Last March blood tests suggested my Vit D level was 44, not terrible but not "high" despite my diligently consuming 5000 ius daily (or more) since covid.
In April, I got serious about my diet and supplements and "absorption issues" even though I had no idea what issues I might have. Went gluten-free (still 98% gluten-free, vacations have thrown me off some,) sugar-free (now just way reduced sugar,) artificial sweetener-free; started taking a probiotic, digestive enzymes; started taking cod liver oil with my Vit D. And, I don't avoid the sun, though we're told that as Oregonians (and I work inside,) we will never get enough Vit D just from sun exposure.
Two weeks ago I saw an endocrinologist for the first time ever, for my raging osteoporosis. (I decided I have to do meds.) He took some blood, not fasting, said everything looked good except in his opinion I'm hypothyroid (what? I usually can tell...ack, not sure I want to up my thyroid meds, my PVCs are already out of control, maybe the two are not connected but they started when I started taking thyroid medication, so....) But also, my Vit D level was 107!!
Guess I'm absorbing it now! To what might be attribute this? Is it the cod liver oil? Am I improving absorption generally? Is it seasonal? Anyway, I won't be taking as much if it now.
😅
Just looked up Vit D in cod liver oil...guess it has quite a bit on its own. I'm probably more evidence that doing this on your own, not knowing what you're shooting at, isn't wise. But when you find out you have severe osteoporosis and it's upending your life (due to pain) you...want to DO things, you feel like you've already waited too long...so, I've been lighting fires.
Edit again: I think I'm being tested for hyperparathyroidism (24 hour urine, haven't submitted yet.) Blood tests for same suggested all is well but of course I've read that this can be deceiving. I really WANT there to be some (fixable) reason for my osteoporosis, and it does sound like Hashimotos and hyperparathyroidism together isn't that uncommon, also symptoms seem nearly identical. I know I just need to wait and see but now I'm wondering about that Vit D level and how it might affect the 24 hour urine test. The nurse or whoever talked to me yesterday didn't seem too concerned but after decades of being a bit laissez Faire myself, I feel like it might be time to start paying better attention. 😮

I have recently been diagnosed with osteoporosis, with the DEXA scan showing weakness in all three areas. I have all the risk factors - family history, age 66, female Caucasian, lack of natural vitamin D as I live in Scotland, also I am slim build. I did think my regular ballet classes (5-6 hours a week) plus a very rarely sedentary lifestyle might have helped prevent but it has not. My doctor has prescribed Alendronic Acid, along with a vit/ca supplement. I intend to balance my mineral and vitamin intake better now, along with your advice. However, I would value your views on the use of Alendronic acid? I am very hesitant to use this as I have read very mixed views.

@OptimalBoneHealth Hi & thank you for your informative video, Doctor! My question is: Is the Master Class free for people that are not living in the US? I live in Sweden 🇸🇪! Thank you in advance for answering my question 🙏🏼🙂
I had to research myself on the Vitamin K.. All the doctors through the years NEVER mentioned it. My recent appointment with my Gyn never mentioned it. I told her I was taking it and she agreed. DUH!
HI Kathy,
That's not surprising. We are taught to fear Vitamin K as physicians because of fear of increased risks of clots but as I discussed unless you are on coumadin/Warfarin that risk has not been proven in research. Dogma dies hard though and this fear will likely persist for decades to come.
Very informative and understandable. Thank you.

Hi Michelle,
My preferences are changing frequently as I haven't found the best version yet! My current go to is OsteoPrev from OrthoMolecular.
Excellent!
May I propose to add one supplement to the equation?
It is Boron, which is a great supplement for bones. It regulates calcium, magnesium and phosphor.
Next to this, it supports hydroxylation and this increases free vitamin D3, regulates testosterone, estrogen, and helps your brain. It is anti-inflammatory, and anti-mold. May help sleeping better.
*Very good to treat osteoporosis, when combined with vit D3, K2(!!), magnesium, silica (e.g. as orthosilicic acid.

