  • Cindy Bauer, BSN, RN is at the forefront of transforming care delivery through leveraging her advanced knowledge of high-risk populations, healthcare systems, models of care, and decentralized deployment of teams in the field.
    Bauer has pioneered innovative technology to optimize clinician resources and drive better outcomes for both payers and providers across multiple healthcare markets including health systems, home health, hospice, and primary care. As a Founder/CEO, working closely with her tech team and conducting market analysis, she has gained powerful insight into bringing healthcare products to market that provide value to multiple stakeholders including clinicians, administrators, business owners, and patients.
    Bauer also serves in Senior Advisor roles, providing guidance and professional advisory services for healthcare organizations seeking to improve performance, adopt innovative solutions, or optimize resources. Current clients include a firm providing business management services for Medicare and Medicaid organizations, and a Managed Services Organization innovating on full scope of nursing services in primary care. Past clients include a digital health caregiver platform and as a hospital liaison for a prior employer.
    Bauer has also led a multi-state Integrated Care Organization in performance in their Transitions of Care (TOC) pilot program for a dual-enrolled population, increasing member engagement in TOC and CCM and reducing readmissions by 30%.
    Bauer has had the privilege to serve high-risk and high needs populations from the ICU to the home for many years in multiple organizations, locations. She's also thankful for the unique opportunities along the way to homeschool her three children, serve in the state legislature, and capture a Junior Olympic National Championship in speed skating.
    Her mission is to work with others in driving positive change in care delivery models focusing on the highest risk, hardest to reach, and most vulnerable populations.
    In her spare time she enjoys running, cycling, kayaking, reading, and spending time with her children.

    It all began with one nurse’s idea after seeing patient after patient fall through the cracks ending up in the emergency room or hospital from situations that could have been prevented, while at the same time, seeing frustrated nurses and social workers all around her, who wanted to make a difference.
