This addresses many of the mistakes that I see companies make (thinking that the chart does the evaluation, misunderstanding ALARP, thinking that severity can be reduced, thinking that risk is a single point on the chart instead of a profile, not understanding that some risk controls transfer the risk to a different hazard or hazardous situation entirely, thinking that risk control is unimportant/optional or that it is ok for reasonable risk controls to be defective). All spot on. I am not sure I follow your distinction between likelihood and probability, unless you are equating one of these with P1 and the other with P2 or P total. It seemed like you had elements of P1 in both, and this may be either incorrect somehow or minimally, at least a little unclear.
I was confused by your use of "likelihood", "probability", and some of the other words around these on the slide near the end of your presentation, but I think that I understand how you are using these now. When I interpret these as follows (to be more consistent with the perspective of ISO 14971, though ISO 31000 would also be good), I understand and fully agree with the concepts. Perhaps the use of more common terminology in these areas would make your presentation even better: "outcome for a given scenario" to mean "exposure to the hazard", "exposure to the risk" to mean "hazardous situation" (or "the sequence of events that leads from initiating and contributing causes to the exposure of an object at risk to a given intensity of a hazard"), "likelihood" to mean "the probability of the occurrence of harm" (given that the hazardous situation has occurred) (which is P2), "probability" to mean "the probability of a hazardous situation occurring" (which is P1), and "consequence" to mean "severity". Then, the total probability (P total) associated with the risk of a given hazardous situation and severity would be the probability of the hazardous situation occurring (P1) multiplied by the probability of the occurrence of harm (P2) for the given severity.

Andy, A single pass, single dimension use of a matrix as shown does result only in varying the likelihood, as you say. The replies you respond to are also subject to this limitation. However it is possible to use a risk matrix to dynamically re-examine controls and mitigation on a multi pass basis which, used properly, should result in both measurement dimensions (ideally) being reduced. For example if we do the first pass with 'no controls and no training/specific competencies' (Gross Risk Rg), then as a second pass, apply existing hardware based controls and mitigations for a Nett Risk (Rn), then we can gauge for each scenario the effectiveness of the controls. Now a final pass, accounting for existing procedural/management system controls gives us our existing Residual Risk (Rr). In each pass, we can determine for the proposed 'work' what additional hardware, then procedural controls would be required to reduce Rr to 'Low'. In this instance, the controls should reduce the severity of the consequences first and the likelihood second - in line with whatever current 'hierarchy of controls' is in vogue. Agreed, this effectively means that the scenario is also changing - and this can mean that the likelihood actually increases! For example complex tasks can show a significantly reduced risk of fatality (consequence and likelihood reduced) but a significant Rr for 'LTI' for example. I know that the same results can be found by multiple single pass assessments (eliminate/replace hazard 1 for scenario2; repeat until Rr is 'low enough'), but this is often more difficult to tie together? Interested to hear your thoughts.
Gary. Sorry for the late response, I have only just seen your comment. I agree that a multi-pass approach to risk management is required, and a matrix can be used to assist this. But I am not sure that there is a robust method currently available to properly document the process you describe. For example, if your original plan is to use a MEWP but from the risk assessment you decide to use a scaffold instead you have to complete a new assessment because your method has fundamentally changed. That is not a problem, but it can be difficult to take credit for the fact that you changed the method. One concern I have with your suggestion is the aim to reduce risk to 'Low.' Of course this is what we would always like to do, but it is difficult to manage this expectation without people feeling obliged to 'manipulate' their risk assessments to the 'correct' result. I actually think the principles of ALARP address this problem, but for some reason these have not really been developed into a practical tool. I have been playing around with some ideas to address some of the points raised. I haven't had the time to develop them sufficiently, but I think a multi-dimensional approach, consistent with your suggestions, is what we need.
@@AndyBrazier Multipass and multi-dimensional analysis and assessment may be valuable, but again, spot on regarding risk control and ALARP. In my experience, people continually misunderstand what role risk reduction plays in terms of the acceptance of risk. The idea is not to reduce risk to a particular "level". The idea is to reduce risk using all reasonable actions. The acceptability of a risk is based on whether (or not) it has been reduced reasonably and whether (or not) the benefits outweigh the risks. Risk is not acceptable based on it's position on a chart. Risk can be low and unacceptable (if not reduced reasonably or if the risk outweighs the benefit) or high and acceptable (if reduced reasonably and benefit outweighs the risk). Risk matrices assess risk (for comparison and prioritization of risk reduction and for comparison to benefit). They do not evaluate risk acceptability.
This video has great content. However, each sentence fades off in volume. I tried turning my volume all the way but it was still a struggle to hear you from the middle to the end of each sentence.
I could fall out fall out an elevated work platform from 8meters ...Risk is very high with a fatality .but very rare,,.my control is now heights training ,certified harness and fall restraint ,full rescue plan ,barriers ,spotter etc, i see my control measure reducing the severity,as a fatality is no longer the likelihood. Gareth. We will probably have to agree to disagree. My view is that if you are working on an elevated platform the possibility of falling remains and if from 8 metres the possibility of fatality remains. The controls you list do not result in you working at a lower height, so the potential energy of your body remains. But they do clearly reduce the likelihood. The key point is that implementing those controls will reduce the likelihood of fatality so far that the risk (consequence x likelihood) is reduced to the point that it is not our main concern if there are other potential outcomes that have a greater risk. I would point out the following scenarios that apply to the activity with your controls: * Person at height may not attach their harness correctly; * The harness may fail in a fall * The anchoring point that the harness attaches to may fail * The elevated work platform may fail/topple over. Hence none of your controls can be considered 100% effective; hence the likelihood cannot be considered as zero and so there has to be some risk.
You are trying fudge in an additional risks that requires a separate control and separate assessment ...The severity of me falling has greatly diminished with the expected outcome a minor to severe injury ..Your scenarios mentioned are for untrained ,non-certified and unreliable staff and equipment. If i cannot reduce the severity of the outcome by implementing proper controls ,find somebody else to do the job.

