MedCram - Medical Lectures Explained CLEARLY I still think Vascular Endothelial Growth Factor is the crux in this disease doctor after researching more into your hypothesis. VEGF is shown in high levels in kawasaki disease and in COVID. Since it’s main role is angiogenesis and permeability, any elevation should, hypothetically, induce the severe symptoms noted in both diseases. VEGF is the single factor consistent in all the co-morbidities for COVID as well. Modulation of VEGF is key I believe. The cocktail I previously prescribed as an adjunct modulates VEGF while still increasing NO production. ADAMTS13, in an attempt to achieve homeostasis from elevated vWF, also elevates VEGF in a way I still don’t fully understand but am working on. Please look into this pathway. There’s a single trial already underway focusing on this pathway.

"A cohort of 10 critically ill patients with coronavirus disease 2019 (COVID-19) received more effective immune response after leronlimab (PRO 140) treatment compared with healthy controls, announced CytoDyn, developer of the drug."
"To disseminate results that can inform the public health in response to COVID-19, CytoDyn announced the publication of their findings in a pre-print manuscript, which has been submitted for publication and is currently under peer review.
A cohort of 10 critically ill patients with coronavirus disease 2019 (COVID-19) received more effective immune response after leronlimab (PRO 140) treatment compared with healthy controls, announced CytoDyn, developer of the drug.1
Patients treated under the FDA’s emergency Investigational New Drug (eIND) program with leronlimab experienced reversed hyper immune activation, inflammation, and immunosuppression. To disseminate results that can inform the public health in response to COVID-19, CytoDyn announced the publication of their findings in a pre-print manuscript, which has been submitted for publication and is currently under peer review. CytoDyn also shared the manuscript with the World Health Organization.
“We are now most hopeful the entire medical community will understand the potential benefit leronlimab can provide critically ill COVID-19 patients,” said Nader Pourhassan, PhD, president and chief exectuvie officer, CytoDyn, in a statement. “Moreover, this discovery by Dr. Bruce Patterson that leronlimab decreases plasma viral load may have tremendous long-term positive ramifications to bring this pandemic under control. We are grateful that we are able to release this research at such a critical time for patients throughout the world.”
Patients with critical COVID-19 were treated with leronlimab at the Montefiore Medical Center (n = 10) under the FDA’s eIND program. Patients showed evidence of lymphopenia with liver and kidney damage and had significant pre-existing co-morbidities. They received intensive care treatment including mechanical ventilation or supplemental oxygen. Patients received leronlimab on the 14-day study period.2
Four patients died during the treatment period due to disease complications and severe constraints on medical equipment.
At baseline, all 10 patients had elevated levels of inflammatory cytokines IL-1β, IL-6, and IL-8. Plasma CCL5 levels were elevated among these patients compared with those in both healthy controls and patients with mild or moderate COVID-19.
All patients received a subcutaneous 700 mg dose of leronlimab after baseline blood collection. Patients were monitored longitudinally for 2 weeks after treatment. As early as 3 days after therapy, investigators noted a reduction of plasma IL-6, which returned to healthy levels by day 14. More variable levels, however, were observed with IL-1β, IL-8, and CCL5. There was a marked restoration in CD8+ T cell ratio in the blood following leronlimab treatment.
A high sensitivity digital droplet PCR was used to quantify the plasma at baseline, in which SARS-CoV-2 was detected in all 10 patients, underscoring the severity of COVID-19. However, investigators noted a decrease in SARS-CoV-2 plasma viremia in all patients at day 7 following treatment, indicating that leronlimab may be effective in anti-viral immunity.
To establish an unbiased gene repertoire, investigators performed 10X Genomics 5’ single cell RNA-sequencing of peripheral blood mononuclear cells in order to evaluate the transcriptional changes between an uninfected, healthy person and 2 patients with severe COVID-19. They identified 2,890 differentially expressed transcripts, showing that the 2 patients had a greater abundance of myeloid cells upregulating inflammatory-interferon, and chemokine-related genes. Chemokines (CXCL8, CCL4, CCL3), inflammatory and immune activation genes (IL-1β, CD69), and the IFN-related genes (IFI27, IFITM3) were among the most notable overexpressed genes in the COVID-19 samples.
A downregulation was observed in the effector molecule granzyme A, as well as the immunoregulatory gene KLRB1 in patients with COVID-19 compared with the healthy control.
Investigators also aimed to identify markers that could inform the effectiveness of leronlimab. Approximately 2,037 differentially expressed transcripts were identified out of the 4,105 cells at baseline and 4,888 cells at the 7-day post-treatment timepoint in the longitudinal single-cell dataset. IL-6 transcripts were downregulated between day 0 and 7 in monocytes, which remains consistent with previous reports. Myeloid cells expressing chemokine and IFN-related genes were downregulated at day 7 post-treatment compared to baseline. There was also an increases expression of granzyme A, which suggests improvement in the antiviral function of leronlimab."
"These transcriptomic findings further underscore the potential impact of leronlimab- mediated CCR5 blockade on the inflammatory state in COVID-19,” the authors wrote. “In summary, we show here for the first time, involvement of the CCL5-CCR5 axis in the pathology of SARS-CoV-2, and present evidence that inhibition of CCL5 activity via CCR5 blockade represents a novel therapeutic strategy for COVID-19 with both immunologic and virologic implications.”

