not to mention, if someone has to relieve you.. catch my drift? And losing an exposed blade because the patients on the table, and you have no time, everything is everywhere, surgeons are scrubbing in... let's count- oh shit I can't find one of the sharps that I opened because things are everywhere. These are all very real circumstances that every tech will face at one point in time. So, it's very important that they're not cavalier about preparation. Sure, open the sharps on the corner, great. I mean when you scrub in, establish a safe zone immediately.
      7) The chemical indicators should stay on the field so relieving techs can be certain that the sterility check checks out- at least on our end; Leaving them on the case cart is not an option. This mainly applies to relievers.
      Quit with the ad hominem, btw- it makes you look sour. Feel appreciated all you want, you'll still always make significantly less than a nurse. If you don't care about money, great. Maybe one day you'll actually want to start a family, and not have to take call every night/ work two jobs.
      What I'm saying is true, and you know it. You're just being critiqued at a level that you're not used to. Some of the things I mentioned, in theory, increase the risk of infection. Why do you think we minimize our movements? Why do you think we limit traffic in and out of rooms?
      You know what I am saying is correct, btw.

      Forsaken Individual I'm a student and we are taught to ball up the cuffs in our hands and my teacher will yell if we don't do that . Also we have to wear are protective eyewear while we are setting up . I think all your points are valid .

      Yes, you will find what works for you. However, it's nice when everyone sets up similarly at a facility. The reason is if someone has to relieve you out. I have let a scrub out and then try and figure out their set up. Usually, after awhile, new employees would set up like we did. I'm not talking about every single item, just in general.

      Kara Ogburn it doesn't matter where it is but it was a big mayo and those wraps don't cover the whole table so it just makes it easier to toss your gloves and keeps the unsterile mayo that is closest to you covered so there is less change of contamination

    Really informative guys! But it is really poor aseptic technique to drop or flick items onto the surgical trolley 😧 I was always taught to take the item from the scout nurse & if an emergency then open everything into a large splash bowl so as not to accidentally contaminate your trolley. As per standards. Also should always double glove & wear eye protection.

    #2 You should never have the white cuffs showing while opening your glove wrapper. The cuff is only to show when you are putting on the glove. Your bare hands should never enter the cuff because it's porous. You had your entire unsterile head over the top of the back table while making your med label. You didn't check your indicator in your pan to ensure sterility. Never shake supplies, the air isn't sterile. You raise your arms up to your head and the sterile area is nipples to navel. Your case is contaminated. I cannot watch any of your video's because they're incorrect.
