It's sad how he really lived in that world where he murdered people for his own enjoyment, and he really felt like he was going insane and he really was mentally shattered and in constant pain, but it all turned out to be a delusion. And no one will help him get better. Even if he had actually killed all those people, his father could hide the evidence and life would go on as if nothing happened. No matter what Patrick Bateman did, no one would ever help him, and no one could ever save him from that life. He's trapped.
Well, in the movie it’s slightly different than the book in portrayal. I also got the impression of “oh it was all in his head” first time I watched it, but the director herself said in an interview she regretted how she did it, as this interpretation of him was not intentional. A number of the murders were real, we just don’t know exactly which ones, as is more clear in the book
