It was never the fog! Karma knew it was Death messing with her so she deflected the blame onto the fog to not upset Death further. When Death had her conversation with Karma she pulled her to an empty space(she was in her lab but no one was there even tho karma had just been talking to people) so Death was just repeating that - as for the verryyy end - Karma was just back to the normal Nether nothing special there
