The Heilwald Klinikum used to be a super-renowned medical facility until Joseph Randalph showed up and took over, intending to revolutionize healthcare with new treatments and emphasizing that they would basically do everything in their power to prevent a patient from dying, hinting very early on with his ominous phrasing that they were more than willing to step into unethical territory to do so. Whatever he did to ensure that no one died at the clinic appears to have distorted reality within the clinic, making you and other inhabitants immortal but also warping the staff into monsters/giving them odd abilities and seemingly amplifying the worst of their traits (that's what the puzzle with the statues was about, identifying how each staff member betrayed the code of ethics they were supposed to follow as caregivers to the ill.) The loophole is why the patients appear unable to die despite being more or less corpses and constantly being neglected by the staff, living in squalor with nothing to eat but either garbage or the literal flesh of the cook. Astrid, the former nurse who was the "monster" in the iron lung, was one of the only people to directly speak up against Randalph's unorthodox and harmful medical practices and she likely ended up the way she did in an attempt to silence her, experimented on in gruesome ways and then locked away where the rest of the staff wouldn't see her again, hence why they thought she'd left the clinic entirely. Helene appears to be the only member of the staff entirely unaffected by everything going on, probably because after Astrid disappeared, Helene was too busy to do anything but care for her patients, but it may also be simply because she was too steadfast in her commitment to the code to be affected like the other staff were. While the nature of the loophole and the voice from the plugged holes are a little less clear, it's most likely that Randalph either created/discovered the voice which somehow aided in his experiments or - the more likely option to me - the loophole and the voice from the hole are mostly separate things, and the voice in the hole, essentially the consciousness of the clinic itself, used to guide the staff and practitioners and earned the clinic its old prestige, which is why the voice admonishes the current staff for all of the ethical/code violations they've made. Once Randalph started his awful experiments, he plugged up the holes to prevent the voice, the clinic itself, from interfering with his work. I don't think the how or why of where the loophole itself even came from is ever actually explained, though, so these are just theories.

Electroconvulsive therapy was invented as an alternative to the existing insulin overdose therapy. Insulin dosing caused a seizure, which seemed to treat depression and delirium, among other things. Being shocked could also cause seizures, so an experiment was performed on a man with severe schizophrenia who hadn't been lucid or really able to communicate in years. They set him up, slowly cranked up the voltage, and let him seize for a little bit. The result was the patient more or less opening his eyes and asking "What are you sons of bitches doing to me?" He remained lucid for several weeks before reverting. These days, a general anesthetic is used to prevent pain and muscle strain from the seizure. I had twelve sessions a few years ago for my treatment-resistive depression. There was never any pain from the procedure - I literally slept through it every time. Though it failed to help me, I still recommend it for its intended uses. It's not fun, but it's not torture by any means.

Also no water for the costipated guy. 48% completion means you missed a lot, though. I definitely would like to see a second playthrough.
About the statue and ominous voice: Spoilers! The white statue most likely represents Hippocrates, a greek physician, who created ethical guidelines for people who practice medicine or healthcare. To this day, physicians take a form of the hippocratic oath to ensure that they always remember the importance of their tasks and that their goal is the wellbeing of patients. Some of the guidelines we saw during the numbers puzzle. Hippocrates is depicted as an old man with a beard. You see his Statue everywhere, because the Hospital is supposed to act according the oath and he watches over everything. But in the final part, the statue is gone and instead we see a statue of Randolph. While the others went against some parts of the oath, Randolph is the main issue, because he put himself above the oath. The ominous voice might be the previous clinic chef. His voice got suppressed. Hence the many plugs. Those would be relatively easy to loose with some outside help but Randolph added an extra layer to the seal. His influence enforces everyone around him in their negative behavior.
I don't understand the ending of this game at all because at the start the doctor said nobody can die but the trash collector can die & at the end the doctor dies how does that make any sense oh yeah & what if somebody in this games world got their head blown or cut off or they were split or cut in half would they die
