    Hi Kendra! Thank you for providing excellent content. I am not a nurse and have never been in the medical field. I watch because I can relate to you as a professional woman who strives for excellence. I want to say thank you for that video where you spoke about people being sicker than 10-15 years ago and how some of it is totally avoidable if you take care of yourself and eat right. Well it stuck with me. I was diagnosed with diabetes, morbidly obese, and depressed. Your message stuck in my mind. It’s helped me to develop discipline to get my health together. In the past few months I’ve lost almost 25 pounds and reduced my A1c to a 6.1. I’m now drinking a minimum of 64 oz of water daily. I feel great! I’m just getting started on my lifelong wellness journey. I said all this to say, keep speaking truth. You are helping more people than you know. Be blessed!

    I worked 31 years on a cardiac/Neuro PCU it was rare when we didn't have at least 2 or 3 vent patients on the unit. If vent/trach patients were stable they were routinely transferred out to med Surg as long as they did not have cardiac or Neuro issues. Those that scream the loudest tend to know the least!!

      Correct, stable vents in med surg, if they need telemetry monitoring, they’re not that stable.

    They always tell people if u don't like ur job leave. Guess what? People r leaving & they can't replace all of them. It sounds good until it becomes a crisis, which n some facilities it is. That person is all bark with no teeth 2 bite.
    LTACs can b brutal. I did 1 n Cali last yr. 99% of those pts were vented fresh out of ICU. Many were unstable & overstayed their ICU stay due 2 insurance. LTAC gives them the extended care they need.
    Levindale has a HICU (high intensity care unit) where we would get ICU pts who were still n comas. We had a coma recovery program & would try 2 wean them off the vent, if possible. If not, they would go 2 a long-term vent facility once stabilized.
    I just worked @ a Trauma III hospital on LI where we got vents on the M/S tele unit. They were mostly chronic vents & acute vents placed n ED 4 whatever reason. Some hospitals don't get vents on M/S units, but many do, esp if they don't have a lot of PCU or Stepdown units. It's impossible 2 keep every vent n ICU.
    Trauma I, II, or III, the workload is insane. I'm currently working Tele Stepdown float & it's savage. 4 patients is too much. The comorbidities r insane. 20 yrs ago, it was HTN, GERD, DM. Now it's HTN, CVA, Parkinsons, DM, Lung CA, Multiple wounds, Sepsis, BPH, Lupus, A-fib, Hypothyroidism, CKD 4, CAD. I'm like sir, mam, what is not wrong with u?

      "What is not wrong with you" I'm laughing but I'm crying too. Seriously, when you look at the patient's summary on EPIC and you keep scrolling...down...the PMHx & comorbidities. I never realized how sick our society was until I became a nurse. Even patients from other cultures that are usually associated with good health, they immigrate here and get all our modern diseases. It's not right.

      @@ada5851 Some of them have poor health @ home as well. They get access 2 healthcare when they come 2 the states. Fast food has also made its way across the globe. A lot of people r not active & drive everywhere, even if it's up the street. Walking is frowned upon by too many. The government allows toxins n our food that r not 4 human consumption. Even Whole Foods isn't organics.

    Whattttt that person needs to SIT DOWN. The state of nursing and health care is in SHAMBLES. She must work at some imaginary facility where you get breaks and a fair wage. The rest of us don't. Tell me why I took a desk job at my hospital but picked up an extra shift in the ICU today and I was assigned 3 patients. THREE. Its the regional medical center for a tri-state area, so very busy. Oh and this hospital doesn't staff CNAs in the ICU. Because, money. Fuck all the people who decide what is standard of care.

    i agree-i work on a neuro epilepsy floor and we may not see vented patients but it’s entirely plausible that some institutions get them because we get patients wildly inappropriate for our nursing ratio and our expectations of our patient population just because what else is the hospital gonna do? turn them away? the icu is full! I’d say that 1/3 of my workdays im really in a stepdown floor and not a medsurg level floor. Like i have a patient who is suddenly coughing up blood and who is also a stroke care patient and another patient in acute respiratory distress at the same time who is swollen up and obtunded, then two more patients.

    I just quit nursing job ! I was an LPN and. Currently in school to get ky ADN RN.... But lately i just have not been wanting to be a nurse at all . I started as a CNA my whole family pushed me onto the nursing industry! I love caring for people..... But I do not have a desire to do it anymore its very taxing on my mental and physical . People are so unappreciative sometimes and I am ready to try something else.

    I currently work on a med-surg unit in one of bigger hospital in NYC. We take ventilated patients (chronic vents, new vents that take a long time to decannulated, trach collar). Ratio on good nights 1:6 but it can go up to 1:7 sometimes. So the nurse who said there are no vented patient on med-surg don’t know what she talking about. Lot of total care patients too.