Thanks so much for all your help Dr! At present I am taking a Canadian supplement called "Pro Osteo" by the company Naka. Here is the list of ingredients. In this order. Hydrolyzed collagen-Type 1 grass fed bovine, ELEMENTAL calcium ( from calcium citrate ) 804mg, ELEMENTAL magnesium ( from magnesium citrate ) 404mg, ELEMENTAL zinc ( from zinc citrate ) 10mg, Elemental silicone ( from silicone dioxide )74mg, ELEMENTAL boron ( from boron citrate ) 540mcg. Vitamin D3 (cholecalciferol ) 1000IU 25mcg. It is in liquid form. What do you think?
Hello,
I like the combination and form of these supplements. The amount of Ca required in supplement form will depend on the amount supplied by diet. We test magnesium to verify how much needs to come from supplementation. The zinc is okay but also something we measure. The silicone, boron and D3 levels are all pretty low. The D3 requirement will also depend on testing. In summary, it's good but I'd want to know more before recommending this as a primary tool.
Greetings!! from the Uk. Great! advice. Thank you. What is your opinion on testing vitamin levels? Blood testing or strand of hair? I do not remember where I got this information from, but I read that hair strand testing is more accurate and picks up more vitamin deficencies, than the blood testing. I have osteopenia, abrasions of the stomach lining, arthritis of both knees and fibromyalgia. Loved this video by the way. I take purolabs, vitamin d3 3,000 iu & k2, (mk7) 100ug. What is your opinion on the the k2? And the dosage of d3? Thank you so much for this information, when it comes to vitamins, it certainly is a minefield out there. 🙂
@@lizrigby-jones3700 Hi Liz,
Hair testing is a form of functional testing that looks at metabolites. This form of testing and other functional tests can give detailed information on nutrient levels over time. Blood testing gives current levels of specific nutrients which can be great for some like Vit D but not others. We use a combination of both but lean on blood testing for the primary nutrients. Our preferred functional testing modality is through urine. We use a test called Nutreval in the states. I don't believe it is available internationally. K2 I recommend to my patients as MK-7 and there is no clear evidence to support dosing. One product we use as 300mcg in a serving. D3 is highly dependent on genetics, diet and more. Some patients get by with as little as 2,000iu daily and others need over 10,000. That's why testing is critical!

When talking about Vit D levels on a global video platform, please always mention the units. A factor of 2.5 between the two common units is something that people need to factor in.

Sun bath in the morning and evening is a bit unspecific. There was a whole study made on vitamin D production based on the colour of your skin to the time of the day and it was found that you need to be in the sun between 10:00 am and 3:00 pm. A white skinned person can get enough Vitamin D in 15 - 20 mins, and on the darker shades it can go up to 45 mins to 1 Hour.

Thank you for this information- I have osteoporosis, many bone spurs along my spine and some disc degeneration- I take Vit D3 (Cholecalciferol) 1000 I.U. , I cannot take calcium and cannot take dairy products- IBS. What could help my bones that are sore but at the same time not progress my bone spurs (osteophytes) on my spine.
Hi Marika,
Sounds like you've got a lot to consider. I can't give specific recommendations through this platform BUT let me mention some generalities you may benefit from.
First, bone spurs are a sign of osteoARTHRITIS not osteoPOROSIS. They can be pain producing but often are not. Improving your bone quality should have no negative impact on these bone spurs.
For us anyone that presents with IBS gets a full gut workup to see why they have symptoms. While I believe IBS exists it is often a diagnosis delivered by providers who don't know where else to look. gut dysfunction can strongly contribute to poor bone health and poor health overall.
Consider our free masterclass for more information on tools you can apply!

Hello,
Great and challenging question. The human body is amazing and is designed to carry and deliver beautiful new humans to the world. Bone loss is a part of that process in general but the post delivery time frame is filled with hormonal changes that reverse the losses for most. Oxytocin and estrogen play pivotal roles here. Osteoporosis should not be a contraindication for pregnancy in most women. Discuss this with your team for recommendations on your unique scenario.

Hi Margaret,
Take a look at our vide on calcium. You can find it if you search Dr Doug Lucas Calcium. It'll help clear that up. We have our patients take Vit K with Ca to help with appropriate movement of Ca after absorption.
Shouldn't it be K2 instead of K1, as K1 directs the calcium to the arteries, causing calcification, but K2 directs it to the bones? @@Dr_DougLucas
@@wnhlwntk I don’t believe that’s true, no. If that were true people who eat lots of plants would have lots of artery calcification which they tend to not have.

Brilliant videos, can vitamin d, k2 and calcium be taken at the same time, someone said that D & K should be taken a few hours apart, and that D should be divided and taken at morning and lunch, not later.
It's a bit confusing. Will do your master class. Thank you very much.
Hello,
Yes, there is discrepancy and concern about stacking all of these together. I understand the utility of trying to maximize absorption and prevent conflicting supplementation. However, realistically, clinically creating a plan that requires supplementation to be taken every few hours isn't practical. We stack Vit D, K and calcium together with great results. Could we do more with less? Maybe, but I can't schedule in multiple supplements throughout the day so I don't ask my patients to.

