    In this video, we’re going to look at inserting a foley catheter in a male. Of course make sure you’ve verified your order and told the patient what’s happening. You’ll also typically want to perform perineal care before you start.

    If you look at the anatomy of the male urethra, it has an S-shape. The first bend is stretched out by lifting the penis to a horisontal or a bit upwards position. The second bend is streched out by pointing the penis down between the legs and pulling it gently with a firm grip, thereby pulling the prostate forward, stretching out the bend between the penile part of the urethra and the prostatic part, hence turning the urethra from an S-shape or U-shape into an almost straight tube. By doing this one avoids perforating the very thin part of the urethra right before the prostate (membranous part) and avoiding making what is called a "via falsa" = the false way, behind the prostate wich causes bleeding, pain and stricture formation. Also remember to lubricate with local anestetics gel plentiful = rather 2 tubes than one. Make sure to press the tube against the head of the penis (the glans), making sure the gel goes into the urethra and bladder and not out onto your hand where it has no effect. I´m sorry to say that I have seen too many damaged urethras by the above procedure that doesn´t take into account the anatomy of the urethra.

      Is lidocaine jelly ( urojet) appropriate? And do you mix with regular lube?
      I have always had a theory with prostates that if you hold steady pressure once you start to coil when hitting the prostate the tissue will slowly open up and let the tube slip in.

      Had always been successful for me and I have never needed a coude. I do this when installing 3 way 22fr l and it always ends up working. Sometimes it actually can take 3 or 5 minutes of holding steady pressure but I have never missed a catheter once I started doing this.
      What are you thoughts? I am an RN and have never done a cadaver so I really don't know the true internals of the penis anatomy.

      We are never given lidocaine gel, though, just KY jelly. Also we are never supplied with a coude cath for the elderly men, just a cheap rubber “in and out” catheter. I have tried so hard to i+o cath into two older male patients with no luck. (One was supposed to have been sent to hospital for a guided cath insert-the doctor accidentally told me to I + o cath him- she remembered her mistake and apologised to me-and he was sent to hospital and no harm done). I have successfully catheterized women without giving them a UTI. I don’t even agree with doing an I + O cath for PVR on men, but we do not have a bladder scanner. The nursing homes are so filthy. Please consider the reality of the setting and the available supplies when ordering intermittent cath on nursing home patients-especially the men.

      For years when I worked as a nursing assistant in nursing homes I watched nurses catheterize patients and break every single rule of sterile technique; ESPECIALLY on women. Ive seen nurses insert a catheter into the wrong hole-usually the vagina; then take it out and REinsert the same catheter into the urethra. And nowadays the patients in nursing homes are sicker; we have to deal with contracted limbs, 400 lb bariatric patients, etc. Fortunately nursing homes are moving away from catheterizing patients for convenience, and doctors are more willing to allow us to attempt clean catch procedures. This has reduced the overall number of patients being catheterized, which is great.

    Took the ER six tries for me because of my enlarged prostate even with lidocaine and morphine. Hurt like hell, they kept telling me to relax but I couldn't because it hurt. Finally the urologist suggested I bend my toes to tense up but relax my sphincter. That allowed the catheter to get past my prostate. I hope I never have to experience it again.

      Had it done today they are crazy telling you to relax that shit hurts I screamed they kept going deeper telling me I should feel it near my bladder and Relax

      For alert patients I always ask for some Ativan prior to insertion.
      Always insist on Ativan. You won't always get it. But you might and I'm sure it is a much more pleasant experience.

    I thought Catheter was just clipped on my thing but it was inserted. That's why it hurts when it was removed hahaha

    I have been at both ends of a catheter and I couldn't imaging this done to me by a male. Many of the gentlemen patients I have done looked so embarrassed and I felt so sorry for them. If a male is available, I offer to get one of them.
    When I was in school in the last century, my instructor told me to cover the patient's legs as much as possible to help minimize embarrassment. I told her that the text stated to uncover the legs because the sheets aren't sterile, she replied, "The patient's legs aren't sterile either." So I've always done it that way.

      @leslie Beddows, it is nice to hear such a good comment from a nurse. I Had this done for my L4S1 back surgery. There was a male nurse available and he did the procedure. It seems like you care about your privacy. I was lucky that a male nurse was available when he placed the catheter in and when it was time to remove. But I’m sure it not always the case for everybody.

      I'm bawling right now. Must be nice to get the option to consent to it, I didn't get the choice it was forced on me. I feel so invaded, so violated. I'll NEVER get another surgery again, I don't care what the reason for it is. I'm so disgusted I want to cut and burn my skin off my body, I'm so pissed off that I wasn't informed about this part of the procedure that took place during my surgery. I would've NEVER CONSENTED to it if I had known they were going to catheterize me. I would have rather let my appendix rupture. I'm so appalled and disgusted with knowing what they did to me. I might as well have been raped, I can't see the difference. I can't let it go. It's too much for my psyche to bear. I'm so fucking miserable with this weighing on my mind. I can't live with the knowledge that was performed on me, by female nurses, no less. Thanks, New London Hospital! Saved my life just to have me live with the misery and embarrassment of knowing that someone touched me in an inappropriate manner without my consent to do so. ☹️😢😢☹️😢🤬🤬🤬🤬😶 ⚱️🪦☠️🩸🧠🩸🙁💥🔫

    Its Wrong you should use lubricant and lidocaine and insert the folly gently !!!

      Yeah, gently is good. Other than being cruel, it can hurt the delicate tissues inside urethra. Go slow with steady pressure, don't coil, and it will eventually slip in.
      And yeah, lube is your friend.
      I don't think lidocaine jelly is that helpful for relieving discomfort. I prefer IV Ativan.

      Lidocaine gel yes it can help, but also only in the front in the urethra area and exposure time it also needs. This thing is painful and will always be so even with lidocaine because when the catheter is about 10cm in, the catheter is no longer coated with some gel and without gel will just be pushed through the prostate and bladder sphincter where it is needed most. This procedure will always be painful, unfortunately!

    I would never run the risk of placing the STERILE FILED in between the patients legs on their unsterile bed. The risk of it getting contaminated is super high as catherization is painful, patients are bound to flinch. Placing the sterile set on a trolley (that has been washed with soap and water in the past 24hrs then wiped down with alcohol wipes) beside you has NEVER failed nor has it ever been an inconvenience. Also you can't leave the patient exposed, create another sterile field using bigger sterile towels covering their thighs and expose the penis only. I would actually take down this video, it can be confusing to graduate nurses.

      This is done in people's homes you know?. It is hardly a sterile procedure. You do the best you can.
      I am so tired of the sterile field nazis with catheters. Trying doing this by yourself in a home with dog hair floating around, a small folding coffee table that hasn't been cleaned in forever, and a patient who refuses to get out of their wheelchair.
      You do the beat you can. Good for them for taking the time to be educational.

      Not really! I've done many of these. What is true is their first urination is a screamer.
      It's actually worse for females since their urethan is typically smaller.

      There's also the option of surgically inserting a catheter directly into your bladder above your pubic ramus if that's your preference. But urinary stasis and subsequent hydronephrosis (where the pee turns your kidneys into water-balloons) are no joke. With the pain you'd be in, I think the urinary catheter would be much more pleasant.
