Yes, DOT is a requirement for Curri. It'd free to get DOT from FMCSA website. Just submit an application. There are plenty of RUclips videos that show you step by step hiw to apply for your DOT. You will receive it same day. Insurance is not a factor, only if you apply for an MC.

No, you don't have to have commercial. I have it because of state law, but the gig apps and some courier companies do not require it.