But they're very likely dutch state sponsored. They want to keep us dutch people as ignorant, in the dark and pliable as possible. Don't know why. More and more dutch medical practitioners are complaining about information restrictions, everything has to be according RIVM and thus WHO guidelines. Only the use of Quercitin is allowed, luckily.

I have been discussing how COVID-19 and ROS seem intimately related with clients who are aerospace & chemical engineers, geologists, exercise physiologists with the caveat that this is speculation and research is underway. Tonight I brought it up with a veterinarian with 1000s of hours of research experience and she became extremely interested. Her experience with feline corona virus exactly matches everything you have been describing. Healthy pink lungs surrounded by fibrosis in older cats. Younger (3 yo) felines full of thromboses. Cats have a particularly virulent form that fills the perineum with fibrotic tissue. All the fibrotic tissue of endothelium. Told her that absence of Angiotensin 1,7 was implicated and her response was “that’s what I’ve been saying!” She lamented that too many clinicians get lost in the symptoms that they lose sight of the root cause. Your work is paradigm changing and thank you for for sharing this with the world. Should the state of Texas allow me to return to my vocation supporting your group is at the top of my priorities.

As a layman, I'm curious as to how these hypotheses are integrated into the medical community for peer review, and consideration as a valid understanding of the workings of this virus. There appears to be a significant understanding presented by Dr. Seheult, of the characteristics of COVID19 that differ from the original assumptions. One would hope that the efforts presented here, along with other like studies, can be disseminated within the medical community toward an accurate and up to date understanding of this virus' effects. And as a layman, I fear the political aspects of this crisis may cloud some of the more innovative presentations of the pathology and treatment of the COVID19 virus. In other words, I hope there are people out there listening to this stuff. Dr. Seheult is my goto source for objective, analytical information that I can begin to comprehend. It's been many years since Dr. Rossmore's microbiology class at Wayne State, but I seem to be able to follow along quite well. Thanks so much for your efforts.

That said, I believe that perhaps the endothelial oxidative stress, modulated by NADPH Oxidase Nox2 observed in blacks is in actuality, primarily due to the fact that "NADPH Oxidase Nox2 Mediates Vitamin D-Induced Vascular Regeneration."
"Data from NHANES III show that, in the southern states, 53%-76% of non-Hispanic blacks (depending on sex and age) compared with 8%-33% of non-Hispanic whites had 25(OH)D concentrations below 50 nmol/L in the winter (4). The lower 25(OH)D of blacks and other groups with dark skin results primarily from the fact that pigmentation reduces vitamin D production in the skin."
In fact, many studies point to Vitamin D playing a strong role in Black health, "In a controlled trial of African women, a low dose (800 IU a day) of vitamin D resulted in a three-fold reduction in cold and flu symptoms compared to those given placebo"
In short, all dark skin people should be taking a vitamin D3 supplement.

