  • What do my knees feel for BJJ training nearly 20 years after my 1st meniscus tear surgery in 2004? Well, that's the question I'll answer in today's video.
    I've gotten a lot of questions about this from Brazilian Jiu-jitsu practitioners who are either getting ready to have surgery on their meniscus tear or just recovering. So, in today's video I'll share how things are feeling from both of the surgeries on my knee (1 in 2004, 1 in 2014) and I'll also share what I do that seems to help alleviate pain and keep my joints feeling healthy.
    Just for the record. Both of my tears were treated with removal and not a repair.

    I've torn both my ACL and meniscus doing BJJ. I've been training like Ben Patrick from Knees Over Toes over 1 year now (not all the exercises) and i am doing full time jiujitsu again without surgery. His exercises really helped me; haven't had any issues in over 9 months!

    It’s crazy you just posted this. My right knee got injured in BJJ and I’m getting an MRI in a couple days and I’m already sold on a meniscus tear and possible fold. It’s so devastating and depressing I just want to keep learning and training.

      I'm 26 and have a torn meniscus and acl and I still train 5 days a week. Honestly I don't really notice anything outside of not having the best lateral movement. So don't get too down. You'll be back up and at it again before you know it.

    Just real quick for those that have meniscus issues. I have a meniscus tear about 3 months old now and been doing a ton of research on it. There is some new science that has a control group and a surgery group on meniscus and both groups after 5 years were the same as far as recovery, however the surgery group had an elevated rate of arthritis in the knee that was performed on. Food for thought, but don’t take my word for it do some research and you will see.

      Im in the UK - surgery for meniscus tear is very very rare here. Doctor told me that the pain was due to nerves being exposed by the tear and pressing against the knee cap. He just told me that after the months or year the nerves would get ground away and the pain would go. He said to basically ignore it as the knee was not actually deteriorating (just felt that way :( ). Dunno if surgery is good but after 12-18months without it I think all pain went and knee is fine now - zero loss of function.

    Good info .. I had a meniscus injury as a blue belt when I was 56 .. Surgery went very well and I walked out of the hospital .. As a 2nd degree brown and black in judo now at 67, I never miss a legs day lifting and keep the weight down to where I get a great pump and wear knee supports when rolling .. I also focus a lot on my bottom game to avoid the twisting that occurs when in someone’s guard .. My bottom game has improved dramatically and my knees are fairly healthy .. I find that lifting weights is critical and the knee supports limit the movement plus gives a lot of side support to keep from twisting .. Great video ..

    I ❤ the comment about taking your time. I had my first surgery at 27 and I was REAL BAD about my rehabilitation. I would stress that as well about taking your time. Had my 2nd at I believe 40 and I was WAY BETTER at listening to my body and not rushing things. I have been through the ringer at 51. I’ve torn (all in my right knee) my medial meniscus twice (currently torn a 3rd time) my lateral meniscus once, torn my acl, (currently don’t have an acl) have had scar tissue taken out of my right shoulder and currently have a torn labrum in my left… oddly enough none from jiu-jitsu (which I’ve done just over 10 years now) BUT I’m convinced w/o stretching and just being active that I would be in WAY WORSE shape. I just share my story to try to inspire people that you can be beat up but as long as you train smart you can keep training. That’s my 2 cents. Good health to all.

      @@luissoares946For the most part they do. Usually only hurt when I do something that jars them, like running or jumping on or off something. I started wearing volleyball kneepads early on in bjj. At first it was because I would bang my knees on the mat, years later, I don’t do that, but I still wear the kneepads at this point. It’s just become habit I would say.

    46 year old blue belt with 1 catastrophic knee injury previously.....knee over toes guy has helped me immensely. The atg split squat and nordic curl in particular.

    That's a good point about needing to exercise
    The main thing that keeps me in the gym is it keeps the pain away
    If I go too long without a workout my issues in whatever area I skipped start to flare up

    38 year old black belt here. Had meniscus surgery 4 years ago for a bucket handle tear. I was able to get back on the mats after about 2 months. My game is a little different now but it’s actually better because I’m more careful with the way I move overall. A lot of people tear their meniscus doing little nothing things because they’re overweight or out of shape. So hey, at least I tore mine in jiu jitsu, right? 😉

      I tore mine the same way. Jiujitsu is a knee killer lol why do I love this sport. How are you doing nowadays? Did you have removal surgery or repair?

