With the iron crown you could have gotten the flaute with 5 thanks to ballet shoes and bag of holding. That is an amazing combo!

I am actually suprised there are not any(many?) mods for this game. custom items and such would be kinda fun and something as simple as you said that just showed if they would flee (or just your total hp count) would be great.

33:53 RE: Katar + Triple Shuriken combo, here paste code for the fight (Modes > Creative > Paste), check the inventory to reorder the items. Tho I don't think you can proc the trio since the second Roulette's cantrip will always has chain regardless the item order.

Yeah, the ordering matters only for the current (to be used) side, whether triple sees its own pips as ×1 or ×2; But the previously rolled side will always count as its net value.
However, it's not strictly impossible to proc both at once. With Katar after Shuriken, you just need to triple off rolls with the chain bonus active, i.e. hitting cantrip on all 3 rolls for 4 uses (×1, ×2, ×2, ×6).
