How To Safely Come Off TRT?

  • Have you been on testosterone replacement therapy and wondering how to safely discontinue are you worried about high estrogen levels are you worried about whether or not your testosterone levels are going to come back into their normal range. In this article, we will look at how to come off TRT (testosterone replacement therapy) safely. We will address luteinizing hormone, estrogen, do you need to take things like HCG or Clomid, etc. We will also look at some of the questions surrounding testosterone replacement therapy and shutting off the natural production of testosterone when you stop replacement therapy. How To Come Safely Come Off TRT?
    Once you decide to stop TRT you may ask if it's safe to stop it abruptly or if there's something specific you need to do to reset your body. Do you need to do something or to make sure everything is working the way it was prior to starting, for instance. The main reason people ask this question is because of the feedback loops between the testes the pituitary and the hypothalamus. These feedback loops can get interrupted when you are taking testosterone from some external source. This is true whether it is an injection, something you take by mouth, a pellet, or even a topical cream. That outside source of testosterone will actually interrupt the feedback loops between your brain and your testes. Sometimes it will just turn down or dampen that loop or it will shut it off completely and this is a dose dependent phenomenon.
    First we want to look at the feedback loop itself, then we will look at what happens with testosterone replacement therapy. Finally we will look at the things you can do to mitigate the interrupted feedback loop when you stopping TRT.
    Testosterone is produced from specific cells in the testes called the ledig cells. Those cells are stimulated to produce the testosterone by hormone that comes from the pituitary gland in your brain called luteinizing hormone. When your pituitary gland receives high levels or sufficient levels of testosterone (and to a lesser extent estrogen) it will decrease or dampen the amount of luteinizing hormone it produces. So when the Ledig cells produce the testosterone, it feeds back and shuts off or decreases the amount of luteinizing hormone that's produced there. In addition, there is a higher level in the brain called the hypothalamus that produces gonadotropin releasing hormone (GnRH). This hormone stimulates the pituitary to produce luteinizing hormone. The production of GnRH is partially influenced by the luteinizing hormone which feeds back to the hypothalamus to tell it there is enough. This will then cause a decrease in the amount of gonadotropin releasing hormone that it produces. The hypoothalamus also has an internal pulsatile stimulus of GnRH too. So that there is not a constant stream of gonadotropin releasing hormone, it is done in a pulsatile manner. When the the overall feedback loop is interrupted that pulsatile stimulation of gonadotropin releasing hormone is also interrupted.
    Pituitary Feedback Loop While On TRT
    When you are taking an exogenous source of testosterone, the amount of luteinizing hormone goes down. As that luteinizing hormone decreases, you will have less stimulation to the testes to produce that testosterone. In this case it's okay given that you are taking it exogenously. The moment you stop, it will take some time for your pituitary to reset and produce that luteinizing hormone again. Keep in mind this is not a light switch it is more like a dimmer switch. The more testosterone present in your body the less luteinizing hormone you will produce. If you are doing injections, day one after the injection the luteinizing hormone production will be very low and stay low for several days after. It may start to kick five days after or so depending on the dose you are taking. The more saturated your body is with the testosterone, the less LH you will produce. The same principal is true when you stop testosterone completely.
    Many people have commented to me that they don't want to go on testosterone replacement therapy because they don't want their testosterone production to completely shut off. The signaling does not really work that way in my experience. It's more of a dimmer switch and it does come back when you stop taking it. There seems to be some suggestion that once you start testosterone therapy you have to continue taking it because it shuts off the internal production completely. This doesn't mean it won't happen but it seems to be more extreme cases. I wouldn't know because i've never seen it.

    Thank you doc, this was exactly the video I was looking for. I started trt (24 year old using 250mg a week, T levels were on the low end of the range with minor symptoms) about 3 months ago and recently have gotten a lot of anxiety and other psychological side effects. I’ve decided to cease use to improve my mental health and get my lifestyle factors in order to improve my test naturally before trying TRT again. I was scared of stopping without any idea of how to do it but your video helped me so much, subscribed.