How important is vitamin C for bone health / osteoporosis management... and at what dosage?
Hi Joseph,
When it comes to maintaining strong and healthy bones, it's not just about consuming enough calcium. Vitamin C is also crucial in the equation. This essential nutrient plays an important role in producing collagen, which is a vital component of our bones. Collagen helps to give our bones their structure and strength, and without enough vitamin C in our diets, our bones can become weak and brittle. Additionally, vitamin C is a powerful antioxidant that helps to fight off free radicals that can damage bone cells. So, if you want to keep your bones healthy and strong, make sure you're getting enough vitamin C in your diet from sources such as citrus fruits, broccoli, and strawberries. I recommend between 1,000 and 3,000mg per day depending on their unique situation.
@@Dr_DougLucas thank you for your detailed response
I'm enjoying your videos very much - very informative. I've had an interest in and been reading about vitamin K2 for some time now. My impression has been that it has nothing to do with coagulation at all - that's the role of vitamin K1. You seem to have a different take - can you expand on that or steer me to info you base that on? Also, do you see many patients whose cause of osteoporosis is hyperparathyroidism? That might make a good topic - maybe you have one already.
We have a video coming out on this topic this week! Make sure you are subscribed and signed up for notifications so you see it post.
I do 1 1\2 hours exercising (walking) in the sun 3 to 4 days a week - I live in Ghana - sun all year. My question is “will my vitamin/hormone D be sufficient?
Hi Carol,
I have found there is too much variability to know. Testing is critical for Vit D.
With all the synthetic/not bioavailable vitamins, that are "fortifying" our food supply, how is it possible to get adequate vitamins from food? My sister and I grew up eating tons of dairy, especially milk, cheese, and ice cream. We ate them continuously .We are in our 50's, and both have osteopenia/osteoporosis. Our mom went through menopause at 28, due to health issues. Her doctor had her on 500mg calcium carbonate, twice a day. Now knowing what I know about that, it's no wonder she had a quadruple bypass at 50, and broke her hip at 58.
I have 2 questions: first, is osteoporosis hereditary, and what are your thoughts on Osteo Bi-Flex?
Btw, Thank you so much for enlightening us, with what you do. I have learned so much so far!
Hello! So many things to unpack there. We have a dairy video coming so look for that. May have been a contributor to your bone loss.

Thank you for this video, Dr. Lucas. I am currently taking Calcium Bone Maker Complex with MCHC with k2mk7 for my osteoporosis. What do you think of this supplement? I would appreciate to hear from you about it. And also is it better to take this supplement before bed time for better absorption? Please advise.
Hi Pauline,
Wow! That's a lot of calcium in a supplement! How much you need will depend on your dietary intake. We tell our patients to aim for 1,200 - 1,500mg calcium per day total. This supplement would get them there all by itself. It does have some D and K2 in it but not much of either. I would not consider this for my patients it is too high in calcium and may put them at risk for side effects from the calcium content imbalanced with adequate D3 and K2.
@@Dr_DougLucas Thank you for your reply. In addition i am also supplementing with D3 5000 iu and K2 mk7 100 mcg. Is it ok to continue to use the MCHC calcium that I am current taking it? If no, what kind of MCHC would you recommend?
What do you think of Annatto -GG 150? I watched another youtube saying this is also good for the bone health. I am also taking it, is it too much bone supplement for one person to take?
My T score didn't change, it remind at -2.5 and I just want to improve it. Thank you again.
@@paulineyu4472 Annatto seems to be an extract of Natto. I'm not sure how much of the beneficial K2-MK7 is maintained in this approach. I don't see any evidence of it's benefits. Regarding which product to use it depends on individual circumstances. I can't make specific recs through this platform.
Did the supplement work ? Did it improve your dexa scan? I hope it did

