It's not as common as you think. Canonically, Embodiment of Scarlet Devil and Subterranean Animism (which doesn't even count because the Extra boss is related to the Stage 4 boss) are the only games to do that.

First games even didn't did that at all. Also, from what I know Am. Akiko isn't much related to Am. Tokuko, they just have the same names.

Biologically, none of them are related. Also, Nue never met Byakuren until after the events of UFO.

you are forgetting the point of an extra stage, of course they should relate in any way with the actual story of the game.
So, Extra's should be random without any relation to the story? So like, Flandre should've been in Mountain of Faith or something?