      @@swintegrativewhat if you were on an HCG already? I was on 200mg a week of test C. Along with an HCG and an estrogen blocker for 7 weeks. is 200 mg too much to where is shut my LH off completely? And while everything returned back to normal quitting this cold turkey I haven’t taken anything for a week now. My hematocrit was way too high so I had to get off of it

      lol. you didnt start trt, you just started doing roids. replacement is when u dont produce, and 250 is no near trt dose.

      200 a week is too much for most guys, I take 120 to 80 a week microdosing every other day with insulin needles. I'd rather start low than high. My T levels were over 1200 and free T was 330with this low dose. I know a guy that was doing 350 a week for years his was over 5000 lol. Check out more plates more dates micro dosing you learn a lot.

    Taking Clomid for about 3 months can resolve it , it helped me a lot my T levels was 152 and i started taking clomid now my levels are near 500 and now i am again starting clomid i hope i will break the levels of 500 ,wish me good luck

    This is interesting. So many other docs talk of complete shutdown on TRT, they never mention that it’s partial shutdown which is dose dependent. Good to hear a less dramatic view on it all.

      Great info, I see a lot of people on TRT but do not make any videos of how to come off if one decided too! Even andrew huberman claims to be on for a year, but not one video of how he came off, which in his line of work i guess would be beneficial( if he did come off)😅😅😅

    Thank you for providing these answers, I rushed into TRT without giving the lifestyle changes a real shot and was scared I had screwed myself over, this info has made me feel more confident about coming off of it to give those lifestyle changes a chance before committing myself to a lifetime of injections.

      i think i am in the same boat as you. i dont think it's helping me at all. i just need to workout daily. the motivation isn't really there so i will have to discipline myself. it seems like biologically, we have the tools to benefit from the gains of testosterone naturally, not injecting it. are we thinking the same thing?

      I got on trt after 2 years of lifting and getting lifestyle in check. TRT does wonders but the lifestyle must be in check. Little to no drinking and whole foods that aren't processed. ​@@bradfowler1759

    Wow best advice I've gotten, my doctor just put me on TRT and I got almost nothing out of it, now I'm feel bad, it seems to be doing the opposite of what TRT is supposed to be doing and my doctor is clueless! I need a specialist to help me with this issue.

      @@Rog-Nature-vs-Nurture I gave up on it, but I was on it for about 4 months. It seemed to help a bit at first but as time went on I felt no benefit so I stopped. Please don't let my experience stop you from trying it, many men get a huge benefit from it, we are all different. Make sure you get your estrogen levels check regularly because if it gets to high, you could develop a gynecomastia. If this happens, your doctor will need to prescribe an estrogen blockers. Let me know if you have any other questions. I really don't have that much experience but I'll help you all that I can. Take care.

    Just what I needed.. thanks Doc. I went carnivore (and some fruit) after starting TRT 3 years ago. Now Im seeing a bunch of older guys like me going carnivore and cleaning up their diets/lives and their testosterone is coming back.. Im assuming because of the high LDL. Im going to come off TRT and see what I can make on my own now at 54. My nads also shrank by 50% so itd be nice if they returned to my reg size as well.

    Thank you for the video. I’m 34 and have considered getting off TRT mainly because it’s been a struggle to keep the estrogen in check and also because I figured I had hypogonadism(total T was 200 when I started) but it could’ve been stress, outside environmental things with where I was at, at that time in my life( I’m type1 diabetic). I wish I had looked into why my levels were so low instead of jumping right onto TRT. I have been on it for 3 years but really I just want to see if I could get back to where I was before I needed TRT. I’d definitely want to get back on it later on in life like after my 40’s, I think it’s great for those cases, but for now, I’d like to see if I could get back to decent levels being natty while having a solid diet and workout plan.

