Do not confuse or complecat keep it simple encourage everyone to be self organised there is no harm rather thinking too much and dilute.

What is prescription?
Difficult to give, having said one needs to find ways to build trust, respect and openness in the team. The action of leader should promote this and whenever things goes wrong, the leader can use it as an opportunity to build these values.
How your experience has been on this?
I always prefer to explain/answer based on my experience. But last 6 months shows that majority public is looking for bookish answer or something they like personally.
I think trends are changing or I'm lagging to catch the nerve of interviewers.
Also it looks like many interviewers lacks skill of conducting interview.
I have 1 humble request, put 1 session on how to conduct interview.
Its a good idea and we look forward for people for same.
unfortunately we do not find people wanting to learn how to take interview.
Saket, kindly answer what we should do when we give responsibility and freedom, but team member commits mistakes again and again though unknowingly
Difficult question since we need to understand what is happening with the member. It might be lack of skill and person needs to be trainer and mentored on doing the job first.
