If I recall correctly, he doesn't end up dying until they rip his face off and start eating his brain. When he does eventually rewind to his save point, he has a seizure because he thinks he's still being eaten by the bunnies. I believe they also mention that some of the rabbits went inside his body and started eating him from the inside out.
This episode became the real reason why everyone who has watched the show up to that point have been traumatized by Subaru getting devoured by rabbits from outside and inside and showing it on screen. That death to this day still remains as the most brutal, violent, horrifyingly, disgusting one of all times he has died and this one I think will scar renegade for a very good while but the rest of us viewers will end up saying "another has joined our nightmare that will remain sickening each time we watch new people react to that scene" in which we all still do exactly that and go "yep that was us back then when we first reacted to that scene but remain scared for the rest of time".
Subaru's screams were bone-chilling. Major props to Yūsuke Kobayashi. Also you are correct in that Garfiel's voice actor is the same as Bakugo's. Nobuhiko Okamoto to be specific. As @ridley7498 mentioned, this was not only Subaru's worst death so far, but it's even more brutal in the novels. Not only for the fact that he was still alive until they cracked his skull open, but some entered into his body _through his anus!_ Subaru not only was being eaten alive both in and out, dude got violated in the worst possible way. I could be wrong, but if I heard right, if it wasn't for Echidna, her tea, and Subaru's sloth witch factor, he would've mentally shut down while having a seizure. Even Return by Death would not save him.
The crystal teleported him to a particular location in the sanctuary, just like it did the first time he went through the barrier. He probably forgot he had it on. I honestly forget what the crystal does, but I'm pretty sure it isn't required to enter the sanctuary (since all the villagers entered the sanctuary already), but rather it's proof to Garfiel that his sister sent them there since it belongs to her. Also you'll learn more about Garfiel as well, as to why he's like this.
The Crystal is just a type lagmite crystal, a magical stone that stores mana. It's very small and rare, compared to other magic stones. For Garfiel and Frederica it helps them transform without using their own mana, but there are numerous ways one can use them, Frederica's just had a teleportation spell imprinted on it, which activates when it comes in contact with the barrier (that much is known from episode 6), but who did it and why was not yet revealed