      Bro im 34 as well and I'm wondering if I should do the same. Don't get me wrong I don't regret starting trt, but I started to notice too much acne on my chest and on top of law's currently changing, plus the fucking cost to do this shit legally... i figure i need to fix all these other things around my life and be more strict with my dieting and health and stop smoking weed and jump back on when I'm older. My question is what is the best way to get off Testosterone replacement therapy because my estrogen level has gone up. Before when I started Testosterone my estrogen was at 18 and now my estrogen level is at 43. How can I bring my estrogen level down to 20 to 30 while being on Testosterone replacement therapy?

    Please make more videos like this. A lot of guys I’m the same Situation like me with no answers. I’m 26 and been on trt for a year. I never even ended trt in the first place but tricked myself into thinking I needed it. Test levels before I got on before was at high 500’s. Started at 200mg a week and now dropping to 100 a week. Took HCG on and off and now I’m taking it constantly with trt at 500 iu HCG a week with 100 trt. Should I continue my weed off cycle? It’s only been 1 week at 100 a week. I have a new job and am unsure how bad the crash is going to be.

      Due to that crash I came off with HCG and Enclomaphene which was completely painless.

    I am on TRT one year (age 29). I have been on keto all year and lost 27kg.. soon to be 30kg of fat. (120kg to 94kg as of now 5’10)
    I have very low Testosterone… should I stop TRT and see if my levels return to a normal level? Above what I was when I was 30kg heavier?

    I did hcg for two weeks after getting off at 500 IU per day and 25 mg enclomid it helped with symptoms but my blood work is still absolute shit . I started 2 weeks after my last test prop shot and felt good for 2 weeks and now it’s weird . Hopping I start feeling better in a few weeks . I kept all my gains tho like still looking like a savage beasts

    What about the use of an AI to come off trt? I remembering hearing something about using an AI (no hcg or Clomid) to come off trt. Is that bogus? I think some RUclipsr suggested it.
    I’ve been on trt for about 4 years. It’s helped in different ways, but, I’ve never been able to get estrogen under control. Ever. And I don’t tolerate the addition of an AI to trt,so it’s a lose lose. I want to come off. I’d be willing to use the AI if it helped stimulate production of testosterone. (For a small period) Thanks!     24 years old with total test at 82 and Free test at 4. I’m thinking of going on trt for the overall benefits, but I’m concerned about coming off. I want to roll with it, but I just don’t want to screw my self up naturally or if I make some “gains” I don’t want to lose that either.

    43 years old. Was only on trt for 3-4 months. Caused cystic acne and I made the decision to jump off and used a pct protocol of weaning shots to nothing 3 1/2 weeks ago , then took clomid and tamoxifen. Kinda feel like shit. When will I get to baseline feeling of normal??? Thanks

      Probably yes. I would expect the psa to drop which is a proxy for prostate size.

    I came off trt 4 months ago cold turkey, my hormones are all Back to good levels however my LH is still elevated , I’m assuming this will start coming down into normal range soon as my testicles are clearly working

    Currently, my doctor has me on 2500. I use three times a week for the next five months. Is that normal? What will that do? How will that help? I should say thanks I forgot to mention that’s HCG

      That is a higher dose but it will stimulate testes to make spermatozoa and testosterone.

    i have been on it over 4 years now. i do not think it does anything for me. i think it's caused me to gain weight, some muscle, but it's also made my low back issues worse, exposing my weak core. diminishing returns... i don't see a point in continuing it. i just need to consistently work out. not worried about my libido, just being able to look down and see my penis again would be great. thoughts?

      Sounds like u know what to do. Hard to say though without knowing all the health details. Testosterone can be very helpful for motivation and muscle growth when in range and balanced with other hormones but it’s not magic. You have to put in the work ( not to say u are not). The effort u needed at 25 will not work the same at 45 though. Good luck

    Thanks for the video doc. I’m scared of coming off of testosterone as being a body builder I was taking 2cc a week for almost a year. I’ve been having heart palpitations and chest pains that are very new. I came off of it cold turkey a few months ago and crashed super hard. I thought I was dying. My body was freaking out. And not I’m pinning .5 cc twice a week. Trying to stop taking it and I’m just scared of the crash again and that it might hurt my heart.