      @@Refillyowill they just removed the torn part. Doc said he could have attempted a repair, but only gave it about a 75% chance of actually working based on where the tear was. If it didn’t work, I’d need a second surgery. Plus, the recovery from a repair is about the same as an ACL. I just decided to have them take out the torn part and I’ll deal with the earlier arthritis when it happens.

    Just got knee barred this weekend on fight2win, was really sore for the day after but it's feeling much better now. Hopefully there's no significant damage though, gonna take it slow coming back to training.

    I just had a shattered radius that took a steel plate and screws to reassemble, from work. But… BJJ has been been hugely helpful to my mental health, and I already feel myself getting depressed at the thought of losing it even for a couple months or so, how do you deal with that part of the injury/recovery process? I almost emailed your “newsletter” back but didn’t want to bother you…
    Or maybe you can’t really relate, but I’m betting the wise Chewster can. 😁 Anyway, wether you have time to answer this question or not, thanks for the video!

    Yes knee pain doesn't always leave . For the younger 20s year old. Don't Squat heavy all the time. It will mess up your knee cartilage with time. Light weight for high reps. I've had two tears and meniscus surgery.

    Thank you chewy for motivation. I recently underwent ankle surgery. It’s been a bit rough with pain and trying to get some sleep.
    I just hope I can do BJJ again. I’ve convinced myself that I’ll be doing only instructional portion of class and no sparing from now on.

    chewy what you just said then was perfect matey..... solid information muchly appreciated... lifting for purpose over lifting for ego!!!! such good advice man.. love it... i have terrible knees on both sides due to meniscus damage which has ruined my bjj training atm.. im 35 years old and both knees are meniscus screwed atm.. surgery on the left one 10 years ago but now the right ones cooked too, keeps locking up and causing me to have 2 weeks off every other month... i don't want the surgery atm as it locks up but i can get it back into place soo its okay when its back in... soo depressed that it might stop me being able to train jits eventually,,, ive just orderd two of them anaconda knee braces to see if they help at all... if thats a no go then i dont know atm... hopefully it will be okay!

    I've had my ankle broken as a two stripe white belt by a blue belt competitive wrestler. No hard feelings. He was a bit too rough for me being a beginner but I was enjoying the experience shit happens.
    Always take care of your injuries the right way!!!!!!!! It will effect you long term!!!

      Good luck! If it's ACL consider non-operative treatment. A lot of surgeons will push for the knife immediately so get a second opinion.

If I don’t have an ACL doesn’t that put more strain on the remaining ligaments, muscles, meniscus and overall joint? I want to continue to train BJJ and be really active with my little boy.       @@davidneighbors9263 I'm currently going through an experimental bracing protocol. Basically you wear a brace to keep the knee at 90 degrees for two months (you'll be on crutches). I'll find out in another month if it's worked. Needs to be put on within 2 weeks of the injury and only works for some types of tears (if the ligament has fully ripped out of the femur it won't work).
      Option 2 is rehab only but again, this only works for some tears. Nicky Ryan is currently trying this method and this has worked for my last ACL tear (MRI shows reattachment of the ligament).
      You need to be diligent with the rehab either way and surgery may still be needed if it doesn't work. Find a good sports doctor and go from there is my advice. You're looking at 9-12 months before returning to sport any way you slice it.

    Ack! I trained BJJ with a friend last month and was thinking of joining a school, but pre-covid, I took an MMA class and hurt my knee sparring. It's the only reason why I haven't signed up for BJJ: I don't want to roll with just anyone. I'm debating just training for fun with my buddy because we're always safe and being able to walk is important to me.

    Our bodies are amazing machines and can repair themselves most of the time. However, it depends a lot of what genes an individual has also. I have plenty of injuries from playing rugby when i was younger that i still feel today when i am 45.