This is why regulations and standards for medical devices (at least) recommend / require implementing risk controls in the following order: inherent safety, protective measures, information for safety. Inherent safety, properly understood, is complete avoidance of the risk, but this is not reduction of probability/likelihood or severity. It is avoiding exposure to the hazard altogether. In this specific hazardous situation, inherent safety would be something like allowing the work to be done without being at height at all (no risk because no severity because no exposure). Many times, though, even using inherent safety for one risk ends up transfering the risk to a different hazard or hazardous situation. For instance, using a crane or drone to do work at height instead of a person doing the work at height may introduce a suspended mass hazard in place of a fall hazard.
A matrix is a tool that you can use for certain tasks. If you find it useful to modify the matrix to cover more scenarios there is nothing to stop you doing that.
I believe that Andy's point about different risk matrices being used (instead of only a single one) is that different hazards may have different total probability (P total) or severity thresholds for high, medium, or low risks, especially if the thresholds on your matrices are intended to represent a surrogate for benefit vs risk analysis. This might also be affected by how you stack up risks from different hazardous situations within the same hazard. If the differences are small, maybe there is no value in using different matrices. If they are large, however, there might be significant value in using different matrices.

Andy, A single pass, single dimension use of a matrix as shown does result only in varying the likelihood, as you say. The replies you respond to are also subject to this limitation. However it is possible to use a risk matrix to dynamically re-examine controls and mitigation on a multi pass basis which, used properly, should result in both measurement dimensions (ideally) being reduced. For example if we do the first pass with 'no controls and no training/specific competencies' (Gross Risk Rg), then as a second pass, apply existing hardware based controls and mitigations for a Nett Risk (Rn), then we can gauge for each scenario the effectiveness of the controls. Now a final pass, accounting for existing procedural/management system controls gives us our existing Residual Risk (Rr). In each pass, we can determine for the proposed 'work' what additional hardware, then procedural controls would be required to reduce Rr to 'Low'. In this instance, the controls should reduce the severity of the consequences first and the likelihood second - in line with whatever current 'hierarchy of controls' is in vogue. Agreed, this effectively means that the scenario is also changing - and this can mean that the likelihood actually increases! For example complex tasks can show a significantly reduced risk of fatality (consequence and likelihood reduced) but a significant Rr for 'LTI' for example. I know that the same results can be found by multiple single pass assessments (eliminate/replace hazard 1 for scenario2; repeat until Rr is 'low enough'), but this is often more difficult to tie together? Interested to hear your thoughts.
Gary. Sorry for the late response, I have only just seen your comment.
I agree that a multi-pass approach to risk management is required, and a matrix can be used to assist this. But I am not sure that there is a robust method currently available to properly document the process you describe.
For example, if your original plan is to use a MEWP but from the risk assessment you decide to use a scaffold instead you have to complete a new assessment because your method has fundamentally changed. That is not a problem, but it can be difficult to take credit for the fact that you changed the method.
One concern I have with your suggestion is the aim to reduce risk to 'Low.' Of course this is what we would always like to do, but it is difficult to manage this expectation without people feeling obliged to 'manipulate' their risk assessments to the 'correct' result. I actually think the principles of ALARP address this problem, but for some reason these have not really been developed into a practical tool.
I have been playing around with some ideas to address some of the points raised. I haven't had the time to develop them sufficiently, but I think a multi-dimensional approach, consistent with your suggestions, is what we need.