      When I’ve tried stopping testosterone i get those same symptoms, i would believe you should know this information Doc. Chest pain and heart palpitations with high blood pressure are symptoms of low T. Does it cause heart problems??? Who knows, but those of us who have experienced those symptoms sure do feel like it could cause heart problems. Heavy breathing, fatigue and palpitations make us feel that way.

    Some people in forums say that if you stop you should expect to feel like hell for up to 6 months, why that long ? (Assuming the person had normal levels before)

      that is the not how it works for everyone. Yes some people do feel worse for a few weeks or a few months. 6 mo is longer than i would expect and likley these people felt this way proir to starting so they just feel bad again. Everyone is different and there are so many different situations that can occur.

    What are the long term effects of trt? I've been taking for over a year, 180 cc a week due to levels being 275 at the age of 34. I have no plans on wanting to stop any time soon as i feel great. However, I need to donate blood to reduce red blood cells every 6-8 weeks. Is this normal? Your thoughts are greatly appreciated.

    Also, is it normal for your blood pressure to be high and also be hot quite a bit when normally you’re cold a lot can it also cause breathing issues lately my breathing has been a little bit harder when I’m taking the hCG

      Estrogen can go up with HCG use and has been known to cause hot flashes. Sounds like these r things to discuss w your doctor

    How safe is HCG? I have some in my fridge but I have been hesitant to inject it. I have been on TRT for a year I’m thinking about getting off and started the HCG cycle to get everything firing on all cylinders again.

    Thanks for the info. I started TRT 4 weeks ago. I'm 46 and i feel amazing but i am terrified about the idea of coming off it. I don't believe i want to live on it for the rest of my life. I am going in for my first blood work (next week) since starting April 14 and will talk to him about HCG and how to restart my system. My testicles have already shrunk. I got on TRT because i was dealing with exhaustion and my weight has crept up to 270. I am 6 foot 5 but I was a marathon runner and did triathlons in my 20's and 30's and i haven't been able to figure out how to get back into shape. In 1 month i've already lost 20 lbs and i'm training like i did in my 20's . Im still afraid of long term side effects.

      Same age here. Been on trt for about a year on a low dose (1ml) every two weeks. For the past month, I've been on .7ml every week. I feel a lot better. No fatigue, depression, etc. I don't like the idea of being on something for the rest of my life, but I'm not going back to my old life before taking try. I was in a quagmire mentally and physically.

    Im off cold turkey after 6 months of 80 a week, its too expensive w blood tests and cyp at a clinic,not much gain in my experience, gained 10 unwanted lbs of water wt even w working out at times twice a day. My opinion. Peace

    Coming off TRT doctor is saying to take HCG and clomid every other day for 8 weeks? Isn't that too much ? should I take HCG and Clomid both ? or one or the other ? Please somebody advice.

    Hey doc, I tapered my trt dosage down to my last pin which was 7 days ago today. I have clomid and hcg ready to go. Just wondering what the recommended dosage and frequency is? I was on low dose trt for 2 years got myself out of hell and my lifestyle is good now I’m healthy. 29yo male

      I can't give you specific treatment plans like that but I'm glad you are feeling better. You shoudl be able to get that from the prescriber. Some people take clomid once weekly others need to take is 3 or even 5 times per week. Good luck

    Man, I'm gonna attempt getting off. Been on for about a decade and my sperm count is 0 and LH and FSH also very low. Wanna try and have kids again. Didn't think I would want more until I met my lady. Got all my meds lined up to help. I'm getting back on later, for sure. But I really hope I can start producing sperm again. Heard some get pregnant after 2 and 3 months off. Heard some take a year. Some never. Scary stuff. Didn't know this would shut me down like this when put on. Gonna do a huge blast for the next 3 months, and then start to come off. Outta be fun 😭

    Iv been in trt (100 weekly )for 1 month, first 2 weeks were weekly, than after everything 2 weeks. Reason by blood pressure is high. I feel like crap. I want to come off. My org trt was 346.
    My estrogen was a bit high. And im taking an estrogen blocker every time intake the trt shot. I just cant handle this high pressures. It reads from 134/70 to 146/80
    My norm pressures was 112 to 120ish, should i come off or lower it.