    Just a nitpick, we have a ton of “out of the box” flaws, for example after bilateral carpel tunnel surgeries, I’m better than “out the box”. But in general, I agree :) with your overall point of assuming that you’ll not be 100%. Caveats do include that sometimes once you’ve had serious work done, then you are more aware and you take better care of the body (joint or whatnot) through knowing physical therapy, exercises, or safer modes of moving. So effectively you may become even better, just not the same way :)

    Great stuff as usual! I would also add another thing to consider - diet! I'm training like crazy at the moment in preparation for my first comp next week but my knee (torn meniscus and broke my foot on the same side a year ago) is better now than it was a few months ago and I think its because I had to cut all the junk out of my diet to lose a bit of puppy fat

    Funny this video popped up today. have been out of BJJ for 7 years, came back 2 weeks ago, and currently laid up with a knee injury. My bad knee is now my good knee, two knee sleeves and a padded volleyball sleeve over one of those didnt keep me safe! :D

    and don't forget that after you tear it once, the chance of retear greatly increases, because of the broken fibers. Also, stretching is esential, especially the stretching exercises they show you in the rehab. Had I known those stretches, I might have gotten away with not having the knee surgery at all.. Stretch, your joints to avoid injuries...

    I had a similar situation when I was young. I had subluxation of the shoulder. The doctors put me in a sling and rest. Over next two years it would constantly slip. Then one day I said fuck this, I'm working out. I diligently worked out my chest, back and shoulders, (went ham on legs). Never had an issue again. Firm believer in keeping muscles strong, joints will last.

    I think this is a disheartening message, and not always the case. I had knee surgery age 26 (Judo injury) and shoulder surgery age 37 (BJJ injury). I'm now 45, and both joints have been at 100% ever since (continuous BJJ training). In both cases, I selected top surgeons who specialised in the joint in question, so I think that helps.

      I've always thought of my injuries as lowering my ceiling of potential. Sure, I'm stronger in some ways at 40 than my 20s, but, had I never injured shoulders, back, knee, neck etc I'd be even stronger in the ways I'm better than my 20s.
      I wish we knew what we know now back in the 90s and 00s in regards to training, recovery and dealing with injury.

    Drinking unflavored gelatin every couple days helps my knee a lot. And running stairs (up AND down) is great for strengthening the knees and supporting glutes.

    Had knee and back surgery.
    The two best recovery workouts for Jiu jitsu is A1 swimming and a close second is hot yoga. Trust me 😊

    I've had three ACL surgeries (once right knee, twice left), and in both cases, I had significant damage to the medial meniscus. I've got roughly 20 years on the right knee since the initial ligament tear (further damage occurred to the meniscus between the time of the tear and the surgery I finally underwent. I'm only just now really noticing more frequent pain in the joint...I assume due to the arthritis I know exists. My left knee is in better shape primarily since those tears came about a decade later. At any rate, I still actively participate in both karate and bjj, but I've learned to work around the injuries.

    How do you get elbows to stop hurting. I’ve been lifting for 20 years and just started BJJ but my elbows are on fire after every class!

      I have the same thing happen every once in a while. I found that it's usually from over worked arms/forearms. Try stretching them both before and after training, use some icy hot, etc. Might relieve some of what you're feeling.

      I also started getting elbow pain after starting BJJ, but it's usually only painful in the morning after I wake up and subsides after that until the next morning

      I use collagen daily, and Ice my elbow after training and use a nice tight sleeve/brace on them. I’ve also found that a decent diet helps my joints recover faster

      I’m late, but the one thing that helped me was doing SUPER high rep, low weight curls. I’m talking 3 sets of 40 reps, with 3 Lb dumbbells.

      @@rafikcheraitia3957hi Rafik. I’m at ~13 months postop now and it feels as if nothing ever happened to be honest. I’m squatting my old weight again.
      I have been back at BJJ for ~4 months now as well. No more knee buckling, no more brace.

    A great resistance training regimen is outlined in the book entitled, "Body by Science" written by Dr. Doug McGuff. It's high intensity with moderate weight. Very low chance of injury. I resistance train once a week. Gives me more time to train BJJ. I'm 57 btw and many at BJJ practice talk about how strong I am (I'm trying not to use so much muscle in training btw! LOL) Enjoy.

    If the Gyms all had nice foam soft matts,,,there wouldnt be half as much stress on your joints,,i dont know why they all have Rock Hard Matts for Bjj,,dosnt make sense...

    Been told that you can get bad knees/hips by not exercising, doctors are starting to say its not old age destroying those things, its not using them anymore. Thought my knee was going at 38 but pretty sure it was not getting NEAR enough exercise on the inside leg muscles/tendons which was making me walk with duck feet. 2 months into a 100 percent diet change (all raw, tons of protein now) started working out and joined Karate/Jiu Jitsu (combined school). Knees are fine now, duck feet are gone (took 2 weeks of really putting in effort to keep them straight and some pain). Feel like im 20 something again.