Thank you so much do your informative videos- really appreciate them! I am on the bone saver program which uses New Chapter calcium pills. This calcium comes from organic algae. Is this as good a form of calcium as the one you recommend? There is also strontium and vit k 2-7, and vanadium. No boron though. Your thoughts on this supplement will be greatly appreciated. Thank you so much!
Thank you so much for watching! I actually have a video coming out soon about calcium from algae - stay tuned!
Question for the doctor: I read that 30 minutes of sun exposure can give you 10,000 to 20,000 IUs of vitamin D. If this is the case what would be the down side of taking 5000 to 10,000 IUs daily? Didn’t our ancestors who lived outside get lots of vitamin D from the sun?
I don’t know if it’s that much but there is definitely value in sun exposure! Most civilizations started on the equator and indeed got a lot of sun. Our current culture though is much different. I’m a fan of sun exposure for Vit D and A conversion!
I would like to know what kind of doctor to see about this. I saw an endocrinologist and she wanted me to take Actonel or Prolia. I have heard that taking those kinds of meds are not good, that they can cause severe fractures because of how they work. Also, they can cause jaw problems and my dentist recommended that if I did take anything like those drugs that I should take a weekly pill and not the infusion every 6 months. The endocrinologist didn't offer any alternatives to those drugs. Thank you.

Thank you for all the great knowledgeable information you share. I have checked many hydroxapatite MCHC calcium supplements and they all have different formulations and ingredients.
Would you please tell me the one that you think is best and by which company?
We use multiple brands to fit various needs and unique circumstances. Some of these include metagenics, Jarrow and Orthomolecular. We find that most of them have too much calcium and are not balanced in a way that fits our patients needs.
Great video

What about Collagen? And Strontium; wasn't it found to be causing more damage than benefit?
Hello! I haven’t looked at literature on collagen yet. Many of our patients love it but it’s on my list to dig into.
I have a video discussing strontium!!!! Check it out and let me know if you still have questions.

Thank you for all your information and good advice. Is it true that you need to take calcium citrate if you take a PPI everyday? I will be signing up for your master class.
Hi Kiki,
Better question is why are you taking a PPI in the first place? We work with out patients to get off of them when possible. There are legitimate reasons to take one but they will increase fracture risk. To answer if you have to take calcium or not depends on what you get through diet. I'll see you in the masterclass!

I started taking K2Mk7 in September 100mg. No issues. But on Dec 31, I started getting very itchy only to find a severe rash on my body. The next day, dizziness when I got up. I decided to stop it when I realized the symptoms were worse after dinner when I took it. It took 3 weeks for the symptoms to go away. Not sure what I am allergic to and want to try again but it was scary so I’m leery.
Thank you for sharing your experience with this!
Thank you so much for the information on supplements. Doctors tell you to take calcium and Vitamin D etc. but never explain what to look for in the composition.
Glad you enjoyed this video!
Thank you very much, very useful video. At what time of the day we can take this combination of supplements? For example, with the meal?
Great question. We generally will recommend that people take supplements with food. There are a few things that do better on an empty stomach like probiotics and certain peptides. Most supplements are absorbed better with food, and generally, my patients will consume their first dose of supplements, with their first meal.

I have been taking 100 mcg of Vit K2 as MK7 and 5,000 IU of D3 daily. it is a combination supplement. Do you think i should take another supplement that is 100 mcg of K2 at a different time of the day to get to 200 mcg? Can you get too much Vit K2, or does body excrete extra? i get a blood panel every year. Is Vit k2 measurable?
Hi Candie,
Vitamin K2 is not measurable through blood tests. There are functional tests that look at indirect measures of vitamin K but it is short lived in blood so testing is difficult.
I'm not aware of an upper limit of safety. The most I've seen in a supplement is 300mcg of K2 as MK7. This is what I'm recommending to most of my patients with bone health concerns.
Love your videos! I have a question about taking coral calcium, specifically a brand (VitaSupreme) that says they harvest from more eco-sustainable areas in the Caribbean instead of Okinawa. Have you come across any studies or have an opinion on if coral calcium is better than the calcium citrate/carbonate that is typically found in the health food stores?

Coral calcium is mainly calcium carbonate, the least absorbable form of calcium.

Thank you so much for doing this very informative video. I presume it’s ok to take Asprin with the vitamins you mentioned?
You would have to ask your team. I don’t like aspirin in general for mine but we would look at the combined risk and make a decision.
Background. I’m a 70 yr old woman. Suffered a tri malleolar ankle break with dislocation in April
of 2023. I was told to take calcium. Nothing more. Subsequently diagnosed with osteoporosis, prescribed Alendronate, researched it and am reluctantly taking it. Now into my third month with only four weeks to hardware removal surgery. I believe I can correct my bone health through the supplements you suggest After much research I find you to be most informative. My question. Should I continue my supplements with the addition of those missing now and continue on Alendronate until after surgery? Thanks in advance!
You’ll have to check with your team. As a surgeon I had my patients continue on bisphosphonates but that is controversial. If I were to do it now I probably wouldn’t but it is going to depend on the case. The supplement question will depend on dietary calcium intake.

