For medical assistance, the documents needed include a photocopy of prescription, original medical certificate, valid ID, and hospital bill. On the other hand, clients asking for funeral assistance must bring the original documents such as registered death certificate, funeral contract, and valid ID
How to apply for DSWD medical & financial assistance?
The form must be accompanied by a death certificate, proof of indigence, and a funeral assistance request letter. Once the form is complete, you will need to submit it to the nearest DSWD office. A social worker will then review your case and determine whether or not you are eligible for assistance
What do you need for medical assistance in DSWD?
DSWD/CSWD: Department of Social Welfare and Development
Clinical Abstract / Medical Certificate duly signed by a physician with license number (3 Months Validity)
Statement of Account or Prescription (for medications) or Laboratory requests (for laboratory tests)
To qualify for DSWD medical assistance, you must be a Filipino citizen who does not have the financial capacity to cover the costs of your medical treatment. If you are confined in a mental institution due to a mental illness, you may be eligible for medical cash assistance from the DSWD