    I was on 160 mg of test cyp. Wanna try for a kid.. sperm count was low but not gone dropped to 100 mg test.. while taking clomid 50 mg and 1500 units hcg 3x a week.. for about a month.. decided to stop test all together a week ago.. you think soon i will be more fertility or am i still looking at that 3 month mark?       More yes hard to say how much more. Just had someone have normal sperm count despite being on TRT (and no hcg) last week so it is possible.

      Look up Dr Scally Power PCT.
      Day 1-20 : 2000iu HCG every other day.
Day 1-30 : Nolva 40mg/day (20mg was taken twice per day) ; Clomid 100mg/day (50mg was taken twice per day)
Day 31-45 : Nolva 40mg/day (20mg was taken twice per day)
      Some do lower amounts of hcg. A few Doctors state you don’t need more than 500iu per day but I believe this all comes down to many factors.
      Please consult a dr for their recommendation on this protocol. This is just info I know from research, but a DR should be the one telling you what you should do.

    So your saying you really don't need PCT coming off Test if your a healthy individual? And HCG is a good boost to get things up and running quickly? Is Tapering a good way for your natural production to come back? When tapering down, does your natural production begin to make up the difference in lack of injected Test? Or will your natural production just completely stop when your body knows its getting it from TRT or will there still be natural production if your taking a very low dose? I guess I am trying to understand if taking test will change your level of natural production rather than just completely shutting down. Im 51 and beginning feeling the symptoms of what I think is just getting older. I want to bring some of that energy, muscle back and thinking of TRT, but want to know if I can come off easily after trying it for a year or two. Thank you!

      If you are on a small enough dose, you will still produce LH. If you have LH, you still have internal production. How much idk.

    Question. I was on 200mg a week of test C. Along with an HCG and an estrogen blocker for 7 weeks. is 200 mg too much to where is shut my LH off completely? And while everything returned back to normal quitting this cold turkey I haven’t taken anything for a week now. My hematocrit was way too high so I had to get off of it

      200mg per week would likely reduce lh and fish to zero.

    Out of curiosity, would a cold reboot of TRT be a good idea? My hematocrit, hemoglobin and RBC went high after about 7 months on 200mg/week then backed down to 140 for a few weeks.
    I'm thinking of giving my body a few months off then starting again around 120-140 mg/week in hopes it doesn't raise my blood again.

      @@swintegrative thanks for the input! I was experiencing light headedness, tingling in arms and hands, and my blood pressure was a bit higher for me. Not out of control but 125/65 - 130/70 when I was normally 115/70 or better. Interestingly my fasting glucose was always about 103-106 which I attributed to the ratio of blood being off from the thickness of it. Was always 90 or below.
      Been off for 7 days now feel ok waiting for the inevitable symptoms.

    I'd rather not quit TRT because I haven't felt this great in memory. I have to come off of it though because high PSA (14) and will be undergoing biopsy(s?)/ treatment in the future. I'm 71 and tried HCG previously and it made me feel like crap. Would Clomid be a reasonable substitute or other??

      It’s not the same for everyone. My experience short term use of TRT you will return to baseline after stopping. If it was short term more likely.

      Hard to say for sure as it is not “needed” anyway. It just helps ease the landing. Given the short duration it probably would be overkill.

    Get onto HCG and / or HMG while on the TRT. Then, lower the TRT and eventually come off as your FSH, LH, and testes grow back.. Do not just come off it!! Why this doctor is recommending to just come off it is quite bizarre. That is not an option unless you were blasting(steroid cycle) and are in your 20s... Most people on TRT are 30+. Have high SHBG and/or some sort of problem along the HPT Axis, which means they needed it in the first place anyway

      Thanks for your feedback and input. The content of this video is based on direct experience working with patients coming off testosterone. There are many reasons people benefit from TRT and equally many reasons for it being low to begin with. This kind of nuance is not always captured when doing a video like this. That’s why it is best to discuss with your prescriber. I do find that many do not need hcg but some do. I have another video coming out soon that tries to capture a little more nuance.