I have been told by my former doctor that here in Canada they don’t do vitamin d testing anymore. She said she would look into it years ago but I never heard back about it. Needless to say my former dr is pretty useless for me and all of her patients so I fired her as my doctor and I’m on a very long waiting list to get another. We have a huge shortage of family doctors here in Canada. We don’t pay anything to see one and that’s great, but it’s almost impossible to get a family physician right now.
The other thing is I’m a vegan and that will never change, it’s an ethical thing. I do take a vegan vitamin D3.
Hi Jacqueline,
I'm sorry to hear of your troubles with your doctor. No medical system is perfect and there are weaknesses to both the US and Canadian systems. Perhaps you could explore companies that allow DIY testing and take some things into your own hands.
Eating adequate protein on a vegan diet can be difficulty. I tell my patients to aim for 1g/lb of body weight. If that protein is coming from plant sources exclusively it will need to be more.
Yup, I'm in Canada too. Same messaging around the vitamin D testing😢
Testing for vitamin D depends on the province. In Ontario you can get tested for it but you need to pay about $30 for the test.
Here in the UK they don't really test vitamin D and if you do get it done they won't repeat until a year later if lucky. I had serious spiral fracture that was taking long to heal. Im post menopausal. Did private rest through the post. B12 active ( again NHS don't test this)& vitamin D cost me about £40 I had to take the blood myself lol.
@@1STBUCKLEY Every system has its issues. It’s an international problem. Resources are different everywhere but optimizing health requires an investment of time, energy and usually finances. We cannot allow the system to dictate our quality of health regardless of where we live. Taking your own blood is another level of commitment. I’ve done it too but I wouldn’t ask my patients too!

What is the best brands of calcium to take? I have osteoporosis and I don't want to take a substandard product. I presently take a calcium carbonate. So from your video I find out its not the best. Thank you for your videos.
Hello,
First, congratulations on being proactive in your health. That's a big step in the right direction. I can't give personal advice through this avenue BUT I can tell you the form of calcium that we recommend for patients
MCHC (Microcrystalline Hydroxyapatite Complex) is calcium in the form that your bone stores it in. Evidence support better improvement in with this form than others. There are several brands that use this form.
Remember, that it takes more than calcium supplementation alone to improve bone health. We use a comprehensive approach optimizing as many nutrients as we can through diet and supplementation. We then take a deep look at what other tools are available to us based on each patients unique story.
I hope that helps!
As a retired PhD biochemist, I watched this on behalf of my wife. I've been taking K-2 MK-7 for 2 years now. Not for osteoporosis, obviously, but because I was getting huge red-purple bruises on my ancient skin every time I bumped into anything. If I gently bumped into a shelf while walking through the supermarket, I would get an instant large bruise. It always took about 2 weeks for the bruises to fade away. At any given time, I would have 4 - 6 major ugly bruise spots on my arms. After a bit of literature review, I concluded that K-2 might help. I also learned that whereas K-2 persists in the body only for a matter of hours, the MK-7 version persists for about 3 days. Within a few days after starting on it, I have literally never got a purple bruise again. If I bash my arm hard against a sharp object I might get a faint pink bruise right at the area of impact, but it goes away after 2-3 days.
Thank you for sharing this!!

Do you know anything about the chemical structure of estrogen vs photoestrogens found in flax seed? I am taking anastrazole which is supposed to remove estrogen from my body. (Which also removes calcium from my bone so that is why I am on this channel). I am trying to find out if flax seed is still okay to take. Some science studies say no… don’t take it, others say yes do take it because what I read was the phytoestrogen adheres to the estrogen receptors of cells in place of regular estrogen and can block estrogen from attaching. But…. My question is, does the phytoestrogen behave exactly like the estrogen and defeat the purpose? My cells are supposed to be starved of estrogen (I have cancer). Would the phytoestrogen from flax seed behave the same as estrogen or differently? Any tips? Or any reading sources you could point me to? Some studies say the phytoestrogen found in flax seed protect the cells from estrogen. So confusing when studies are found to say the opposite of each other.
@@sheila7814 Since phytoestrogens have the same function and activity as mammalian estrogen, if you are supposed to minimize the effects of estrogen, then that would also apply to phytoestrogens. In Asia, boy children are often told not to eat soy products (which contain phytoestrogens) because it gives them feminine characteristics, including man-boobs. Flax seeds, of course, contain phytoestrogens. If you want the flax seed for its omega-3, why not just take the fish oil extract?
