Kittyplier i dont think thats the reason he has a mental illness but it is true that if mental illness or any other kind of illness runs in the family chances of getting the illness/disorder increase tremendously. which makes sense because both parents have that gene because they are in the same family. so the child has no other "backup" genes. this is why incest causes a lot of medical issues and why its discouraged and even biologically unpleasant to humans

Like the comment above said, there’s probably a higher risk of them inheriting a certain type of “rare” mental illness because both participants carry a gene. However, problems health wise don’t usually start to happen until the incest is 4th generation. 1st generation, and with cousins not too closely related like in Dandy’s case, wouldn’t cause immediate medical issues that incest usually carries. However, obviously there are some risks.