    Hi Kendra. About the ventilator issue. I see both your points. I can see that lack of resources (nursing staff) can sometimes cause patients on vents to be on the telemetry floor but ideally for patient safety I can see why we would not want to put a vent no matter the acuity on a telemetry floor where nurses have a 1:6 ratio. It is not about the fact that the RN is not capable but it is about the time that the RN can dedicate to that vent patient. Where I work (level 2 trauma hospital and community hospital) all vents are discharged from step down. They never go to the floor. And I still work at the bedside in leadership.Nurse for 29years. Please come back to ICU we need you ❤

      Vents that need telemetry are not stable patients. They won’t be on a PCU, they would be on an IMC or step down ICU. Stable, chronic vents are appropriate for med surg and usually the ratio is lowered when we are assigned vents. 1 vent patient counts as 2 in the unit I work. My ICU days are over, I miss it but I don’t think I can do that kind of nursing anymore.

    That ring light is everything!!! You look like a bag of golden brown sugar 😁😁 I thought about taking a weekend only (Sat/Sun) RN position on a MS unit just so I could have M-F off! This weekend position is considered FT because of the weekend commitment! But I’ve been away from bedside since 2016 and ppl are sicker, the ratios are higher and I think my anxiety would be on 10000…. So for now I will just keep working my M-F patient as a RN Care Coordinator. **sigh** I pray to be financially stable to work very very part time in the next two years with benefits cause sis is tired!!

    Hi! The lady in the comments is sooooo wrong! I've been a nurse for 20 years. 11 of those years I've been working at NIH (NATIONAL Institute of Health) on a medical surgical research floor, and we got ventilator patients all the time. My first 2 nursing jobs in 2004 were in a Long Island hospital and in New York Presbyterian in NY, and we took care of ventilator patients. So I wonder if she ever worked as a floor R.N.

      Yes because patients are staying longer in the hospital and vented patients who aren’t critically ill cannot take up ICU beds.

    Kendra, don't second guess yourself on how you responded to that comment. If you were face to face with her, you would have and should say it the way it is. Trauma 1, that's like Magnet Status. It's a load of bull! At the end of the day, every nurse on any unit has to clean up poop. You can't tell me that in level 1 facilities patients poop smells like roses! Nice to see you back.     3 levels in Texas of EMS caregivers. EMT Basic-very basic skills. EMT Intermediate- some higher skills. EMT Paramedic. Highest skill level. Can give all meds in ambulance, starts IVs, uses all types of airways including ET tubes. Code leader

    Hey Kendra, I work in med surg, we have 2 chronically ventilated patients in our units now. She is wrong for saying that, maybe back in her Mezozoic Era days, they don't admit vent patients on med surg but now they do. That's why I tell nurses to stop "super nurse BS " cos Nobody knows it all & times have changed

    You are 100% right about the ventilator. I’ve worked in the NICU and a LTAC for peds and it’s exactly how you described. That other nurse is crazy. She may have years of experience but it shows that her experience is very limited when it comes to variations of care. Keep standing your ground!

    Speak truth! The name and reputation of the facility does not make you a great nurse. She probably does not know patients can be managed at home on a vent!! Why is she trying so hard?!     chronic vent makes sense but can you imagine 4 acutely ill vented pts and they all start bucking the vent at the same time, low pressures from sedation, copious secretions, paralyzed, proned, desating, etc. i wouldnt be surprised if nursing comes to this lol

    Hi Kendra,
    She has no idea what's she's talking about my hospital has a unit call med-Surg/chronic vent
    We get pts from step down ICU, who are not get ready to be fully weaned off the vent or who will live on the vent and awaiting placement in an LTAC. Sometimes they are there longer than expected due to insurance issues, HD setup, wound care etc
    Sometimes these pts have no insurance and no documentation so they literally stay on the floor until they pass away unfortunately
    We are ventilator train medsurg nurses

    I had a vented patient for years in the home setting. He would at times have to go to the hospital. They put him on a med surg floor generally not icu because he was chronic ill and stable. Not the idea setting for him because he was used to 24hr one to one at home. He feared hospital in general because of not getting the attention and care he wants. He to this day rarely agrees to go for anything. But you find a lot of those patients in acute rehab when first injured but they do Like Kendra said in up on the medsurg floor if they are care chronic and stable.

    I know you said you are tired, I think you should take advantage of your expertise and social media presence. You can be an acute care nurse consultant for new nurses, nurses returning back to the floor due to hcol, nurses struggling to manage high acuity patients. You can consult nurses with resumes, interviews, finding agency and hospital assignments. Lastly, write a book about your nursing work history, education, pros and cons, would love to read your journey. I am an RN and feel nurses need to shift into using knowledge they have as a source of income to move into other means to gain financial freedom. We Are the Business! I think utilizing this new digital world we live in will get us there faster. I would never imagine working remotely as a nurse but here I am. Good luck on the boards!

    Hello Kendra! First I want to say that I absolutely love your videos and your personality. Congratulations on completing your NP program and you will pass your boards😃.
    I have been a medsurge nurse for the past 6 years. I have been doing travel for the last 3 years and just accepted a position in the MICU at the hospital I am currently doing a local contract with. I am so excited but do have some fear about the transition. Do you have any advice for transitioning from medsurg to critical care? Thank you so much ❤❤❤

      ICU ration varies, standard is 2:1. If the patient is on CRRT or Balloon Pump, should be 1:1. If there are patients that are downgraded you sometimes can have 3:1.
