Every time a list like this comes up, the first Surprise Box in Mario RPG should always be mentioned. You get one shot to make a jump to grab a hidden chest that you didn’t know was a thing at the beginning of the game, and missing it means you can never find every Surprise Box and complete that (otherwise meaningless) side mission.

Something I felt that should be added, in Fallout 3 when you witness your Dad succumb to the radiation inside Project Purity, you can manipulate the camera to loot him and Autumn for their unique weapons and armor. Doing this actually allows Autumn (who I’ll never understand how he survived that) to carry a second unique weapon during the last mission of the main game, which is semi pointless since the game ends right after that. (not counting the Broken Steel DLC that was added later that lets you keep going post game)

    • @GearShotgun

Yeah that confused me too, so I looked it up. Apparently Autumn survived by injecting himself with some kind of serum or anti-radiation medicine before passing out, possibly a more advanced or powerful form of Rad-X or RadAway developed by the Enclave. Its easy to miss since most people are more focused on their character's dad than Autumn, and nobody in the story mentions or calls attention to it either.

Barret's Ultimate Weapon in original FF7 was my first time experience this kind of situation I had to bail on saving and replay a late section again because if Barret isnt with you then the item box doesnt appear and you never get to return to the area, this also forces you to use Barret for a certain few bosses as a fight is right after his weapon box appearance

Yes, all well and good if you've saved such that you can reload an earlier one but it's incredibly easy to miss because as was pointed out, the chest isn't there unless you've got him. Cyberpunk 2077: If you wanted to do a Katana-build you'd better make sure you double check everything in the intro-mission where you infiltrate the Arasaka company. When you get to the top floor where you need to be you get to explore a big room, but you can't go outside on the balcony because the glass doors are shut. After you get what you need and watching a pretty long event the alarm goes off and you have to get out of there quickly while the game points you to the escape route and gives you the illusion that enemies are coming to you. What it doesn't tell you is that those glass doors to the balcony on the other side of the room are now suddenly unlocked and you can open them. If you go through them and up some stairs outside you get on top of the roof where an helicopter is with a katana inside. This katana is one of the strongest, if not THE strongest katana you can get in the game and if you miss it in this moment you'll won't be able to get it again unless doing a New Game. For the next 20-30 hours you won't get a katana even nearly as strong and people probably googled where to find a better katana after about 10 hours only to find out they missed it and have to play all over again if the want it.

I feel like Nahkriin is kind of a poor inclusion on this list when you consider the fact that killing Nahkriin is a soft requirement of entering Sovngarde and continuing the quest, as most players likely aren't going to rush up and stop Nahkriin from disabling the portal, which then requires you to kill and loot Nahkriin to re-open it. So while it's rare in the fact you only get one chance at it, basically everyone will be able to easily acquire it unless they either rush past Nahkriin to jump into the portal before they close it, don't do the main questline, or deliberately choose not to loot the mask when looting the staff. A better example would be the unique Dragon Priest Staff that Nahkriin uses and that serves as the key to Sovngarde's portal, as the only way to get and keep it is taking extra steps to kill Nakhriin right as he takes the staff so you can loot his corpse and jump into the portal before it fully closes (as the portal remains active for a couple seconds after the staff is removed).

You have a 1/128 chance to find one after each Starman Super you defeat, but they only appear in one place in the game, and they vanish once you've cleared it. Technically, the American manual tells you about it, but as much as people ignore manuals (like RUclipsrs embarrassingly calling Duck Hunt's controllable ducks a "secret"), you know *MANY* ppl never even know he can do better than just bare hands.

Star Ocean 2 The Second Story is a great classic title that belongs on this list. The entire game is designed around how specific choices you make that are as minor as speaking to a specific villager or party member at a specific time, can alter who joins your party, and what sort of items you can get. Much like how pokemon have various natures, your character can actually start with background skills that effect what combat skills or even support skills you can easily learn. My favorite one, and I may have some details wrong after all these years, is the ability to pickpocket. 95% of the time you will only get trash and basic items, maybe a few coins, but there is some of the most powerful armor in the game you can get only if you pickpocket people in once in a game timeline chance as well as all sorts of random unique items, some of which are pretty awesome.
It involves starting off with the skill dexterity. if you dont have it, restart the game till you do. Then farming lots of cash so you can afford a super expensive item thats actually skipping ahead of progression to visit the town its in because if you follow the story till you get sent there, you miss your chance. Thats how nuts it gets. And your pickpocket chance can FAIL too, so you have to grind it up to max level so it doesnt before you go for the items. It takes a lot of time and effort to do but the payoff is pretty nuts because as i said, while most of the loot is trash, some of it is unique and very powerful. There are entire guides out there spelling out how to do it all. And thats just one of the many, MANY secrets and hidden things you can do or unlock in that game.

When you teleport to Mournhold there is a quest that takes you to some huge abandoned Dwemer ruin and there was something there you can find... but once that mission ends you can't go back to get the good stuff (been so long now I cant remember what it was).
Skyrim also has a mission where you infiltrate and become a spy... it is a once only mission with no return as well.
Skyrim also has that mission where you are pulled way into the sky and given something by one of the gods....
    Several like that...

The game wants you to run away, but if you stay and manage to defeat him (if he blows up and you are still in the fight, I believe you die), he will give the Trader Card, which essentially forces rare drops from enemies when equipped. Highly, highly useful.