@@AndyBrazier Multipass and multi-dimensional analysis and assessment may be valuable, but again, spot on regarding risk control and ALARP. In my experience, people continually misunderstand what role risk reduction plays in terms of the acceptance of risk. The idea is not to reduce risk to a particular "level". The idea is to reduce risk using all reasonable actions. The acceptability of a risk is based on whether (or not) it has been reduced reasonably and whether (or not) the benefits outweigh the risks. Risk is not acceptable based on it's position on a chart. Risk can be low and unacceptable (if not reduced reasonably or if the risk outweighs the benefit) or high and acceptable (if reduced reasonably and benefit outweighs the risk). Risk matrices assess risk (for comparison and prioritization of risk reduction and for comparison to benefit). They do not evaluate risk acceptability.

I could fall out fall out an elevated work platform from 8meters ...Risk is very high with a fatality .but very rare,,.my control is now heights training ,certified harness and fall restraint ,full rescue plan ,barriers ,spotter etc,

Gareth. We will probably have to agree to disagree. My view is that if you are working on an elevated platform the possibility of falling remains and if from 8 metres the possibility of fatality remains. The controls you list do not result in you working at a lower height, so the potential energy of your body remains. But they do clearly reduce the likelihood. The key point is that implementing those controls will reduce the likelihood of fatality so far that the risk (consequence x likelihood) is reduced to the point that it is not our main concern if there are other potential outcomes that have a greater risk.
I would point out the following scenarios that apply to the activity with your controls:
* Person at height may not attach their harness correctly;
* The harness may fail in a fall
* The anchoring point that the harness attaches to may fail
* The elevated work platform may fail/topple over.
Hence none of your controls can be considered 100% effective; hence the likelihood cannot be considered as zero and so there has to be some risk.
You are trying fudge in an additional risks that requires a separate control and separate assessment ...The severity of me falling has greatly diminished with the expected outcome a minor to severe injury ..Your scenarios mentioned are for untrained ,non-certified and unreliable staff and equipment.
If i cannot reduce the severity of the outcome by implementing proper controls ,find somebody else to do the job.

This is why regulations and standards for medical devices (at least) recommend / require implementing risk controls in the following order: inherent safety, protective measures, information for safety. Inherent safety, properly understood, is complete avoidance of the risk, but this is not reduction of probability/likelihood or severity. It is avoiding exposure to the hazard altogether. In this specific hazardous situation, inherent safety would be something like allowing the work to be done without being at height at all (no risk because no severity because no exposure). Many times, though, even using inherent safety for one risk ends up transfering the risk to a different hazard or hazardous situation. For instance, using a crane or drone to do work at height instead of a person doing the work at height may introduce a suspended mass hazard in place of a fall hazard.

your videos is very helpful, but my question is that is it not possible to use one matrix accommodating more scenarios?
A matrix is a tool that you can use for certain tasks. If you find it useful to modify the matrix to cover more scenarios there is nothing to stop you doing that.
I believe that Andy's point about different risk matrices being used (instead of only a single one) is that different hazards may have different total probability (P total) or severity thresholds for high, medium, or low risks, especially if the thresholds on your matrices are intended to represent a surrogate for benefit vs risk analysis. This might also be affected by how you stack up risks from different hazardous situations within the same hazard. If the differences are small, maybe there is no value in using different matrices. If they are large, however, there might be significant value in using different matrices.
