It appears that the lower hydraulic cylinder broke off from the boom. It didn't have anything to do with working on energized lines. Sorry for your loss.
Lineman for 24 years. Half of my injuries are due to equipment. Back when bucket trucks used chain to extend the bucket. I watched a co worker slide a full 20’ back into the beam and fling him out of it. Had his harness on but that did nothing. We’ve all retired on disability. Crazy thing is today fast food workers make half of what we do or did that took risk and skill working in all sorts of weather, day or night. $20 an hour for serving fries is an insult to anyone in the trades, medical or first responders making just double that.
I don't know anyone making $20 in a fast food join unless it's the store mgr, mcdonalds only pays 13-15 from "shift leaders" lol. Pathetic you would care what they make really like they don't have responsibilities in life, sure alot are teens but I see people I worked with back in the 90s still flipping burgers and making fries and they don't make $15 an hour for it. Food service is horrid, you get people that think they are better than a fry cook coming in hangry demanding it be their way like you work at burger king where everyone with a dollar is a king lol. Dude you should hope fry cooks make $20 an hour then maybe they won't spit on your food or handle it after taking out the trash, back in the 90s I was doin good to earn $5.5 an hour lol, at that wage if you were rude than I wouldn't want to be the person eating the food I threw together lol, I'm sure you get that alot. I'd prob put a gun in my mouth before working food service again, thank God I've never had to consider it, I get to sit at home and make over $20 an hour without having to risk any injury and I can chain smoke lol.

Hydraulics can and do fail, sometimes with an fluid leak and a slow loss of hydraulic power, other times a total failure. We were trained to never walk under the garbage truck back door when unloading due to the possibility of a hydraulic failure and the door slamming closed. It's been known to chop drivers in half.
My first thought was equipment failure and explosion. Now it seems it was a hydraulic failure of the bucket truck and the bucket slammed him into the ground. What a tragedy.
When I saw the headline, I assumed it was electrical in nature, a company I worked for had the misfortune of having to test a few boom trucks that had been involved in fatal incidents so I've seen what happens when they contact wires etc, but not once did I ever have any concern about the hydraulics, wow
It appears that the lower hydraulic cylinder separated, either the cylinder end cap failed or the big nut on the end of the ram that holds the seal plate came off or the end of the hydraulic ram broke off. Dangerous job!!
Had the boom extended to far for the weight in the bucket, and down it went. Sad but that's why they put stickers all over those things stating weight and reach limits.
