    Dr. Ghada i have a question please. Question number 40 @ 1:46:36 the question states that post CS patient is suspected of intra abdominal hemorrhage. But want to start thromboprophylaxis.
    My question is that if we are suspecting a case of hematoma ... which means that the patient is already bleeding why are we giving the patient thromboprophylaxis with Heparin ? Wont the patient bleed even more if we do that ?

      If you suspect the key ward in this question suspect so you can used hydration, AES and unfractionated heparin as its short half life mostly will not afftect bleeding remember also that this patient at risk of thrombosis if you leave her without thromboprophylaxis but if he give you another question patient has severe bleeding or you confirm that there harmatoma you want and you want to evacuate at this point you can stop heparin and restart again after operation.......

      i mean that patient need heparin according to her risk assessment score postdelivary in general.

    Dr. Ghada one more thing i didnt understand about the questions were that while at start we were solving questions relating to the scoring for VTE later we started solving questions concerning with Very high risk, high risk, intermediate risk and low risk chart. Now how do we know that the questions asked are based on either scoring system or the later system ?

      According to data in the question...if very high risk you will give high dose LMWH and involve haematology regardless other risk factors...but if questiom depend maily to calculate the risk in total you need to check how much score first..