Indigenous African populations like the Maasai in Kenya and Hadza in Tanzania (who live in an equatorial, tropical climate and spend hours outside in the sun) have an average vitamin D level of 119 and 109 nmol/L respectively. That is between 40-50 ng/ml www.ncbi.nlm.nih.gov/pubmed/22264449
From an evolutionary perspective, this may be the ideal vitamin D level we should all be aiming for.

The amount of sun a white person needs to acquire sufficient VitD is 3-6hrs per day naked under 12noon sun for 7-14 days. (per my endocrinologist). In other words, most people wont get proper VitD with sun exposure.
Deficiency also is more on how we barely go out now because hot sun and how our work is indoors. Diet is next.
The thing is, vitamin D can help, but its not everything. Genetics, lifestyle, and environment also has a major effect on body response to disease and pathogens.

    • @Eniral441
      @Eniral441 4 года назад +4

@@poopsiedear1127 This is one of the reasons anthropologists and others (I am an anthropologist) don't want to get rid of race as a biological descriptive...ridding only the social construct from which you speak.
Those whose ancestors lived in sunnier areas developed more pigment in the sun through evolution . This filters the sun some because not as much is needed for Vitamin D production and it also protects from the sun more. However, those in more northern regions of Europe didn't get as much sun exposure and lost their pigment over time. This made vitamin D absorption easier because it was needed to create the same balance. It is the body's ability to adapt to its environment. However, this is over a generational evolution that is necessary for these changes to be substantial.
Although those with darker skin are in many ways no different than anyone else, their bodies are different. Race is more like the clothing we wear, as a non-literal example. Someone in Namibia may be no different than myself in the mountain West of the US, but if I dressed as they do in January I would freeze to death. I would be super hot if I wore my warm clothes there. This is where an understanding of race differences in biology can be helpful.

One minor detail: G6PDH can really be considered part of the glutathione redox system, since it is what snatches electrons from glucose-6-phosphate to reduce NADP+ --> NADPH. NADPH, in turn is the electron donating cofactor used by glutathioine reductase to regenerate the reduced GSH from the oxidized GSSG.
One other point: ONOO- is peroxynitrite, not -nitrate.

As I understand , the Kawasaki is often treated with high doses of Aspirin as the best medicine to stop the inflammation. This NSAID is not a good choice by Covid?

Only in cases where fever is the result as it mask the effects of pyrexia and dampen immune response. Aspirin is fine to reduce inflammation but steroid therapy should be used as an alternative

Both Covid-19 and Kawasaki makes blood clots in arteries and veins. It seems like they use Aspirin on Kawasaki to reduce the inflammation.
They try to avoid the platelets to stick together. Blood clots in the lungs are widely a problem on both virus attacks. May be a bad choice on Covid-19, but mainly described as the best medicine for Kawasaki.

From the reference article mentioned in this video (www.ncbi.nlm.nih.gov/pmc/articles/PMC3595783/), "To investigate this hypothesis, they analyzed bacteria isolated from the oral cavity and duodenal mucosa, and clarified that gram-positive cocci with superantigen activity and gram-negative bacteria, which produce heat shock protein 60 and induce inflammatory cytokine production in peripheral blood monocytes in pediatric patients, were isolated from children with Kawasaki disease at a high rate, suggesting that these microorganisms act together and induce Kawasaki disease.50)"
The pathogens involved were gram positive cocci and gram-negative bacteria. The gram+ve cocci induced inflammatory cytokine storm which causes vasculitis in multiple organs.
What is the proof that there is a cross reaction with SARS-CoV-2 virus? Our body can generate cytokine storm in reaction to infection (from virus and bacteria), is this the case of co-morbidity?
From the same article, “The involvement of an infective factor in the development of Kawasaki disease has been assumed, but the cause is still unclear. A nationwide epidemiological survey of Kawasaki disease has been continued from 1970, and the total number of registered patients exceeds 240,000. The peak onset age is one year, and 70% of pediatric patients develop the disease at 3 years of age or younger. More than 10,000 patients have been reported annually in recent years, and the number of patients is still increasing.5) After Furusho et al.6)"
Note that most of the patients are under age of 5, peak onset age is 1 year, this is the time that children start getting vacinations, sometimes receiving multiple vaccines at the same time. How do we know that this is not the over reactions of immune response from vaccination?
Thank you for your informative video.

