I wonder if the initial issue was due to some kind of contamination from the wood that you found.
Maybe agricultural runoff that later caused the ammonia spike too.

I think Limnophelia sometimes melts when put into a new aquarium. I also had one in my Blackwater aquarium and it melted aswell. And woah it stank lol