      That's a longer ester, but you're looking at a minimum of 6 weeks before the drug is out of your system.
      Get blood levels checked after 3 months

    Hello! One question: if you are already on trt and you have to do the next blood test to test your testosterone levels, is it better to stop taking supplements like multivitamins and stuff a few days before or it is not necessary and will be totally not affect your testosterone levels at all? because I hear some say it will have an effect on it! What is your opinion on this? Thanks Doc

      Most vitamins are not going to affect it especially when u are taking exogenous sources

    Iv been on a pretty strong dosage of test c for about 20 months now, starting at the age of 19. Iam now 21 and looking to come off, is it possible for my natural testosterone to come back as it was before with the right pct such as HCG and clomid. Also how long can you estimate it to come back ? It was a big mistake coming on and I regret it.

    I finally tapered i waited two days after last prop shot started on 400-500 IU hcg everyday until my vial runs out so that’s about 12 days . I am also using 25mg enclomid with this. Then for 6-7?more days I’ll run hcg at 800iu a day with 25mg clomid. That will be a 3 week SUPER charged pct. I think I’ll be fine was only on 87.5mg test prop a week. I actually feel so much better just on the HCG honestly

      Possibly to much estrogen. Id try skipping a dose and then coming back on at a lower dose. I feel best way down at 80mg / week, but I build muscle better at higher doses. Anything over the 80 mg and I need some aromasin to knock my estrogen down. Generally speaking the older you are the more you aromatize into estrogen.

    Thank you for your video. Prior to trt I had 259 ng per dl and 59 free. I don't know what I am now but I've been on it for maybe 2 months taking it daily at 70 mg per week taking small daily doses with insulin needle. I'm starting to feel tired again and I've been losing erections and feeling weak. I think maybe I should get off because I don't like how I feel and I think there's probably a better way to raise my levels to acceptable like long jack or onions. I don't like how I feel at all anymore.

    I don’t say this to be rude whatsoever or condescending, but it’s funny how much people will believe what is said in this video and jump on TRT, I know TRT has a ton of benefits, but the cons are significant as well. I’m no doctor but I think it’s very safe to say that testosterone injections stop your natural production of TRT which will not start again after injections. Very hard to find information with people admitting or talking about this. if you are on the fence about this I would wait until your older and would consider staying on injections 4 Eva.

      Hi Ernest, thanks for sharing. You mentioned "testosterone injections stop your natural production of TRT which will not start again after injections." I do not find this to be the case. On numerous patients testosterone levels do seem to come back to their pre-injection levels after stopping TRT. This seems to be the rule and not the exception in my practice. I know it is possible for production to permanently be suppressed (but not completely shut down) with long term use. After 14 yrs of practice, I have yet to see this though. I am curious if you have different information/experience.

      Good job you’re not a dr as what you advised is completely false. Jumped on and off trt for years (8 months longest time on) and regular blood work. My test always recovers to pre levels within 4 months. Rough time for around 6 weeks after it’s out of my system, but always a gradual return.

    I decided to come off due to fertility problems even after taking hcg.
    My doctor said just go cold turkey and you will be fine.HOW WRONG WAS HE!
    4 weeks later I'm so emotional like I could cry at any minute
    I get anxious quite frequently.
    I'm fatigued more
    Everything is a challenge, even getting to work which always puts up my fight or flight response.
    Horrible experience

      Most people do bounce back but it can take 5-8 weeks… depends on your dose when you stopped.

    Need your advice brother. I have all the symptoms of low T. I'm 35 years old lifelong Natty. I normally transform my body way faster than the average person with proper food and decent sleep training hard of course. However when I stop training and eat like trash I also get out of shape just as fast. So i can work out for 12-20 weeks 5 days minimum a week and literally look like I been training for year or years. I been working overnights for the last year and not sleeping well I been training again from taking 5 months off got out of shape and now am some months into training now but this year has been weird I'm not transforming as fast.