Would love to get your take on treating with recombinant ACE2 or ADAMTS13 (BAX930). There's an ongoing study looking at rhACE2 (Apeiron Biologics) that looks very promising, where the rhACE2 theoretically acts as a decoy for the virus to bind to AND provides the typical protective functions you've discussed about ACE2 in previous updates. Would love your thoughts on this.

This is very concerning. My grandson had Kawasaki disease about 3yrs ago, he has since recovered. They treated him with heart medication since they said it creates holes in their heart but since they are so young they heal quickly. His follow up visits cleared him of any danger.
I have just heard he is now having fevers and vomiting with pain in his abdomen. They had a video visit which the doctor prescribed a medication for the nausea. He is still running a fever. I hope this isn’t a recurring thing.

You seem to have missed two important points about "hot-cold" therapy. First, it is called "banya" and to be most effective it is extreme. A dry sauna at 150F for 10-15 min, a cold dunk in sub 50F water for 5 min, repeat two more times. I do the hot sauna for 15-7 min, but take a cold shower for 5 min each time. I doubt that a 14 min warm-cool shower regimen would produce much benefit. Second, you never explain how it helps immunity. Conventional wisdom is that it "squeezes" the lymphatic system, gets it moving and cleansed. It is the cellular level sewer system that cleans out inter and intra cellular debris. Perhaps you could explain this better.

Hi Dr. Seheult. When you mentioned Kawasaki's disease, you had my rapt attention. Over 40 years ago, my 4 month old niece suddenly got sick after having had a series of vaccinations. She ended up in a Boston hospital, where she died 2 weeks later. Her Drs were baffled by her illness and death, but sometime later, my sister received a letter from the hospital, saying they thought she may have died from a strange new syndrome called Kawasaki's disease, and that hers was one of the first cases in the country. We always felt it was related to the vaccines, and years later saw an article about a rash of Kawasaki's in the children of a village in Africa that had been traced back to vaccines they had received. I don't know if this information has any connection or significance whatsoever to these current findings, but I thought I would share it just in case. I have learned so much from your postings about Covid 19 even though I sometimes struggle to understand everything because I am not in the medical field and I thank you for your work. Stay well.

Kawasaki, rheumatic fever, share the same classically enunciated diagnostic criteria. Etiology of Rheumatic fever / Scarlatina paradigm, "allergic- immune response, following acute inflammatory response to streptococcus pharyngitis/ tonsilitis..in Rheumatic Fever, synovitis of large joints, ( knees ) are also a major feature, and Cortisone was first used in the fifties, to reduce that inflammatory DOLOR RUBOR loss of function.. CARDIAC COMPLICATIONS IN RHEUMATIC FEVER INCLUDE VALVULAR LESIONS.AND ENDOCARDITIS.

Very informative videos for doctors and rest of technical staff. However, you talk at length about the causative issues, like how oxidative stress is occuring, but you give no time or just skip the issue about possible practical ways and means to reduce this stress. After all,Covid maybe new but oxidstive stress, cutokine storms are not new phenomenon. While you go many years back to dig out articles showing causative factors, many of us will appreciate you more if you fo equal justice to current therapeutic options to mitigate oxidative stress, cytokine storms, thrombosis etc etc.

In areas where deaths from COVID-19 are on the decline, are there data available to show what can be attributed to those declines? Is it a matter of fewer infections? Love your updates! Keep your eye on New Zealand as well as Australia. We have a colder winter across NZ (even in the subtropical north) than parts of Australia do, which would confound their numbers a bit. Ultimately, though, looking at these two countries may not say much as even if the virus is seasonal, because they've been so successful at suppression and may continue to be so because of their determination. It doesn't seem to be taking off in the rest of the Pacific, though, where a few countries have cases (Fiji, etc.), and it doesn't seem to be taking off much in Africa, either.
